Mortality and disability after hospitalisation for injuries in low-income and middle-income countries: a cohort study.

BACKGROUND: Mortality and disability after injuries are high in low-income and middle-income countries (LMICs). Improving outcomes for injured patients requires knowledge of their associated factors. We aimed to assess post-injury mortality and disability and their associations with access to health care across LMICs with different injury burdens, health-care systems, and contexts. METHODS: Hospitalised patients in Ghana, Pakistan, Rwanda, and South Africa with moderate-to-severe injuries were recruited and followed up until 3 months after discharge or death. Injured patients of any age who w

Conclusions and Therapeutic Implications of AA.

AA is essential for several tissues in the body, as they are involved in many physiological and pathological processes. Its regulatory role in inflammation, immune response regulation, cell cycle, and membrane structural organization and its ability to control the expression and function(s) of several receptors and convey messages to and from the nucleus are of particular significance. Fine-tuning the metabolism of AA may lead to the development of prevention and management strategies for several diseases, including but not limited to sepsis, inflammatory bowel disease, diabetes mellitus, canc

Global, regional, and national burden of exposure to chewing tobacco, 1990-2023: a systematic analysis of the Global Burden of Disease Study 2023.

BACKGROUND: Exposure to chewing tobacco is a known health risk factor but has received less attention than smoking in global tobacco control efforts. As the evidence base linking chewing tobacco use to poor health grows, it is essential to closely track the changing use of chewing tobacco distinct from other forms of smokeless tobacco, as well as the estimated burden associated with its use. These data are essential to inform policy makers who seek to identify and avert the health burden associated with these dangerous products. METHODS: We estimated the prevalence of chewing tobacco use and

Chikungunya Virus Infection in Paraffin-Embedded Tissue: Analysis of Histological Alterations and Viral Detection by Immunohistochemistry.

Histopathological analysis of tissues infected with Chikungunya virus (CHIKV), including biopsy and autopsy specimens, can provide valuable insights into the pathogenesis of atypical and fatal cases. By examining tissue architecture and cellular alterations under the microscope, it is possible to identify patterns of injury, inflammation, and cellular degeneration. These morphological findings provide evidence of how the pathogen interacts with host cells and tissues. Moreover, immunohistochemistry may be performed in tissue sections for the detection of viral antigens. Importantly, this techn

Intestinal Stem Cells: Regulation, Plasticity, and Therapeutic Potential.

Intestinal stem cells (ISCs) are central to the maintenance and regeneration of the gut epithelium, supporting its rapid turnover and adaptive capacity. Recent advances, including the identification of LGR5+ stem cells, 3D in vitro organoid technology, and single-cell transcriptomics, have uncovered the remarkable heterogeneity and plasticity of ISCs and their niche. The ISC niche comprises dynamic interactions among epithelial, stromal, immune, and microbial components, enabling responses to injury and disease. Notably, both active and reserve stem cell pools, as well as progenitor and differ

Mechanisms underlying rheumatic heart disease immune-mediated pathogenesis: T-cell responses, cytokine networks, and immune dysregulation as drivers of valvular injury.

Acute rheumatic fever is an immune-mediated sequela of Streptococcus pyogenes (group A Streptococcus) infection, presenting with fever, arthritis, chorea, rash, and, crucially, carditis. Without effective secondary prophylaxis and care, acute rheumatic fever can progress to chronic rheumatic heart disease, a leading yet under-recognised cause of cardiovascular morbidity and death that disproportionately affects children and young adults aged 5-24 years in low-resource settings. Robust evidence implicates the role of antigenic mimicry-particularly between streptococcal M protein and cardiac or

Arthroscopic Management of an Unusual Fibrofatty Loose Body Presumed to Arise From Hoffa's Fat Pad: A Case Report.

We report the case of a 31-year-old man with persistent anterior knee pain and mechanical locking and catching despite conservative management. Preoperative magnetic resonance imaging (MRI) demonstrated an intra-articular loose body in the patellofemoral compartment with associated lateral meniscal and synovial abnormalities. Arthroscopy identified a greater than 5-mm fibrofatty loose body whose gross appearance was considered suggestive of a detached remnant of Hoffa's fat pad. Histopathologic analysis was not obtained, and direct continuity with the infrapatellar fat pad was not documented;

Arthroscopic Saucerization and Anatomic Repair of a Wrisberg Variant Discoid Lateral Meniscus: A Physeal-Sparing Knotless Anchor Technique.

Discoid lateral meniscus (DLM) is a congenital variant of the knee characterized by an abnormally shaped meniscus prone to tearing and instability. When symptomatic, the treatment plan is based on the location and pattern of the tear, as well as the presence of instability. In most cases, posterior horn instability is present, but some meniscotibial or root attachments remain. This technical note describes a comprehensive arthroscopic approach to a Wrisberg variant DLM, with a complete absence of posterior root and lateral capsular attachments. A comprehensive arthroscopic reconstruction using

Revisiting the Walburg hypothesis that radiation affects only tumor deaths but not non-tumor deaths in mice.

Starting in the 1950s or earlier, irradiation was found to shorten lifespans in mice, which raised the possibility that radiation science might contribute to an understanding of the nature of aging. However, in 1975, Walburg published a review paper to express his hypothesis that radiation exposure increases the risk of tumor deaths but not for non-tumor deaths. Subsequently, it became a mainstream belief that irradiation induces oncogenic mutations and thereby increases the risk of cancer and thus life shortening. However, we now think that the Walburg's hypothesis is not tenable as it did no

General technical considerations on reirradiation and focus on brain and head and neck tumours, suggestions from the Recommendations Commission of the Société française de radiothérapie oncologique.

The process of reirradiation in oncology presents a distinctive set of challenges, wherein clinical decision-making relies on a delicate balance between the efficacy of the treatment and the risk of severe toxicity. Despite the paucity of randomized data, there is an increasing number of data encouraging its consideration for patients with local or metastatic recurrence. While awaiting dedicated guidance from supranational scientific societies, the following publication is based on the "Recommendations Commission" of the Société française de radiothérapie oncologique (the French society for ra

A criticism of three arguments on recurrent imaging and risk.

Managing radiation protection (RP) in relation to recurrent imaging continues to be debated. Recent arguments have focussed on high cumulative effective dose (CED), especially where it exceeds 100 mSv. This article argues that CED should not be the primary focus for tracking concerns on recurrent imaging. Arguments which advocate for increased RP measures after 100 mSv is exceeded are criticised as being in conflict with the linear no threshold model. Finally, a view of dose history as adding to background risk is discussed.

Jump-landing mechanics following mid-flight external perturbation: anticipation and core muscle endurance.

The purpose was to quantify the effects of anticipation on landing mechanics associated with ACL loading during double-leg landings following mid-flight medial-lateral pushing perturbation. A total of 11 male and 11 female recreational athletes performed a jump-landing task under three perturbation conditions (no, left, vs. right perturbation) and two anticipation conditions (anticipated vs. unanticipated). The perturbation was created by having researchers throw a medicine ball (5% of body weight at an average speed of 4.4 m/s) to contact the participant's lateral trunk near their maximal jum

VIDD or CIADD? The ventilator remains a bona fide culprit.

Rosà and colleagues recently proposed that mechanical ventilation (MV) may not be the primary driver of diaphragm dysfunction in critically ill patients, advocating the term "critical illness-associated diaphragm dysfunction" (CIADD) over "ventilator-induced diaphragm dysfunction" (VIDD). While we support distinguishing VIDD from CIADD, we contend that the review's central conclusion underestimates the causal role of MV itself. This commentary addresses three questions in turn. First, does MV injure the diaphragm under controlled conditions? Here the evidence is strong: brain-dead donor studies

Single-cell Raman spectroscopy reveals multidimensional spectral phenotypes associated with curcumin protection in high-glucose-stressed cone cells.

High-glucose-induced oxidative stress plays a critical role in retinal cellular dysfunction associated with diabetic retinopathy. We investigated the protective effects of curcumin in high-glucose-exposed 661 W cone cells and evaluated single-cell Raman spectroscopy for label-free characterization of treatment-associated cellular responses. Cells were exposed to 55 mmol/L glucose for 24 h and then treated with 1.5 μmol/L curcumin for 6 h. Cell viability, reactive oxygen species (ROS) accumulation, TUNEL positivity, and Nrf2/HO-1 expression were assessed alongside Raman spectra from individual

Hemostatic Considerations of New Treatment Options for Hemophilia During Surgery, Infection, and Bleeding in Children and Adolescents.

In recent years, the therapeutic landscape for hemophilia has changed significantly, with the approval of several innovative agents, some featuring entirely novel mechanisms of action. These advances have substantially improved outcomes and reduced the treatment burden. However, clinical experience with these novel therapies in special situations-such as bleeding, surgery, trauma, and infection-remains limited. Factor replacement therapy has been the standard of care for a long time, with extensive clinical experience. The introduction of non-factor therapies has raised important questions: Is
