Porous tibial tuberosity advancement without using flange for the treatment of cranial cruciate ligament rupture in dogs-a clinical pilot study.

INTRODUCTION: Porous tibial tuberosity advancement (porous-TTA) is a surgical technique used to treat cranial cruciate ligament (CCL) rupture in dogs. However, there is a lack of studies evaluating its application without technical modifications such as the inclusion of a flange- under real-world clinical conditions and with standardized monitoring of bone healing and osteoarthritis progression. This pilot study aims to provide preliminary clinical data on porous-TTA without a flange. MATERIALS AND METHODS: Seventeen stifles diagnosed with CCL rupture treated with porous-TTA without a flange

Ultrasound-Activatable Piezoelectric Hydrogel Reprograms Mitochondrial Epigenetics for Osteoarthritis Therapy via the mTOR/GATD3A Axis.

The avascular nature of cartilage hinders drug delivery for osteoarthritis (OA) therapy. We engineered a biomimetic piezoelectric hydrogel (CMB Gel) by embedding chondrocyte membrane-camouflaged, CAP peptide-grafted barium titanate nanoparticles into a dynamic borate ester-crosslinked network, enabling active cartilage targeting and on-demand ultrasound activation. Under ultrasound, the piezoelectric component generates local electrical signals, triggering Ca2 + influx via voltage-gated calcium channels and AMPK activation. Activated AMPK inhibits the mTOR pathway, inducing epigenetic reprogra

Clinical technique for augmented reality-guided tibial resection in kinematic alignment total knee arthroplasty: Real-time ligament elongation enables personalised soft tissue balance.

PURPOSE: Kinematic alignment (KA) total knee arthroplasty (TKA) aims to restore native knee kinematics by aligning prosthetic components with each patient's constitutional anatomy. While femoral resurfacing is well established, tibial resection in KA-TKA remains largely subjective, particularly in cases with advanced deformity or bone loss. This study introduces a novel augmented reality (AR)-based technique for tibial resection that utilises real-time ligament elongation patterns to support personalised alignment. METHODS: More than 200 consecutive KA-TKA procedures were performed using an A

Loss of connexin 43 in cartilage causes mitochondrial dysfunction and accelerates post-traumatic osteoarthritis progression.

Osteoarthritis (OA) is a major cause of chronic pain and disability worldwide, characterized by progressive degeneration of cartilage and subchondral bone. Post-traumatic OA (PTOA) develops in as many as 25-50% of individuals following major joint injury, making it a leading cause of OA in younger and otherwise healthy populations. 1,2 Connexin 43 (Cx43), a gap junction protein involved in intercellular communication and cellular stress responses, has been linked to OA; however, its role in the progression of PTOA remains unclear. Here, we examined how cartilage-specific loss of Cx43 influence

Lutein Attenuates Synovial Hyperplasia, Cartilage Loss, and Bone Erosion by Suppressing Inflammatory Pathways in Rheumatoid Arthritis.

Lutein is a bioactive carotenoid best known for its role in retinal protection, but emerging evidence suggests that it may also exert anti-inflammatory activity in chronic inflammatory disorders. In this study, we investigated the anti-inflammatory and joint-protective effects of lutein in rheumatoid arthritis (RA)-related experimental models. Transcriptomic profiling was performed in MH7A fibroblast-like synovial cells treated with lutein, followed by Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG), and gene set enrichment analysis (GSEA). The results showed that lutein reg

Occult osteophyte fracture as an atypical source of massive hemothorax in diffuse idiopathic skeletal hyperostosis: illustrative case.

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) limits spinal mobility, predisposing patients to unstable fractures even following minor trauma. While spinal fractures are a well-documented complication, massive hemothorax remains a rare presentation, particularly when the bleeding source is from the osteophyte fracture site. OBSERVATIONS: An 84-year-old man with DISH, currently on a factor Xa inhibitor, presented after a 2-m fall. Initial imaging revealed a T11 reverse Chance-type fracture and a right hemothorax, notably without associated rib fractures or pulmonary contusions. O

Ultrasound-guided long-term platelet-rich plasma injections for moderate-to-severe knee osteoarthritis: a case report.

BACKGROUND: Moderate-to-severe knee osteoarthritis (KOA) involves progressive cartilage degeneration, meniscal tears, synovitis, and osteophyte formation, leading to persistent pain and functional impairment. Conventional therapies often provide inadequate relief. While platelet-rich plasma (PRP) injections have emerged as a promising regenerative treatment, their efficacy is predominantly established in mild-to-moderate KOA, with limited benefits observed in moderate-to-severe cases. CASE PRESENTATION: A 57-year-old Chinese male with bilateral moderate-to-severe knee osteoarthritis was treat

Eight-year clinical and radiographic outcomes after meniscal repair augmented with autologous synovial mesenchymal stem cell transplantation: A long-term follow-up case series.

BACKGROUND: Biological augmentation using synovial mesenchymal stem cells (MSCs) has been proposed to enhance healing after meniscal repair, particularly the repair of degenerative flap and radial tears with poor intrinsic healing potential. This study evaluated the 8-year outcomes after meniscal repair augmented with autologous synovial MSC transplantation, focusing on clinical results, radiographic changes, and adverse events. METHODS: Nine patients had previously been enrolled in an investigator-initiated clinical trial of autologous synovial MSC transplantation for medial meniscal tears.

The influence of retaining osteophyte on the coronal alignment and joint line height following medial unicompartmental knee arthroplasty.

BACKGROUND: The retention of osteophytes may influence the postoperative hip-knee-ankle (HKA) angle and joint line height (JLH) following unicompartmental knee arthroplasty (UKA); however, the specific impact remains poorly characterized. This study aimed to evaluate the effect of retaining the medial femoral condylar osteophyte on these parameters after medial UKA. METHODS: This sequential historical cohort study included patients who underwent medial UKA between January 1, 2025, and December 31, 2025. The patient cohort comprised an osteophyte removal group and an osteophyte retention group

Automated Kellgren-Lawrence grading of knee osteoarthritis using a multi-scale attention-based deep learning framework.

BACKGROUND: Accurate radiographic grading of knee osteoarthritis (KOA) using the Kellgren-Lawrence (KL) system is essential for clinical decision-making but suffers from subjective interpretation and inter-observer variability. Deep learning approaches have shown promise, yet existing models often rely on single-scale features, lack effective attention mechanisms, and generalize poorly to external datasets. METHODS: We developed a novel multi-scale attention-guided deep learning framework that synergistically integrates three key components: (1) a Feature Pyramid Network (FPN) for multi-scale

Organ-on-a-chip systems for osteochondral units: unveiling biomechanical and pathological mechanisms.

With the establishment of key principles governing osteochondral structure, function, and reconstruction, researchers have gained an expanded toolkit for the precisein-vitroreconstruction of osteochondral tissues. As a convergence of tissue engineering and microphysiological modeling, the biomechanical heterogeneity of the osteochondral layers, which is critical to joint function, can be precisely engineered within osteochondral unit-on-a-chip (OC-OoCs), making them ideal tools for studying physiological activities. Specifically speaking, OC-OoCs are regarded as a promising platform for invest

Structural osteoarthritis pathogenesis correlates with distinct pain and dysfunction profiles after ACL injury in rats.

Post-traumatic osteoarthritis (PTOA) frequently arises following knee injury, such as anterior cruciate ligament (ACL) rupture. Small animal models are critical for studying PTOA pathology and translating therapeutic interventions. Here our objective was to identify key structural PTOA pathologies and evaluate their associations with aberrant pain and function after a clinically relevant ACL injury in a small animal model. This work leveraged a suite of quantitative cartilage and bone analysis techniques using high-resolution contrast-enhanced microcomputed tomography to investigate the associ

Anterior Cervical Osteophyte Thinning for Forestier's Disease: A Case Series, Scoring System and Treatment Algorithm.

BACKGROUND AND OBJECTIVES: Forestier's disease, also known as diffuse idiopathic skeletal hyperostosis, causes abnormal ossification, commonly affecting the anterior cervical spine. Symptomatic patients can experience dysphagia, dyspnea, hoarseness, or aspiration due to the mass effect exerted on midline structures of the neck. When conservative measures have failed, surgical removal or thinning of the osteophytes often alleviates these symptoms. This study offers a case series of patients treated surgically for "diffuse idiopathic skeletal hyperostosis-phagia," a scoring system to identify ap
