The use of stereolithography for an unusual patellofemoral disorder.

A patient with habitual patellar dislocation which was not treated successfully by the Elmslie-Trillat procedure is described. In the knee, a unique morphologic feature of the patellofemoral joint was suspected as a cause, and a stereolithographic model was produced from the patient's computed tomography data to determine the pathologic features. Because the solid model confirmed the speculation, additional surgery was done to modify the geometry of the joint. In the surgery, cancellous bone was removed below the cartilage, and the geometry of the cartilage was modified with the subchondral bo

Measurement of patellar tracking: assessment and analysis of the literature.

Patellar tracking is defined as the motion of the patella relative to the femur or femoral groove on knee flexion and extension. Abnormalities of tracking (maltracking) are thought to relate to many disorders of the patellofemoral joint and may be defined easily or may be extremely difficult to observe. Accurate measurement of patellar tracking, and definition of normal tracking, have not been achieved yet in experimental conditions or in clinical conditions. Such information would be valuable in the diagnosis and treatment of patellofemoral disorders. In the current report, the literature is

Arthroscopic mediopatellar plicaectomy and lateral retinacular release in mechanical patellofemoral disorders.

Arthroscopy was performed on 168 knees of 164 patients with anterior knee pain by a single arthroscopic surgeon between April 1993 and March 2000, with a mean follow-up of29 months. There were 168 mediopatellar plicae, 16 infrapatellar plicae, 8 suprapatellar plicae, and 30 lateral plicae, and all plicae were excised. Lateral retinacular release was performed in 74 patients with patellar lateral compression syndrome, patellar lateralization, and patellar lateral subluxation through anterolateral portal without using a third portal with the help of a hook knife. Débridement and drilling were pe

Gender differences in patellofemoral joint biomechanics.

Patellofemoral pain is associated with patellar malalignment and quadriceps weakness which are seen more commonly in women. The objective of the current study was to determine the effects of gender, vastus medialis strength, and tibial rotation on patellofemoral joint biomechanics. Twelve fresh-frozen knees from cadavers were tested using a custom knee jig. Anatomic multiplane loading of the extensor mechanism was used with varying vastus medialis loads. Patellofemoral contact area and pressure were measured using pressure sensitive film at knee flexion angles of 0 degrees, 30 degrees, 60 degr

The physical therapist's approach to patellofemoral disorders.

Management of patellofemoral pain is no longer a conundrum if the therapist can determine the underlying causative factors and address those factors in treatment. It is imperative that the patient's symptoms are significantly reduced. This often is achieved by taping the patella, which not only decreases the pain but also promotes an earlier activation of the VMO and increases quadriceps torque. Management needs to include specific VMO training, gluteal-control work, stretching tight lateral structures, and appropriate advice regarding the foot, whether it is orthotics, training, or taping.

The effects of tibial rotation on the patellofemoral joint: assessment of the changes in in situ strain in the peripatellar retinaculum and the patellofemoral contact pressures and areas.

The objective of this study was to determine the effects of tibial rotation on in situ strain in the peripatellar retinaculum and patellofemoral contact pressures and areas. Patellofemoral joint biomechanics demonstrate a strong correlation with the etiology of patellofemoral disorders, such as chondromalacia, and are significantly influenced by tibial rotation. Six human cadaveric knees were used along with a patellofemoral joint testing jig that permits physiological loading of the knee extensor muscles. Patellofemoral contact pressures and areas were measured with a Fuji pressure-sensitive

Adaptive time-frequency analysis of knee joint vibroarthrographic signals for noninvasive screening of articular cartilage pathology.

Vibroarthrographic (VAG) signals emitted by human knee joints are nonstationary and multicomponent in nature; time-frequency distributions (TFD's) provide powerful means to analyze such signals. The objective of this paper is to construct adaptive TFD's of VAG signals suitable for feature extraction. An adaptive TFD was constructed by minimum cross-entropy optimization of the TFD obtained by the matching pursuit decomposition algorithm. Parameters of VAG signals such as energy, energy spread, frequency, and frequency spread were extracted from their adaptive TFD's. The parameters carry informa

Atraumatic osteochondral fracture and osteoarthritis in a collegiate volleyball player: a case report.

OBJECTIVE: To present the case of a collegiate athlete with an atraumatic osteochondral fracture influenced by the presence of osteoarthritis. BACKGROUND: Osteochondral fractures are fairly common occurrences in athletes, although it can be difficult to recognize such an injury in the absence of a traumatic event. Osteoarthritis is 1 condition that can increase an athlete's susceptibility to an atraumatic osteochondral fracture. However, because of the atraumatic nature of the injury, the possibility of an osteochondral fracture may be overlooked. DIFFERENTIAL DIAGNOSIS: Meniscal damage, ost

Patellofemoral pain: let the physical exam define treatment.

The complex causes of patellofemoral disorders are most effectively identified through a systematic evaluation of a patient's lower-extremity alignment, patellar mobility, muscle flexibility, strength, and coordination as well as an assessment of soft-tissue and articular pain. By combining information from such an exam with a careful history and appropriate radiographic studies, the physician can make a specific diagnosis. This sets the stage for an optimal rehabilitation prescription, which usually will involve some combination of muscle flexibility and strength training, taping, orthoses, a

Clinical classification of patellofemoral pain and dysfunction.

The patellofemoral joint is a complex articulation based on its dependence on both dynamic and static restraints for stability. Classification of patellofemoral disorders has been frought with confusion. However, progress has been made in the classification and understanding of these disorders by improved understanding of the biomechanics of the joint and by clarification of the terminology to describe patellofemoral pathology. The term chondromalacia patella, although once used as an all-inclusive term for anterior knee pain, is now widely accepted as a term used to describe pathologic lesion

Patellofemoral disorders: a classification system and clinical guidelines for nonoperative rehabilitation.

Patellofemoral disorders are among the most common clinical conditions managed in the orthopaedic and sports medicine setting. Nonoperative intervention is typically the initial form of treatment for patellofemoral disorders; however, there is no consensus on the most effective method of treatment. Although numerous treatment options exist for patellofemoral patients, the indications and contraindications of each approach have not been well established. Additionally, there is no generally accepted classification scheme for patellofemoral disorders. In this paper, we will discuss a classificati

Patellofemoral Instability: Evaluation and Management.

Patellofemoral disorders are a common cause of knee pain and disability. A thorough history and a careful physical examination are essential to accurate diagnosis, and imaging modalities play an important role. Magnetic resonance imaging can provide information on malalignment and soft-tissue injuries. Although there is a continuum of diagnoses, most patellofemoral disorders can be divided into three distinct categories: soft-tissue abnormalities, patellar instability due to subluxation and dislocation, and patellofemoral arthritis. Many patellofemoral disorders respond to nonoperative therapy

Radiological measurements in patellofemoral disorders. A review.

This paper analyzes some of the radiologic measurements used in the diagnosis and management of patellofemoral disorders. Measurements such as condylar width and height and patellar height from the true lateral view are helpful to determine the type and severity of intercondylar dysplasia. Skyline views provide information on the type and degree of patellar subluxation and dysplasia, especially of the medial aspect of the trochlear groove. In asymptomatic subjects, the trochlear angle does not exceed 140 degrees, and the lateral medial intercondylar ratio in dysplasia is > 1.7. Scanograms spec
