Schistosoma mansoni co-infection in tuberculosis patients with or without human immunodeficiency virus and factors associated with treatment outcomes: a prospective cohort study.

BACKGROUND: Tuberculosis (TB) has resurfaced as the leading cause of death by an infectious agent after the COVID-19 pandemic. The fight against TB needs to consider factors associated with mycobacterial reactivation and treatment of TB disease. Recent findings showed that Schistosoma mansoni co-infection leads to a Th2/Th1 profile that results in an immune modulation that favors the escape of the Mycobacteria. Schistosoma mansoni may contribute to tuberculosis (TB) incidence in endemic regions. We aimed to determine the co-infection rate and treatment outcomes. METHODS: A prospective cohort s

QuantiFERON-TB gold plus in people living with HIV: independence from CD4+ T-cell counts in a low TB-burden setting.

BACKGROUND: Tuberculosis screening is recommended for people living with HIV. The QuantiFERON-TB test measures the cell-mediated response against M. Tuberculosis. The Gold-In-Tube measures the response of CD4+ T-cells, often leading to indeterminate results. The new Gold-Plus (QFT-GP) also measures CD8+ T-cells response, thus reducing uncertainties. However, studies on people living with HIV, that would benefit from a test independent from CD4+ T-cells, are scarce. OBJECTIVE: This study addresses this gap by evaluating the performance of QFT-GP specifically in a large cohort of people living

The utility of urine lipoarabinomannan as a tuberculosis treatment monitoring tool in people living with HIV.

OBJECTIVE: This pilot study aimed to evaluate whether urine lipoarabinomannan (LAM) testing can be used to monitor tuberculosis treatment response in people living with HIV. RESULTS: Seventy-five participants were enrolled, of whom 51 (68%) were women, and 43 (57.4%) were antiretroviral therapy naïve. CD4 cell counts were available for 49 participants with a median of 58 cells/mm3 (Interquartile range: 22-175). All participants initially tested positive for urine LAM; however, 12 (16%) converted to negative within two days of starting tuberculosis treatment. During the eight-week follow-up, 4

Feasibility and testing outcomes of task-shared implementation of advanced HIV disease point of care tests in Beira (Mozambique) and Kinshasa (DRC).

INTRODUCTION: Mortality from advanced HIV disease (AHD) remains high and current strategies to promptly test people eligible for AHD screening, are insufficient. Task sharing for point of care (POC) testing utilizing lay health workers (LHW) is recommended, however it is marginally practised in many countries. This study sought to describe the feasibility and testing outcomes of task-shared implementation of the AHD POC diagnostic tests utilizing LHW and professional health care workers (HCW). METHODS: This was a cross-sectional mixed-methods implementation study in seven primary and three se

Optimized Mass Spectrometry to Uncover M. tuberculosis Biomarkers in Extracellular Vesicles From Asymptomatic Tuberculosis Patients.

BACKGROUND: Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains the leading cause of death from a single infectious agent worldwide. Exposure to Mtb results in diverse outcomes: bacterial clearance, latent infection, asymptomatic, or symptomatic TB. Current diagnostic tools cannot reliably distinguish these outcomes. Our previous studies identified Mtb proteins in extracellular vesicles (EVs) from TB patients' serum, suggesting their potential as biomarkers. Here, we aimed to discover Mtb proteins and peptides in serum EVs across TB stages, focusing on asymptomatic individua

An economic evaluation of the LINKEDin study: An intervention to reduce initial loss to follow-up among tuberculosis patients in South Africa.

Tuberculosis (TB) patients who are diagnosed but not registered and initiated on TB treatment are categorised as initial loss to follow-up (ILTFU). ILTFU is a key driver of morbidity and mortality associated with TB and is a contributing factor to high TB transmission rates. LINKEDin was a quasi-experimental study which evaluated two specific interventions for reducing ILTFU in three high-TB burden provinces in South Africa, conducted from October 2018 to December 2020. As part of LINKEDin, we undertook a micro-costing from the healthcare provider perspective using an activity-based costing ap

Deciphering the different Mycobacterium avium complex infections of HIV and non-HIV patients by bacterial GWAS and immune cells flow cytometry.

PURPOSE: Pulmonary diseases caused by Mycobacterium avium complex (MAC) show species-specific epidemiology: M. avium predominates in HIV-positive patients, while M. intracellulare mainly affects immunocompetent hosts. This study aimed to elucidate the bacterial and host mechanisms underlying these differences. METHODS: We integrated clinical metagenomic next-generation sequencing (mNGS), k-mer-based bacterial genome-wide association study (GWAS) of clinical isolates, peripheral blood immunophenotyping of 175 patients, and mouse infection models with or without CD4 depletion. RESULTS: K-mer G

Start4All protocol for a Bayesian cost-effectiveness model of tuberculosis screening and diagnosis in seven high burden low-income and middle-income countries.

INTRODUCTION: High costs of screening and diagnostic tests remain a major barrier to timely tuberculosis (TB) identification in resource-limited settings. Evidence on the cost-effectiveness of scalable screening algorithms is limited. Start4All is a research project aimed at developing and evaluating algorithmic approaches to TB screening and diagnosis, with the goal of optimising technical and allocative efficiency when expanding diagnostic coverage to primary healthcare and community settings. METHODS AND ANALYSIS: Five screening and diagnostic tests will be evaluated: a capillary blood-bas

Treatment outcomes among tuberculosis patients with co-morbidities at Ghana's premier national referral centre: a retrospective cross-sectional study.

BACKGROUND: In Ghana, tuberculosis (TB) cases reached 19,000 in 2023 (15% above 2022). Co-morbidities such as human immunodeficient virus (HIV), diabetes, substance abuse, and hypertension can complicate treatment. The study evaluated co-morbidity prevalence among TB patients at a major Ghanaian hospital and their impact on treatment outcomes (treatment success, default, death). METHODS: A retrospective descriptive cross-sectional study at the Chest Clinic, Korle-Bu Teaching Hospital, Accra, reviewed 2021–2022 records of 993 TB patients of which 258 had ≥ 1 co-morbidity. Demographics, co-morbi

The impact of treatment accessibility on HIV-TB co-infection: a comparative fractional-order modeling approach.

HIV-tuberculosis (TB) co-infection remains a major global health challenge, especially in regions with limited treatment accessibility. This study presents a fractional-order mathematical model that captured the transmission dynamics of HIV-TB co-infection among treated and untreated populations, incorporating disease progression through multiple compartments. The model is examined using three distinct fractional derivative operators, the Caputo derivative (power-law kernel), the Caputo–Fabrizio derivative (non-singular kernel), and the Atangana–Baleanu derivative (Mittag-Leffler kernel) to as

Feasibility, challenges, and solutions for implementing targeted universal tuberculosis testing: perceptions of healthcare professionals in South Africa.

BACKGROUND: Tuberculosis (TB) remains a leading cause of mortality, particularly among people with HIV (PWH). In South Africa, the targeted universal TB testing (TUTT) strategy was introduced to shift from symptom-based to symptom-agnostic screening to improve early case detection in PWH. However, limited research has explored provider perceptions of the TUTT strategy. We explored healthcare professionals’ perceptions of the introduction, feasibility, challenges, and potential solutions for implementing TUTT. METHODS: We conducted a qualitative study using in-depth interviews with 11 purposive

Same-day versus rapid ART initiation in people with HIV and symptoms of tuberculosis (SaDAPT): a randomised, non-inferiority trial in Lesotho and Malawi.

BACKGROUND: Whether same-day antiretroviral therapy (ART) should be offered to people with HIV with tuberculosis symptoms or whether ART should be initiated only after tuberculosis diagnostic results are available is unknown. This randomised trial aimed to determine whether same-day ART initiation for people with tuberculosis symptoms is non-inferior to deferral until tuberculosis diagnostic results are available in achieving viral suppression at 6 months. METHODS: We did an open-label, individually randomised, non-inferiority trial among people with HIV who were outpatients at 11 primary or

Strategies to integrate non-communicable disease interventions in HIV and tuberculosis care contexts in low- and middle-income countries: a scoping review.

INTRODUCTION: Integration of management of tuberculosis (TB) and HIV with prevention and treatment of non-communicable diseases (NCDs) is a global priority. However, delivering the full spectrum of HIV/TB and NCD services is hindered by a lack of evidence regarding effective models and strategies for integrating NCDs and HIV/TB care services in varying contexts and across interventions. We conducted a scoping review to describe service delivery models and strategies used to facilitate integration of NCD care in HIV and/or TB care settings in low- and middle-income countries (LMICs). Risk Factors for Immunological Sensitization to Mycobacterium tuberculosis and Progression to Incident TB Disease Among HIV-uninfected Adults in a High Burden Setting.

BACKGROUND: Identifying risk factors for Mycobacterium tuberculosis (Mtb) sensitization (defined as interferon-gamma release assays [IGRA]-positive) and progression to tuberculosis (TB) disease is critical to guide targeted prevention strategies. METHODS: We analyzed data from a prospective cohort of adults (18-60 years) without HIV, enrolled at five high-incidence South African sites. Participants underwent testing for Mtb sensitization and microbiologically confirmed TB at baseline, and during 15 months follow-up. Multivariable logistic and Cox regression models were used to assess factors
