BACKGROUND: Tuberculosis-HIV (TB-HIV) co-infection remains a major public health challenge in South Africa, particularly in rural settings where clinical, socioeconomic, and health-system factors may influence treatment outcomes. This study examined the epidemiological characteristics of TB-HIV co-infection and factors associated with tuberculosis treatment outcomes in a rural Eastern Cape cohort.
METHODS: A retrospective cohort study was conducted using routinely collected programmatic data from 422 adult tuberculosis patients treated between 2018 and 2022. Baseline demographic and clinical

Human immunodeficiency virus and tuberculosis (HIV/TB) coinfection remains a major global health challenge. Immune dysregulation in HIV complicates TB diagnosis. The type of immune response mounted in tuberculosis is a key determinant in deciding the outcome of the infection. Hence, estimating immune markers is critical for developing diagnostic, monitoring and treatment approaches. A study was conducted to evaluate the diagnostic potential of host-based biomarkers in individuals with HIV/TB coinfection in comparison to HIV monoinfection receiving antiretroviral therapy. Host-based biomarkers

Individuals living with HIV/AIDS may be more susceptible to various opportunistic infections due to an immunocompromised status, which can result in diverse clinical manifestations. Among potential pathogens, Mycobacterium colombiense-a slow-growing bacterium-could pose a significant threat to such patients. Here, we present the case of a 46-year-old woman with HIV/AIDS who developed an initial dark red plaque that gradually expanded, accompanied by persistent sinus tracts and suppuration. Interestingly, the patient reported no pain or itching. Further examination revealed no evidence of disse

BACKGROUND: Conventional culture methods for tuberculosis diagnosis miss differentially culturable tubercle bacteria (DCTB), which grow only in liquid assays supplemented with growth-enhancing factors. This limitation, combined with inadequate contact tracing and screening, often fails to identify asymptomatic individuals, with live bacilli detectable by enhanced culture methods. This shortfall results in undiagnosed reservoirs of bacteria, potentially fuelling ongoing transmission. In this study, we aimed to investigate whether DCTB assays provide greater sensitivity by detecting more Mycobac

BACKGROUND: Tuberculosis (TB) remains the leading cause of death from infectious diseases worldwide, and its control is increasingly complicated by chronic comorbidities. Diabetes mellitus (DM), human immunodeficiency virus (HIV) infection, chronic obstructive pulmonary disease (COPD), and lung cancer (LC) substantially affect TB susceptibility, diagnosis, treatment, and prognosis.
METHODS: This narrative review summarizes evidence on the interactions between TB and DM, HIV infection, COPD, and LC. Relevant literature was identified through PubMed, Web of Science, and World Health Organizatio

BACKGROUND: Hematogenous disseminated tuberculosis (Hematogenous disseminated tuberculosis, HDTB) is a rare, critical form of tuberculosis with a high case fatality ratio and is uncommon in HIV-negative patients. Early recognition of this disease is difficult, and limitations of traditional testing methods often lead to delayed diagnosis. This study aims to investigate the value of metagenomic Next-Generation Sequencing (metagenomic Next-Generation Sequencing, mNGS), as a promising tool, in the diagnosis of hematogenous disseminated tuberculosis in HIV-negative (Human Immunodeficiency Virus, H

BACKGROUND Idiopathic CD4 lymphocytopenia is a rare immunodeficiency syndrome characterized by a persistent reduction in CD4 lymphocyte count to fewer than 300 cells/µL or less than 20% of total T lymphocytes on multiple occasions. This diagnosis is made in the absence of HIV infection or other known immunodeficiencies. Herein, we present a case report of idiopathic CD4 lymphocytopenia in an adult patient, contributing to the limited literature on the management challenges of this rare immunodeficiency. CASE REPORT A 42-year-old male patient presented with shortness of breath, productive cough

Global tuberculosis (TB) incidence is slowly decreasing, partly because undiagnosed TB continues to fuel transmission. Point-of-care TB triage tests (TTTs) performed near a patient or at the site of patient care may help bridge this gap, but head-to-head comparisons of different host blood biomarkers within the same cohort are limited. In ambulatory adults in Uganda aged ≥15 years with any cough, fever, weight loss, or night sweats, we evaluated the diagnostic accuracy of C-reactive protein (CRP), monocyte-to-lymphocyte ratio (MLR), interferon-gamma-induced protein 10 (IP-10), and hemoglobin (

Despite advances in tuberculosis treatment and prevention, tuberculosis remains a global health threat, responsible for an estimated 1.23 million deaths in 2024 [46], with continued concern over drug-resistant strains. In the Western Cape, South Africa, co-infection of tuberculosis and HIV is a significant public health concern contributing to a higher burden of disease. We compared the transcriptomes of two genetically similar Beijing family strains of Mycobacterium tuberculosis under four different growth conditions, including oxidative stress, using three biological replicates for each isol

Mycobacterium avium (M. avium) is an opportunistic intracellular pathogen causing chronic pulmonary disease in immunocompromised individuals, particularly in those with HIV/AIDS. Current treatments present challenges due to duration, antibiotic resistance and toxicity, creating a need for novel treatments. Glutathione (GSH), a key intracellular antioxidant, is depleted in immunocompromised individuals. This study evaluated the direct and intracellular antimycobacterial activity of liposomal glutathione (L-GSH) against M. avium. Furthermore, the minimum inhibitory concentration (MIC) and depola

BACKGROUND: Nontuberculous mycobacteria (NTM) can disseminate and infect various organs throughout the body. However, whether NTM can infect tracheobronchial tissue is rarely reported. This study aimed to address the knowledge gap regarding the epidemiological, demographic, and clinical characteristics of patients with tracheobronchial NTM infection.
METHODS: In this multicenter retrospective cohort study, clinical, demographic, microbiological, and radiological data from hospitalized patients with tracheobronchial NTM infections from January 2015 to May 2025 were collected and analyzed descr

In this work, we propose an HIV-TB coinfection model that incorporates time since recovery from TB. We derive the basic reproduction numbers and the invasion reproduction numbers associated with the two infections, and investigate the existence as well as the local and global stability of the disease-free, HIV-only, and TB-only equilibria. Using data on new HIV and TB cases in China from 2005 to 2019, we estimate the optimal model parameters, and perform a sensitivity analysis of the invasion reproduction numbers. Theoretical analysis and numerical simulations reveal that coexistent equilibria

Multidrug-resistant tuberculosis (MDR-TB) remains a major cause of morbidity and mortality worldwide, with people living with HIV experiencing persistently poor treatment outcomes. Clofazimine and bedaquiline are cornerstone drugs in contemporary all-oral MDR-TB regimens, yet their complex pharmacokinetics and substantial interindividual variability complicate regimen optimisation, particularly in vulnerable populations. Robust plasma-based drug quantification is essential to characterise exposure-response relationships and inform individualised therapy. We developed and validated a rapid, sen

AIMS/BACKGROUND: Tuberculosis (TB) is a highly prevalent opportunistic infection and a leading cause of mortality worldwide, particularly among those living with human immunodeficiency virus (HIV). Among these patients, the diagnostic accuracy of conventional TB testing is generally limited by atypical clinical manifestations, the frequent occurrence of extrapulmonary TB, challenges in obtaining sputum samples, and consistently low bacterial loads in clinical specimens. Therefore, this study assesses the diagnostic performance of a novel chemiluminescence immunoassay (CLIA) for detecting urina

SCIENTIFIC BACKGROUND: Meningiomas are the most common benign intracranial neoplasms with a slow growth presented as the intracranial lesion. These tumors are without any symptoms for a long time. At the time of diagnosis it is frequently an asymptomatic tumor. In that case the therapist may well suggest a wait-and-see strategy. The therapy of meningiomas focuses firstly on the microsurgical treatment. Volume reduction can be achieved immediately after treatment. Stereotactic radiosurgery is an important non-invasive treatment option for recurrent tumors or meningiomas with partial resection.

BACKGROUND: Tuberculosis (TB) incidence peaks in women during their reproductive years and is a leading cause of maternal mortality. Pregnant women with TB have a high risk of failure to initiate TB treatment and poor TB treatment and pregnancy outcomes. We determined the time to treatment initiation in pregnant women diagnosed with TB in a routine programmatic setting.
METHODS: Using routine linked electronic data, we identified women 15-45 years of age with laboratory-confirmed and/or clinically diagnosed TB, October 2018-December 2020, in two high-burden sub-districts in Cape Town, South A

BACKGROUND: Drug-resistant tuberculosis (DR-TB) is associated with low treatment success rates and increased mortality. Household contacts (HHCs) face an elevated risk of infection and disease progression due to prolonged exposure. Consequently, systematic screening and tuberculosis preventive treatment (TPT) are critical to interrupt transmission within households. However, screening practices and TPT regimens vary substantially across countries and settings worldwide, and the available evidence remains fragmented. This study aims to synthesize evidence on screening strategies and TPT regimen

Tuberculosis is the most common cause of hospitalisation and death globally among people with HIV (PWH), even in the antiretroviral therapy era. Diagnosis of tuberculosis is challenging in PWH as bacillary load is lower in sputum and a high proportion have extrapulmonary or disseminated tuberculosis. The clinical features of tuberculosis in PWH are nonspecific and there is a wide differential diagnosis of other opportunistic infections. Tuberculosis progresses rapidly in people with advanced HIV disease, necessitating rapid diagnosis. In this narrative review, key changes are described in the
