BACKGROUND: Congential thrombotic thrombocytopenic purpura (cTTP) is a ultrarare genetic disorder caused by reduced or absent ADAMTS13 enzyme activity. In contrast to the acquired form of TTP, which results from inhibitory autoantibodies against ADAMTS13, cTTP arises due to mutations in the ADAMTS13 gene and lacks such inhibitors. cTTP commonly manifests during infancy through recurrent episodes of hemolytic anemia and signs of microvascular damage. Episodes are often precipitated by infections or physical stressors.

Background: Thrombotic thrombocytopenic purpura (TTP) is a rare but life-threatening thrombotic microangiopathy (TMA) caused by severe deficiency of the von Willebrand factor-cleaving protease ADAMTS13. Pregnancy is a recognized trigger for both immune-mediated and congenital TTP and is associated with increased maternal and fetal morbidity. Clinical overlap with other pregnancy-associated TMAs, including preeclampsia and Hemolysis, Elevated Liver enzymes, and Low Platelet count (HELLP) syndrome, often delays diagnosis. This review synthesizes current evidence on pathophysiology, diagnostic un

INTRODUCTION: Endothelial injury is a major contributor to morbidity and mortality in pediatric patients undergoing hematopoietic cell transplantation (HCT). It appears as sinusoidal obstruction syndrome (SOS) or transplant-associated thrombotic microangiopathy (TMA), among other conditions. Composite indices like the Endothelial Activation and Stress Index (EASIX) and its modified version (m-EASIX) may serve as accessible biomarkers for early identification. However, their utility in pediatric populations is unestablished. We aimed to explore whether EASIX and m-EASIX can help identify endoth

A disintegrin and metalloproteinase with a thrombospondin type 1 motif, member 13 (ADAMTS13), is an enzyme that cleaves ultra-large von Willebrand factor (VWF) multimers. ADAMTS13 deficiency leads to thrombotic thrombocytopenic purpura (TTP), a thrombotic microangiopathy characterized by micro-thrombosis in small vessels and target-organ damage. Given that in long-term TTP survivors, reduced ADAMTS13 activity has been associated with the development of major cardiovascular events, the emerging evidence regarding the dysregulation of the ADAMTS13/ VWF axis in the pathogenesis of ischemic stroke

A young woman presented with a 17-year history of proteinuria, hematuria with persistent hypocomplementemia. She has been followed up for 2 years. The patient underwent comprehensive diagnostic evaluation, including two renal biopsies. The initial biopsy indicated membranoproliferative glomerulonephritis (MPGN), while the subsequent biopsy revealed concurrent thrombotic microangiopathy (TMA) kidney injury, suggesting a complement-mediated disorder. Genetic testing identified two heterozygous missense variants, c.848A > G (p.Asp283Gly) and c.1339C > T (p.Pro447Ser), in the Complement factor I (

Catastrophic antiphospholipid syndrome (CAPS) is a rare, life-threatening variant of antiphospholipid syndrome (APS) characterized by rapid, widespread thrombosis leading to multiorgan failure. Affecting less than 1% of APS patients, CAPS is associated with a high mortality rate of 30%-50%, necessitating prompt diagnosis and aggressive treatment. The mainstay of management includes anticoagulation, high-dose glucocorticoids, and plasma exchange or intravenous immunoglobulins, with biologic therapies such as rituximab and eculizumab reserved for refractory cases. We report a case of a 60-year-o

INTRODUCTION: Complement homeostasis is essential for healthy pregnancy and excessive complement activation can lead to devastating maternal and fetal outcomes by precipitating or worsening complement-mediated disorders. This review summarizes changes in complement regulation during pregnancy, management considerations in paroxysmal nocturnal hemoglobinuria (PNH), thrombotic microangiopathies (TMA), and antiphospholipid syndrome (APS), and emerging evidence regarding the role of complement in the severe hypertensive disorders of pregnancy (preeclampsia with severe features, HELLP syndrome).

Post-infectious glomerulonephritis (PIGN) and atypical hemolytic uremic syndrome (aHUS) can overlap due to alternative complement pathway dysregulation. We report a 4-year-old boy with edema, hypertension, hematuria, and reduced urine output. Laboratory findings included hemoglobin of 11.5 g/dL, platelets of 350 × 109/L, protein/creatinine ratio of 0.6 g/mmol, serum creatinine of 125 μmol/L, C3 of 0.3 g/L, and biopsy-confirmed PIGN. Despite supportive care, corticosteroids, and mycophenolate mofetil, kidney function remained impaired. Ten weeks later, the patient developed thrombotic microangi

INTRODUCTION: IgA nephropathy (IgAN) with concurrent thrombotic microangiopathy (TMA) is an unusual combination of pathological findings that is associated with severe hypertension, proteinuria, and lower estimated glomerular filtration rate (eGFR). This series presents 7 patients with kidney biopsies demonstrating IgAN and concomitant TMA and presents a link by way of complement disorder.
CASE PRESENTATION: Seven patients all presented as IgAN with associated TMA coinciding with hypertension, proteinuria, and low eGFR. Six patients showed tubular atrophy or interstitial fibrosis and 5 of tho

BACKGROUND: HIV is a significant aetiological factor in thrombotic microangiopathy (TMA) in regions of high seroprevalence, but description of HIV-associated TMA (HIV-TMA) remains limited to small case series. We sought to describe the presentation, complications of TPE, and mortality and renal outcomes of HIV-TMA.
METHODS: We retrospectively reviewed 98 cases of HIV-TMA treated with therapeutic plasma exchange (TPE) between 1/1/2010 and 31/12/2020. The effect of HIV infection and clinical presentation on mortality, TPE complications, and renal outcomes were analysed using regression analysis

Evans syndrome (ES), the coexistence of autoimmune hemolytic anemia and immune thrombocytopenia, can unmask systemic autoimmune disease. We report a 30-year-old woman who presented with fatigue, jaundice, pallor, and mucocutaneous bleeding. Laboratory evaluation demonstrated severe Coombs-positive hemolytic anemia (hemoglobin 5.8 g/dL, reticulocytosis, high LDH, undetectable haptoglobin) and marked thrombocytopenia, whereas immunologic testing revealed high-titer ANA, anti-dsDNA positivity, and low complement levels; infections, thrombotic microangiopathy, and malignancy were excluded. She was

BACKGROUND: Atypical hemolytic uremic syndrome (aHUS) is a thrombotic microangiopathy characterized by dysregulation of the complement system due to genetic mutations and polymorphisms in complement activators and regulators. In Japan, C3 is the gene most frequently implicated in aHUS, with approximately three-quarters of the mutations reported for C3 corresponding to p.Ile1157Thr (p.I1157T). Patients with the C3 p.I1157T mutation are notable for their favorable prognosis despite the high frequency of aHUS relapse compared with other C3 mutations; however, some cases progress to end-stage rena

INTRODUCTION: We present a case of acute kidney failure due to post-transplant lymphoproliferative disorder (PTLD) in the transplanted kidney 6 years after transplantation. Epstein-Barr virus (EBV) infection was identified as the likely trigger, and reducing immunosuppression was sufficient to resolve both inflammation and renal failure.
CASE PRESENTATION: A female patient in her twenties with acute renal failure had undergone a living-donor kidney transplantation from an EBV-positive donor, who was her mother, 6 years earlier and continued to receive immunosuppressive medication, including t

BACKGROUND: In the ICU, distinguishing immune-mediated thrombotic thrombocytopenic purpura (iTTP) from sepsis-associated thrombotic microangiopathy (TMA) is time-critical. We tested whether serial ADAMTS-13 combined with targeted coagulation and inflammation markers improves iTTP risk stratification in a Sepsis-3 ICU cohort and whether a pragmatic rule-out is feasible.
METHODS: Prospective single-center study of adults meeting Sepsis-3 with thrombocytopenia and schistocytes ≥ 1% or LDH > 2× ULN within 24 h of ICU admission. ADAMTS-13 activity and VWF: Ag were assayed at 0/24/48/72 h alongside

Thrombotic microangiopathy (TMA) is a pathological condition characterized by microangiopathic hemolytic anemia, thrombocytopenia, and ischemic organ dysfunction due to microvascular endothelial damage and thrombosis. It affects ∼0.8%-14% of kidney transplant recipients, and may manifest as either a recurrent or de novo disease. While systemic manifestations are commonly anticipated, kidney-limited TMA can also occur and is not rare. Histopathologic examination of allograft biopsies shows morphologic features indicating endothelial injury, and repeated episodes of TMA may result in coexisting

Hemolytic-uremic syndrome (HUS) is a thrombotic microangiopathy (TMA) characterized by microangiopathic hemolytic anemia, thrombocytopenia, and renal impairment. The typical form of HUS is most often associated with Shiga toxin-producing Escherichia coli infection, whereas atypical HUS (aHUS) is a rare variant caused by genetic mutations that disrupt complement regulation. This dysregulation promotes complement deposition on vascular endothelium, leading to microangiopathic hemolysis, platelet consumption, and organ injury, with acute kidney injury being the most common clinical manifestation.

BACKGROUND: iMCD-TAFRO (the TAFRO clinical subtype of idiopathic multicentric Castleman disease (iMCD)) is characterized by thrombocytopenia, anasarca, fever, reticulin fibrosis, and organomegaly. Diagnosis is challenging due to its rarity, non-specific early presentation, and overlap with sepsis, lymphoma, and thrombotic microangiopathy (TMA). Endothelial injury is increasingly recognized as a central driver of its severe complications.
CASE PRESENTATION: A previously healthy 20-year-old woman presented with rapid progression of fever, anasarca, jaundice, and respiratory failure. Laboratory

This report describes the case of a 12-year-old female patient with severe lupus nephritis and refractory thrombotic microangiopathy, who required dialysis for several months despite receiving intensive immunosuppressive treatment, including terminal complement blockade with eculizumab. Following a switch in therapy from eculizumab to pegcetacoplan, there was a significant reduction in haemolytic activity and steady improvement in kidney function. After more than 8 months of haemodialysis, the patient could be weaned off dialysis. This case study highlights the therapeutic potential of pegceta
