Chronic health manifestations following hump-nosed pit viper (Hypnale spp.) bites: an authenticated case series.

BACKGROUND: Hump-nosed pit vipers (genus Hypnale) are the most common cause of venomous snakebites in Sri Lanka, accounting for more than one-third of all snakebites, as they are widely distributed throughout the country. It is a highly medically important snake in Sri Lanka. Even though several life-threatening complications, including venom-induced consumption coagulopathy, acute kidney injury, venom-induced thrombotic microangiopathy and cardiac effects, are reported following their bites, no antivenom is currently available for Hypnale - envenomed patients either in Sri Lanka or India. Delayed-Onset Hemolysis in a Case of Hemolytic Uremic Syndrome: A Diagnostic Challenge.

BACKGROUND: Hemolytic uremic syndrome is a rare thrombotic microangiopathy characterized by hemolytic anemia, thrombocytopenia, and acute kidney injury. While commonly reported in children, adult-onset presentations are less frequent and often atypical, leading to diagnostic delays. This case underscores the importance of repeated evaluation when classical features are absent initially. CASE PRESENTATION: A 50-year-old woman was admitted with diarrhea, vomiting, abdominal pain, and visible hematuria. Initial findings included severe thrombocytopenia and Stage 3 acute kidney injury but no evid

Optimizing the management of congenital thrombotic thrombocytopenic purpura.

BACKGROUND: Congenital thrombotic thrombocytopenic purpura (cTTP) is a rare, life-threatening thrombotic microangiopathy caused by genetic mutations in the ADAMTS-13 gene, leading to severe enzyme deficiency. This results in the accumulation of ultralarge von Willebrand factor multimers, which trigger platelet aggregation, microangiopathic hemolytic anemia, thrombocytopenia, and organ damage. cTTP episodes are often triggered by physiological stressors and have a bimodal age of presentation. OBJECTIVES: To summarize the clinical course, complications, and advances in treatment of cTTP, highli

Fibronectin glomerulopathy caused by genetic FN1 mutation: A case report and literature review.

BACKGROUND: Fibronectin glomerulopathy (FGP), also known as fibronectin deposition glomerulopathy (GFND), is a rare hereditary autosomal dominant glomerular disease. Its clinical manifestations are proteinuria, hematuria, hypertension, and hyperkalemic distal renal tubular acidosis, which often progresses slowly to end-stage renal disease. CASE DESCRIPTION: We report a 21-year-old woman with fibronectin glomerulopathy who underwent renal puncture at the age of 10. The pathology was considered to be thrombotic microangiopathy, and it was not treated regularly. This time, renal puncture was per

A case of membranous proliferative glomerulonephritis-type IgA nephropathy modified by nintedanib.

A 73-year-old woman had been treated with nintedanib for 40 months for idiopathic pulmonary fibrosis. She presented to our hospital with lower extremity edema since a month and nephrotic syndrome (serum albumin 2.0 mg/dL, urine protein 7.97 g/g Cr). We performed a renal biopsy and diagnosed membranoproliferative glomerulonephritis (MPGN)-type IgA nephropathy (IgAN). We hypothesized that nintedanib-induced microangiopathy modified subclinical IgAN and caused the appearance of MPGN-type IgAN and nephrotic syndrome. After the discontinuation of nintedanib, the urinary protein level improved to 0.

RTX-family toxin EhxA drives morphological remodeling and thrombogenesis in RBCs during enterohemorrhagic Escherichia coli infection.

Enterohemorrhagic Escherichia coli (EHEC) causes thrombotic microangiopathy, yet the red blood cell (RBC)-centered mechanism has remained unclear. We identify the RTX-family hemolysin EhxA as the driver of RBC-mediated thrombogenesis. Deletion of ehxA abolishes Ca2+ influx, phosphatidylserine (PS) exposure, progression from discocyte to echinocyte to spherocyte, thrombin generation, RBC-endothelium adhesion, and RBC aggregation. Genetic complementation restores these readouts to wild type, and purified EhxA in bacteria-free assays recapitulates them while localizing to intact RBC membranes. By

Case Report: Glomerular microangiopathy and reversible renal toxicity during prolonged bevacizumab therapy in ovarian cancer.

BACKGROUND: Bevacizumab, a VEGF-A inhibitor widely used in ovarian cancer, is associated with kidney adverse effects ranging from mild proteinuria to thrombotic microangiopathy (TMA). However, the longitudinal course, reversibility, and spectrum of kidney-limited injury remain under-characterized. CASE REPORT: We describe three patients with ovarian cancer who developed proteinuria during prolonged bevacizumab therapy, including one with biopsy-proven kidney-limited glomerular microangiopathy. Proteinuria emerged after 6 to 12 months of exposure in all cases. Kidney function remained preserve

Hypertensive Havoc: When Malignant Hypertension Mimics Thrombotic Thrombocytopenic Purpura.

Malignant hypertension (HTN) is defined by a rapid rise in blood pressure exceeding 200/130 mmHg, leading to acute end-organ damage. Although relatively uncommon, malignant HTN can cause thrombotic microangiopathy (TMA), characterized by microangiopathic hemolytic anemia, thrombocytopenia, and elevated serum lactate dehydrogenase (LDH). This presentation can mimic thrombotic thrombocytopenic purpura (TTP), a hematologic emergency characterized by thrombocytopenia, hemolytic anemia, renal and neurologic dysfunction, and fever. We present the case of a 28-year-old male patient who presented with

Syndrome of Inappropriate Antidiuretic Hormone Secretion and Thrombotic Microangiopathy as Paraneoplastic Syndromes Complicating BRCA2-Mutated Metastatic Prostate Cancer.

Metastatic prostate cancer with a BRCA2 mutation is associated with aggressive clinical behavior and poor outcomes with standard systemic therapy. While the BRCA2 mutation predicts response to PARP inhibitors and platinum agents, its association with paraneoplastic syndromes is not well described. We report a 72-year-old male who presented with altered sensorium with severe hyponatremia who was diagnosed with syndrome of inappropriate antidiuretic hormone secretion (SIADH) in the context of newly diagnosed metastatic prostate adenocarcinoma. He was treated with free water restriction, hyperton

Impact of Day of Hospital Admission and Hospital Characteristics on Mortality in Thrombotic Thrombocytopenic Purpura.

Thrombotic thrombocytopenic purpura (TTP) is a life-threatening condition where prompt diagnosis and timely therapy are essential for improving patient outcomes. Previous studies have suggested that weekend hospital admissions may be associated with limited resources, leading to worse clinical outcomes. Given ongoing advancements in healthcare infrastructure, we aimed to re-evaluate the impact of weekend admissions and hospital characteristics on TTP mortality. We conducted a retrospective cohort study using the National Inpatient Sample (NIS) database from 2016 to 2019. A chi-squared test was

C3 mutation-associated atypical hemolytic uremic syndrome with severe renal dysfunction and hypertensive emergency successfully treated with ravulizumab and sacubitril/valsartan: a case report.

BACKGROUND: Atypical hemolytic uremic syndrome (aHUS) is a rare complement-mediated thrombotic microangiopathy characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute kidney injury. Although C5 inhibitors, such as eculizumab and ravulizumab, have markedly improved outcomes, renal recovery remains limited in cases complicated by hypertensive emergency (HE) or malignant hypertension, likely due to complement-independent vascular injury. Angiotensin receptor–neprilysin inhibitor therapy exerts dual actions of renin–angiotensin system blockade and augmentation of natriureti

Identifying novel biomarkers for systemic lupus erythematosus associated with macrophage activation syndrome.

OBJECTIVE: Macrophage activation syndrome (MAS) represents a subtype of hemophagocytic lymphohistiocytosis (HLH) associated with autoimmune diseases. Systemic lupus erythematosus (SLE) is one of the most common autoimmune diseases that induce MAS. The overlapping systemic symptoms of SLE and clinical manifestations of HLH pose challenges in clinical diagnosis, contributing to significant rates of underdiagnosis and misdiagnosis, warranting further investigation. Identifying early diagnostic biomarkers with high sensitivity and specificity for MAS in SLE holds significant clinical importance.

Genetics of thrombotic thrombocytopenic purpura: systematic review in immune and congenital thrombotic thrombocytopenic purpura.

Thrombotic thrombocytopenic purpura (TTP) is a thrombotic microangiopathy caused by severe deficiency of the metalloprotease ADAMTS13. TTP has 2 subtypes: immune (iTTP) and congenital (cTTP). Autoimmune mechanisms underlie iTTP; however, the genetic factors influencing disease risk, relapse risk, and response to treatment are incompletely understood. In cTTP, although variants are described throughout the ADAMTS13 gene, the effect of many of these variants is uncertain. We conducted a comprehensive literature review of both iTTP and cTTP, incorporating findings from genome-wide association stu

Successful living kidney transplantation in a T-cell flow cytometry crossmatch-positive patient with atypical hemolytic uremic syndrome treated with an anti-C5 antibody: a case report.

Atypical hemolytic uremic syndrome (aHUS) is an intravascular hemolytic disorder caused by complement dysregulation. A 41-year-old female patient with end-stage kidney disease and a history of glomerulonephritis had been receiving frequent blood transfusions for anemia and thrombocytopenia with hemolytic findings. She came to our hospital for a living donor kidney transplant from her mother, and a preoperative evaluation revealed ABO incompatibility and strong positivity in the T-cell flow cytometry crossmatch (FCXM) due to broad anti-HLA antibodies sensitized by previous blood transfusions. Case report: Different outcomes of two cases of relapsed/refractory T cell acute lymphoblastic leukemia treated with anti-CD7 chimeric antigen receptor T cells bridging to allogeneic hematopoietic stem cell transplantation: from curative promise to fatal risk.

Consolidative allogeneic hematopoietic stem cell transplantation (allo-HSCT) after chimeric antigen receptor (CAR) T-cell therapy is an emerging modality in hematologic malignancies. Knowledge regarding the optimal interval and pretreatment regimen between CAR T-cell therapy and allo-HSCT remains limited. Here, we report two cases of confirmed relapsed/refractory (R/R) T-cell acute lymphoblastic leukemia (T-ALL) treated with autologous anti-CD7 CAR T cells infusion bridging to allo-HSCT. Case 1 had a poor prognosis due to grade 3 cytokine release syndrome (CRS), infection, drug-related organ t

Avoidance of related donors in CAEBV with germline immune variants: long-termoutcome of matched unrelated donor HSCT - a case report.

This study reports an adult-onset case of NK cell-predominant chronic active Epstein-Barr virus infection (CAEBV) harboring multiple heterozygous germline variants affecting antiviral immunity. Functional assessments of NK cell cytotoxicity and degranulation in the patient and her family members revealed subclinical immune defects in several relatives, leading to the exclusion of related donors. The patient ultimately underwent a fully HLA-matched unrelated donor hematopoietic stem cell transplantation (MUD-HSCT), achieving early virologic remission and complete donor chimerism. However, the p

Pathologic diagnosis incorporating molecular monitoring highlights new pathophysiological mechanisms in kidney xenotransplantation.

Kidney xenotransplantation addresses organ shortages; however, short-term clinical observations provide limited insights into long-term graft changes. A pathology-molecular framework integrating biopsy phenotyping, transcript analysis, and spatial profiling can improve the interpretation of long-surviving pig-to-nonhuman primate kidney xenografts. B-HOTX, a xenograft-adapted version of the Banff Human Organ Transplant gene panel, distinguishes donor from recipient transcripts, reducing the misclassification of physiological leukocyte signals as rejection. Key insights include macrophage-domina
