This case report describes a 63-year-old female patient with hypertension who developed drug-induced systemic lupus erythematosus (SLE) after hospitalization for syncope and severe hyponatraemia. Her complex medication regimen, including antihypertensives and proton pump-inhibitors, complicated the identification of a specific trigger. Positive anti-histone antibodies supported the diagnosis. Treatment with immunosuppressants and discontinuation of suspected drugs led to remission. This case highlights the importance of recognizing drug triggers in elderly patients with extensive medication us

BACKGROUND: Given the high prevalence of prescription drug use in the US population and the possible drug-related oral and dental adverse events (AEs), it is important to characterize drug-related oral and dental AEs from available data sources. The objectives of this study were to identify and characterize various drug-related oral and dental AE pairs reported in the literature and drug labeling.
TYPES OF STUDIES REVIEWED: A systematic descriptive review of the literature was performed after applying the search criteria in PubMed, Embase, and Web of Science. A full-text review of drug labeli

Hydralazine is a vasodilator typically used in the treatment of resistant hypertension and heart failure. N-acetyltransferase 2 (NAT2) catalyzes the metabolism of hydralazine into inactive metabolites. NAT2 poor metabolizers (historically referred to as "slow acetylators") are predicted to have increased plasma hydralazine concentrations compared with NAT2 rapid and intermediate metabolizers (historically referred to as "rapid acetylators" and "intermediate acetylators," respectively), which may lead to both increased clinical efficacy and adverse effects, including drug-induced systemic lupus

BACKGROUND: Drug-induced lupus erythematosus (DILE) is the development of lupus-like syndrome following a drug exposure. DILE has been reported less frequently among children than adults.
METHODS: In this study, we present four children with DILE and similar published cases through a systematic literature review.
RESULTS: We report four children (three girls and one boy) who developed DILE associated with the use of topiramate, doxycycline, etanercept, and ethosuximide. Three of them were positive for anti-histone antibodies. In all patients, the drug was discontinued and symptoms resolved c

Pediatric systemic lupus erythematosus (SLE) constitutes approximately 10% of SLE cases. The diagnosis and management of this condition remains to be a challenge in the pediatric population. The distinguishing clinical features are less pronounced male-to-female ratio, more organ damage, and higher disease activity compared to adult-onset disease. Drug-induced lupus erythematosus is rare in children. We report a case of drug-induced SLE in a child on antiretroviral therapy.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder characterized by the production of autoantibodies directed against nuclear and cytoplasmic antigens. SLE can be induced by various medications, such as hydralazine, procainamide, isoniazid, methyldopa, chlorpromazine, quinidine, and minocycline. Hydralazine-induced lupus syndrome was first reported in 1953, and only occurs in 5-10% of patients taking hydralazine. We present a case of a 76-year-old female with a past medical history of chronic kidney disease (CKD) stage II, hyperlipidemia, primary hypertension, and type II diab

Adalimumab is a tumor necrosis factor inhibitor that has been widely used since the early 2000s for the treatment of psoriasis and psoriatic arthritis. All tumor necrosis factor inhibitors have been identified as causative agents in drug induced lupus, particularly etanercept and infliximab. There have been few reported cases of systemic lupus erythematosus induced by adalimumab, many more are attributable to treatment with etanercept or infliximab. We present the case of a patient with long-standing psoriasis and psoriatic arthritis who was successfully treated with adalimumab, but developed

Drug-induced lupus erythematosus has features distinct from primary systemic lupus erythematosus. It can occur with a wide variety of agents that result in the generation of anti-histone or other types of antibodies. Systemic manifestations of drug-induced systemic lupus erythematosus may include renal dysfunction due to circulating immune complexes or due to other immune reactions to the culprit medication(s). Acute interstitial nephritis occurs due to DNA-drug or protein-drug complexes that trigger an allergic immune response. We report a patient who developed acute kidney injury, rash, and

Hydralazine-induced vasculitis is an established autoimmune entity but widely underdiagnosed despite its initial description in 1953. Hydralazine-induced lupus occurs in 5% to 10% of patients and is characterized by fever, arthralgias, rash, anemia, leukopenia, and positive antinuclear antibodies. Renal involvement is rare in drug-induced systemic lupus erythematosus but can occur. Hydralazine may also be associated with an antineutrophil cytoplasmic antibody to complicate the diagnostic evaluation. There are no definitive diagnostic criteria for hydralazine-induced lupus, and diagnosis relies

Drug-induced subacute cutaneous lupus erythematosus (SCLE) is the most common subtype of drug-induced systemic lupus erythematosus and has been associated with more than 100 drugs. It presents weeks to months after initiation of the culprit medication. The eruption is typically in a photodistribution and it is marked by positive serology to anti-Ro (SSA) antibody. Systemic 5-fluorouracil (5-FU) is a less-common culprit of drug-induced SCLE and its occurrence is likely dependent on exposure to ultraviolet light. Herein, we present a review of drug-induced lupus induced by the pyrimidine analog,

We report a patient presented with recurrent pericardial effusion caused by drug-induced systemic lupus Erythematosus (SLE) following mitral valve repair. The surgery was complicated by hemiparesis and convulsion in early postoperative period. The patient had been received carbamazepine for a paroxysmal seizure that occurred following mitral valve repair. The post operative computed tomography showed embolic stroke and its sequel (seizure) that treated with carbamazepine. In the 3rd month of follow-up, however, hemiparesis recovered by physiotherapy but carbamazepine was not discontinued as by

Drug-induced lupus erythematosus (DILE) is a less severe variant of systemic lupus erythematosus (SLE) that generally resolves within weeks or months after the withdrawal of the implicated drug. DILE is unusual during childhood, with the most frequent age of presentation being at 50-70 years of age. Among different drugs, most commonly procainamide and hydralazine have been implicated as a cause of DILE. However carbamazepine (CBZ) is considered a low-risk drug and very few cases have been reported in children. We describe the case of CBZ-induced SLE in a 9-year-old girl following 3 years of C

Due to their physiological function, the kidneys are exposed to high concentrations of numerous drugs and their metabolites, making them vulnerable to drug-related injuries. This article provides an overview of the pathophysiological mechanisms involved in nephrotoxicity, the most common nephrotoxic drugs, and the risk factors for the occurrence of drug-induced acute kidney injuries. NSAIDs, diuretics, ACE inhibitors, and angiotensin II receptor blockers (ARBs} are the most frequent prerenal causes of an acute elevation in creatinine levels. Primary vascular damage arises from thrombotic micro

Biological therapies are supposed to trigger the development of autoimmune diseases. We report a case of a 27-year old woman presenting with drug induced systemic lupus erythematosus (SLE) associated with infliximab therapy. The development of paradoxical inflammation in immune-mediated inflammatory diseases patients treated with anti TNF-α suggests that an unknown inflammatory pathway may be provoked by inhibiting TNF-α. We suppose that in our case a cross reactivity between anti-infliximab antibodies and autoantibodies may lead to the development of TNF-induced immune disease.

Histone H2B is a common target of autoantibodies in both spontaneous and drug-induced systemic lupus erythematosus (SLE). Recent studies demonstrate that Asp(25) of histone H2B (H2B) spontaneously converts to an isoaspartic acid (isoAsp) in vivo. Our laboratory has demonstrated that the posttranslational modification of an aspartic acid to an isoaspartic acid within self-peptides renders otherwise ignored peptides immunogenic. Analysis of serum from lupus-prone mice and histone antibody positive SLE patients revealed antibodies specific to the Asp and isoAsp H2B(21-35) peptide, and that the ex

The initial appearance of subacute cutaneous lupus erythematosus (SCLE) skin lesions in conjunction with Ro/SS-A autoantibodies occurring as an adverse reaction to hydrochlorothiazide [i.e. drug-induced SCLE (DI-SCLE)] was first reported in 1985. Over the past decade an increasing number of drugs in different classes has been implicated as triggers for DI-SCLE. The management of DI-SCLE can be especially challenging in patients taking multiple medications capable of triggering DI-SCLE. Our objectives were to review the published English language literature on DI-SCLE and use the resulting summ

Drug-induced systemic lupus erythematosus (SLE) is a rare complication of therapies with some drugs. Breaking out after months or years of therapy with a certain drug, its occurrence is likely to increase with the duration of the medication and the cumulative quantity of the drug. The symptoms of this syndrome include, in particular, arthralgia, myalgia, fever, serositis, skin exanthema and production of antinuclear (ANA) antibodies. In contrast to SLE, its symptoms gradually abate after discontinuation of the inducing agent. The authors describe the case of a 43-year-old patient suffering fro

We present a 40-year-old man with erythematous-to-violaceous, broken, reticulated patches on the upper chest, back, and extremities, which is consistent with livedo racemosa. The cutaneous findings appeared after an increase in dilantin dose and subsequently improved after a reduction in dilantin dose. Furthermore, antinuclear antibodies and antihistone antibodies were detected. We therefore believe that the livedo racemosa is a cutaneous manifestation of a drug-induced systemic lupus erythematosus. We review the distinctive features of livedo racemosa as well as its associations with several

The objective of this article is to identify common and atypical features of systemic lupus erythematosus diagnosed following hepatitis B vaccination. We analyzed retrospectively the medical records of 10 systemic lupus erythematosus patients from different centers, who developed the disease following hepatitis B vaccination and determined the prevalence of different manifestations and the time association to vaccination. In this case series, 80% of the patients were female, mean age 35 +/- 9 years, of which 20% received one inoculation, 20% received two doses and 60% received all three inocul
