[Operative treatment of chondromalacia patellae (author's transl)].

Operative treatment of chondromalacia patella should be indicated especially in adolescents with great care. It can be clearly identified by a narrowing of the cartilage on the X-ray (tangential picture of the femoropatellar joint) and the presence of osteophytes on the margin of the joint. The following methods of operative treatment are recommended: Proximal decompressive operation by splitting the lateral retinaculum in light cases and the elevation of the tuberositas tibiae for severe chondromalacia and femoropatellar arthrosis. Follow-up examinations and the results are presented.

[Onset of arthrosis. Physiopathologic, nosologic and therapeutic perspectives].

The author has studied specimens of patellar articular cartilage obtained by drill biopsy during 45 operations for chondromalacia patellae. The samples were examined by electron microscopy. Thirty patients were suffering from early chondromalacia and 15 from long standing chondromalacia. The findings showed that the initial lesions were located in the superficial layer of the cartilage in areas subjected to high pressure. The author considers that a better knowledge of the evolution of the cartilaginous lesion can lead to better treatment, either medical or surgical.

Diagnosis and follow-up of children referred to a rheumatic disease unit.

Juvenile rheumatoid arthritis (JRA) has become the predominant rheumatic disease of childhood. In a survey of 292 children referred to a rheumatic disease unit over a 5-year period, 94 had JRA and only three had rheumatic fever. At follow-up, of the children with JRA, 70% were found to be in remission with little or no functional disability. Febrile onset of JRA or onset before six years of age were worse prognostic features than pauciarticular or polyarticular onset of occurrence after six years of age. Of 28 children who had either growing pains or psychogenic rheumatism, two were subsequent

Chondromalacia patellae.

Chondromalacia patellae is one of the most frequently encountered causes of knee pain in young people. The word chondromalacia is derived from the Greek words, chrondros, meaning cartilage and malakia, meaning softening. Hence chondromalacia patellae is a softening of the articular cartilage on the posterior surface of the patella. It may eventually lead to fibrillation, fissuring and erosion.

48, XXXY Klinefelter syndrome and nail-patella syndrome in the same child.

A patient is described who in addition to having the 48, XXXY Klinefelter syndrome has the autosomal dominant nail-patella syndrome, inherited through his mother from the grandfather. Clinical signs of both syndromes were found. Chromosomal investigation with BUDR incorporation and acridine orange staining showed that one X chromosome stained intensively, while the other X chromosomes were elongated and weakly stained. Difference in degree of stretching of the supernumerary chromosomes was noted. The occurrence of the two rare syndromes in the same patient is assumed to be fortuitous.

Congenital deformities about the knee.

Congenital deformities about the knee are the result of abnormalities of all the anatomical structures that make up this joint. In some cases the structural changes observed are due to primary disorders of formation and growth of one or more of the anatomical components of the knee. Disorders are known to occur in the integumentary, muscular, ligamentous supporting, and skeletal systems. Examples of these disorders are seen in the bone dysplasias and joint laxity syndromes. In other cases exaggerated intrauterine positions of the limb seem to bring about certain adaptive changes in the embryol

[Dislocation of patella also a symptom of other diseases or consequence of local primary alteration of the scelet (author's transl)].

According to a large group of patients there is demonstrated the "symptomatic" dislocation of patella. It is a part of a system-disease, especially Onycho-osteo-arthro-dysplasia. Arthrogryposis multiplex. Freeman-Sheldon-syndrome, mongolism and children with debility or imbecility. There are discussed their pathogenesis and any consequences for diagnosis and treatment.

Chondrodysplasia punctata--rhizomelic form. Pathologic and radiologic studies of three infants.

Pathologic, ultrastructural and radiologic studies are described on 3 infants with the rhizomelic form of chondrodysplasia punctata. Radiologic criteria in the young infant include radiolucent coronal clefts dividing all or most of the thoracic and lumbar vertebral bodies, short humeri with flared metaphyses and punctate calcifications commonly present adjacent to the ossified ischial and pubic bones and less commonly in other locations. In late infancy and childhood the radiologic criteria include demineralization in all bones with slow maturation, flat vertebral bodies, short humeri and femo

Chondromalacia Patellae. A prospective study.

In a prospective study of 105 arthrotomies for chondromalacia patellae, patella alta or an increased Q angle was found in most of the knees and was considered to be the usual cause of chondromalacia. The chondromalacia on the patella was usually centrally located with extension equally onto the medial and lateral facets. Femoral changes were uncommon. Patellar realignment together with excision of chondromalacic cartilage gave a satisfactory result in 79 per cent of the eighty-seven knees that could be followed.

Patello-femoral joint mechanics and pathology. 2. Chondromalacia patellae.

Two distinct lesions affect the articular cartilage of the patella. Surface degeneration occurs particularly on the odd facet; it is age dependent, often present in youth and it becomes more frequent with increasing age. It probably does not occasion patello-femoral pain in youth, but may predispose to degenerative arthritis in that joint in later years and is regarded as a consequence of habitual disuse. The term "basal degeneration" is used to describe a lesion in which there is a fasciculation of collagen in the middle and deep zones of cartilage without, at first, affecting the surface. It

Sagittal hemipatellectomy in the treatment of chondromalacia patellae.

A study of the preoperative assessment, surgical technique and follow-up in 17 surgical procedures (16 patients) indicates that sagital hemipatellectomy is an acceptable alternative form of surgical treatment in patients with chronic painful chondromalacia patellae. The study series comprised 16 patient (mean age, 21 years) in whom the duration of symptoms averaged 5 years, 2 months. In all patients the procedure relieved the pain and permitted a return to their accustomed work and exercise routine within 3 to 6 months after operation. The duration of follow-up is still too short to permit eva

The Achard syndrome.

The Achard syndrome is a well-defined clinical entity consisting of widespread dysostoses and increased ligament laxity. The bony involvement appears most consistently in the tubular bones of the hands and feet, the mandible and the calvarium, but may also be noted elsewhere in the body. Hypermobility and subluxations of the joints, increased lateral excursion of the patellas and other findings reflect the increased ligament laxity.

A familial syndrome of aniridia and absence of the patella.

This is the first report of a syndrome of aniridia and aplasia of the patella. The origin of the defect arises spontaneously in the grandmother of the proband and must be assumed to be a de novo mutation. There is no associated chromosomal abnormality or overt biochemical or other somatic defect. This is an extremely rare mutation exhibiting a dominant autosomal form of inheritance. The gene for aniridia is a dominant gene whose penetrance here is 100%. The combination of aniridia and bone malformations in general is very rare. This linked defect probably represents one of the most rare of hum

Dysplasia of the quadriceps mechanism: hypoplasia of the vastus medialis muscle as related to the hypermobile patella syndrome.

Disturbance of the patellofemoral mechanism is the most common cause of dysfunction in the knee, especially in athletes of all age groups. This is primarily a congenital familial abnormality due to poor development in the vastus medialis. The patella develops in the line of maximum pull of this muscle group; in this congenital condition it develops in line with the stronger antagonist to the vastus medialis, the vastus lateralis muscle. Therefore, the position in the extremity that the patella assumes is the result of abnormal muscle development. This brings about other changes in the lower ex

Trisomy 8 syndrome.

Trisomy 8 syndrome, with or without mosaicism, is characterized by the following clinical signs and symptoms: mental retardation, deformed skull, prominent forehad, high-arched palate, low-set and/or dysplastic ears, long and slender trunk, reduced joint mobility, and deep plantar furrows. The deep furrow or longitudinal crease of the sole may be pathognomonic for the syndrome. It should be noted that it has been seen in other rare syndromes. It is recommended that a child with mental retardation and associated multiple anomalies should be subjected to a cytogenetic study in order to define th

Chondromalacia of the patella. Physical signs in relation to operative findings.

Cartilaginous changes on the patella are common and were demonstrated in 50 out of 100 patients who underwent operation for intraarticular disease of the knee joint. In order to clarify further the signs of this condition, the physical signs were related to the operative findings. Retropatellar crepitation was present in most cases, but cannot be called pathognomonic. Pain on grating and tenderness on palpation of the patella in fact occurred together only in cases with cartilaginous changes on the patella. On the other hand, cartilaginous changes were found in several cases without such signs
