Recognition of ear, nose and throat involvement in patients with ANCA-associated vasculitis: from in-depth interviews to insights for the future.

PURPOSE: Ear, nose, and throat (ENT) manifestations are common in antineutrophil cytoplasmic antibody-associated vasculitis (AAV). Although timely intervention is crucial, diagnosis and treatment are often delayed due to its rarity and nonspecific symptoms. Currently, no standardized tool exists to assess AAV activity in the ENT region. This study aims to determine key indicators of active AAV in the ENT region, thereby supporting the development of a systematic examination method. METHODS: A qualitative exploratory multicentre study was conducted. In-depth interviews with four otorhinolaryng

Likelihood ratios of ANCA fluorescence patterns and anti-MPO/PR3 antibody levels in ANCA-associated Vasculitis: A framework for refined diagnostic interpretation.

OBJECTIVES: To determine the likelihood ratios (LRs) of anti-neutrophil cytoplasmic antibody (ANCA) fluorescence patterns and myeloperoxidase (MPO)/proteinase 3 (PR3)-ANCA antibody levels in ANCA-associated vasculitis (AAV), and evaluate their implications for ANCA interpretation. METHODS: This retrospective study included a screening cohort of 38,610 patients, a diagnostic cohort of 226 newly diagnosed AAV patients and 738 disease controls, and an independent validation cohort of 116 AAV patients and 448 controls. Diagnostic performance was assessed using result-specific LRs. RESULTS: ANCA

Clinical scenarios of suboptimal response to rituximab in relapsing ANCA-associated vasculitis.

BACKGROUND: Rituximab is effective for induction and maintenance of remission in relapsing ANCA-associated vasculitis (AAV), but some patients do not achieve complete remission or experience relapse despite treatment. We aimed to describe the frequency, characteristics, and outcomes of patients with suboptimal response to rituximab within the RITAZAREM trial. METHODS: Post hoc descriptive analysis, including patients with granulomatosis with polyangiitis (GPA) or microscopic polyangiitis (MPA) treated with rituximab for induction (N = 188) and those receiving rituximab maintenance (N = 85). Vasculitis Syndromes with Renal Involvement: A Review from a Case Series.

This review describes vasculitis syndromes that can be diagnosed by kidney biopsy. Microscopic polyangiitis (MPA) and granulomatosis with polyangiitis (GPA) typically follow a rapidly progressive course, often leading to end-stage renal failure within a few months, with crescentic glomerulonephritis being the predominant finding on a kidney biopsy. However, some types primarily present with fever and elevated C-reactive protein levels, while the kidney function is preserved; in such cases, a kidney biopsy shows arteriolitis. Eosinophilic granulomatosis with polyangiitis (EGPA) is characterized

Clinicopathological Spectrum of Head and Neck Mucoepidermoid Carcinoma: A Three-case Series.

OBJECTIVE: To examine the ability of organisms that infect the catheterized urinary tract to migrate over the surfaces of Foley catheters. MATERIALS AND METHODS: In a simple laboratory model, organisms were challenged to migrate across sections of hydrogel-coated latex, hydrogel/silver-coated latex, silicone-coated latex and all-silicone catheters. The sections (1 cm long) were placed as bridges in channels between blocks of agar and the test organisms inoculated onto the agar adjacent to one side of each bridge. The plates were incubated at 37 degrees C for 24 h and examined for growth of th

The risk of primary lung cancer in ANCA-associated vasculitis: systematic review and meta-analysis.

BACKGROUND: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a systemic inflammatory disorder that affects small blood vessels. However, the association between AAV and lung cancer risk remains uncertain. We performed a systematic review and meta-analysis to evaluate the relationship between AAV and the risk of lung cancer. METHODS: We systematically searched PubMed, Embase, Scopus, and Web of Science from inception through March 2025. Studies reporting relative risks (RRs), odds ratios (ORs), hazard ratios (HRs), or standardized incidence ratios (SIRs) with 95% confi

Reversible choroidal ischemia as a rare sight-threatening manifestation of microscopic polyangiitis presenting with crescentic glomerulonephritis.

BACKGROUND: Ocular involvement in ANCA-associated vasculitis is uncommon and usually affects the anterior segment. Posterior segment ischemic involvement, particularly choroidal perfusion disturbance, is extremely rare. We report a patient with microscopic polyangiitis (MPA) who developed bilateral visual impairment due to choroidal perfusion impairment demonstrated by fluorescein angiography (FFA), along with severe crescentic glomerulonephritis. CASE PRESENTATION: A 53-year-old woman presented with acute kidney injury, proteinuria, hematuria, elevated inflammatory markers, and p-ANCA positi

ANCA-associated vasculitis is associated with an increased risk of cardiac and vascular morbidity: results of a large-scale propensity-matched global retrospective cohort study.

INTRODUCTION: Despite significantly improved therapies in recent years, long-term morbidity and mortality in ANCA-associated vasculitis (AAV) remain high. The leading causes of death within the first year after diagnosis are active vasculitis and in subsequent years cardiovascular diseases, malignancies, and infections. Population-based database and cohort analyses suggest an increased risk for major adverse cardiovascular events (MACE) in AAV. METHODS: This retrospective cohort study analyzed data samples from an electronic health records database of the US-based TriNetX network. Patients wi

Anti-Myeloperoxidase (MPO)-Positive Granulomatosis With Polyangiitis Presenting With Pulmonary and Cutaneous Vasculitic Flares in End-Stage Renal Disease: A Diagnostic and Therapeutic Challenge.

Granulomatosis with polyangiitis (GPA) is a systemic small-vessel vasculitis typically associated with anti-proteinase-3 antibodies. Anti-myeloperoxidase (MPO) positivity is uncommon in GPA and often creates clinical overlap with microscopic polyangiitis. Managing atypical flares in patients with end-stage renal disease (ESRD) is particularly complex, as traditional goals such as renal recovery are absent, shifting the focus to life-threatening extrarenal manifestations. We present the case of a 50-year-old woman with known anti-MPO-positive GPA and ESRD on peritoneal dialysis who presented wi

Progressive Pseudorheumatoid Dysplasia Presenting with Cauda Equina Compression: A Case Report.

PURPOSE: Fresh vegetables are essential human dietary components but are increasingly implicated in foodborne infections caused by Escherichia coli. Contamination may occur during cultivation, irrigation, handling, and retail display, and the presence of pathogenic or antimicrobial-resistant E. coli poses significant public health risks. Despite widespread vegetable consumption in Ogun State, Nigeria, limited data exist on the resistance and virulence characteristics of E. coli in these foods. This study therefore assessed antimicrobial resistance profiles and virulence genes in E. coli isolat

Kidney Biopsy of a Patient With Double Positive Antiglomerular Basement Membrane and Myeloperoxidase-antineutrophil Cytoplasmic Antibodies.

We report on an 82-year-old woman who developed rapidly progressive renal failure and whose serum creatinine level increased from 1.46-7.18 mg/dL over a 3-week period. Tests for antiglomerular basement membrane (anti-GBM) antibodies (136 U/mL) and myeloperoxidase-antineutrophil cytoplasmic antibodies (MPO-ANCA; 161 IU/mL) were both positive. Kidney biopsy revealed acute-phase lesions of necrotizing crescentic glomerulonephritis with fibrin deposition in 28 of 33 glomeruli. This finding was consistent with anti-GBM glomerulonephritis. Concentric necrotic lesions with fibrin deposition were seen

A Rare Case of Salmonella Lung Abscess in an Immunocompromised Patient.

Salmonella enteritidis (S. enteritidis) is a Gram-negative, acid-labile bacterium responsible for major foodborne outbreaks in both developing and developed countries. While Salmonella species are often linked with gastrointestinal illness, pulmonary involvement is rare. We report a case of a Salmonella lung abscess occurring in the setting of Salmonella septicemia, in an immunocompromised patient with perinuclear anti-neutrophil cytoplasmic antibody (p-ANCA)-associated microscopic polyangiitis (MPA) and clear cell renal cell carcinoma (ccRCC). Initially admitted to Arrowhead Regional Medical

Therapeutic Plasma Exchange Using Membrane Plasma Separation Across a Broad Spectrum of Indications: An Eight-Year Single-Center Retrospective Cohort Study.

BACKGROUND: Therapeutic plasma exchange (TPE) removes pathogenic plasma constituents such as autoantibodies, immune complexes and monoclonal proteins. Plasma can be separated by centrifugation or by membrane plasma separation (MPS); although MPS predominates in many centers because it can be delivered at the bedside on standard renal replacement platforms, most published outcome data derive from centrifugal techniques. OBJECTIVE: To describe the indications, complications, laboratory changes and clinician-assessed treatment response of MPS-based TPE in an unselected single-center cohort. MET

Rapidly progressive glomerulonephritis in rheumatoid arthritis: an early presentation of ANCA-associated vasculitis.

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis is a multisystem autoimmune disease that may present diagnostic challenges, particularly in patients with coexisting autoimmune conditions. Although the coexistence of rheumatoid arthritis and ANCA-associated vasculitis is recognised, vasculitis more commonly develops several years after rheumatoid arthritis onset. We describe a 68-year-old man who developed rapidly progressive glomerulonephritis due to myeloperoxidase (MPO)-ANCA-associated microscopic polyangiitis within 6 months of a diagnosis of seropositive rheumatoid arthri

Multi-database pharmacovigilance identifies disproportionate reporting of hepatobiliary events with avacopan: an integrative study with network pharmacology and interpretable machine learning.

Avacopan is an oral C5a receptor antagonist approved for severe active granulomatosis with polyangiitis and microscopic polyangiitis. However, its real-world safety profile, particularly hepatobiliary safety signals, remains incompletely characterized. We analyzed avacopan-related adverse event reports from FAERS, JADER, and EudraVigilance. Disproportionality analyses were performed using reporting odds ratio and Bayesian Confidence Propagation Neural Network. Subgroup, sensitivity analysis, time-to-onset, Bradford Hill, network pharmacology, molecular docking, machine learning, and SHAP analy
