Pediatric Patellar Injuries.

BACKGROUND: Pediatric patellar injuries are common causes of anterior knee pain in children and adolescents and are frequently encountered in emergency and acute care settings. These conditions range from load-related overuse syndromes to acute traumatic injuries requiring timely recognition and referral. Unique features of the skeletally immature knee, including evolving ossification centers, open physes, and cartilaginous structures, may complicate diagnosis and contribute to missed or delayed recognition. OBJECTIVE: This review provides an evidence-informed, emergency medicine-oriented app

Complex Genomic Rearrangement Involving the TBX4 Promoter Manifesting With Variable Expressivity in a Five-Generation Family.

Variants involving TBX4 are associated with ischiocoxopodopatellar syndrome with or without pulmonary arterial hypertension (ICPPS; also known as small patella syndrome), pulmonary arterial hypertension (PAH), and lethal lung developmental disorders. The variability of penetrance and expressivity of TBX4 variants remains a prominent challenge in understanding their genotype-phenotype correlations. We investigated a five-generation family with 12 affected individuals presenting with isolated ICPPS, lung-related manifestations with features of ICPPS, and other milder abnormalities. Whole-genome

IL-5 biology and eosinophil regulation: Advances, resistance pathways and future directions.

Interleukin-5 (IL-5) is a central regulator of eosinophil differentiation, maturation, trafficking, activation and survival, and therapeutic targeting of the IL-5/IL-5 receptor alpha pathway has substantially changed the management of severe eosinophilic asthma and other eosinophil-associated disorders. However, clinical responses to anti-IL-5 and anti-IL-5Rα biologics remain heterogeneous, and persistent symptoms may reflect ongoing eosinophilic inflammation, partial clinical response, comorbidity-driven residual disease or inflammatory mechanisms not primarily dependent on IL-5. This narrati

The preclinical discovery and development of depemokimab for severe eosinophilic asthma.

Severe eosinophilic asthma is driven by interleukin (IL)-5-mediated type 2 inflammation and frequently requires long-term biologic therapy. Depemokimab was designed to achieve sustained IL-5 neutralization with a twice-yearly dosing schedule, addressing key unmet needs related to treatment burden and adherence. This review summarizes the molecular design, preclinical pharmacology, model-informed drug development (MIDD) strategy, Phase I-III clinical trial program, and regulatory approvals of depemokimab in asthma and chronic rhinosinusitis with nasal polyps. Depemokimab exemplifies how rationa

Heterotopic Ossification: A Rare Complication of Lateral Condyle Humerus Fracture Treatment in Children.

Pictures certainly are worth a thousand words in the case of the structure of the connective tissue skeleton of normal and diseased myocardium. This report reviews the connective tissue matrix of the normal human myocardial tissue and the pathological myocardial fibrosis in left ventricular hypertrophy due to chronic arterial hypertension in humans and in human chronic chagasic myocarditis. The myocardial connective tissue matrix was studied employing a cell-maceration method that removes the myocardial tissue non-fibrous elements, and leaves behind a non-collapsed matrix, thus allowing a bett

Micronutrients in Autoimmune Diseases: Shining a Light on Vitamin D, Cobalamin, Folate, and Iron Metabolism.

Background: Autoimmune diseases (AIDs) are characterized by chronic inflammation and tissue damage resulting from abnormal immune responses. While genetic and environmental factors play significant roles in disease development, essential micronutrient deficiencies (MNDs) represent a critical and often overlooked contributor. Methods: This review examines the interactions between micronutrients and immune cells, focusing on vitamin D, vitamin B12, folate (FA), and iron, and their roles in AIDs, such as rheumatoid arthritis, autoimmune thyroid disorders, multiple sclerosis, systemic lupus erythe

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Evaluating the impact of changing inhaler color on perception of symptoms and disease burden in patients with asthma: the FEEL study.

OBJECTIVES: Patient perception of treatment effectiveness is key to optimizing adherence. This is potentially impacted by color, yet no such studies have been conducted in asthma. This study assessed the influence of pink vs. white pressurized metered-dose inhaler (pMDI) actuators on asthma symptoms perception. METHODS: In this double-blind, randomized, multicenter, crossover study, adults with moderate-to-severe asthma received extrafine formulation beclomethasone dipropionate/formoterol furoate (BDP/FF) pMDI for a 2-week run-in. During two, 2-week treatment periods they received BDP/FF pMDI

Development and evaluation of a new semi-quantitative and morphometric scoring system for magnetic resonance imaging in adolescents with Osgood Schlatter Disease (The OSIS Score).

BACKGROUND: Osgood Schlatter Disease (OSD) is a common injury in adolescents. A recent systematic review identified multiple tissue characteristics evaluated in imaging studies, but the studies used different imaging modalities, used varying MRI protocols and were of poor study quality, which led to conflicting findings and hamper the clinical utility of MRI scans. This study aimed to develop and evaluate the reliability of a semi-quantitative MRI scoring system for use in adolescents with OSD. MATERIALS AND METHODS: Based on a systematic review, we used an expert-led process to develop a sco
