OBJECTIVES: To investigate the effect of warm needling moxibustion on chondrocyte senescence in rats with knee osteoarthritis (KOA), and to compare its efficacy with that of mild moxibustion.
METHODS: Forty-eight Wistar rats were randomly assigned to 4 groups (n=12):normal, model, mild moxibustion, and warm needling. KOA was induced by intra-articular injection of monosodium iodoacetate (MIA) into the right knee. Rats in the mild moxibustion and warm needling moxibustion groups received interventions at the acupoints "Futu" (ST32) and "Zusanli" (ST36), 30 min per session, once every other day

BACKGROUND: Superior sagittal sinus thrombosis (SSST) is a rare but life-threatening form of cerebral venous thrombosis. Although several prothrombotic and autoimmune conditions are recognized risk factors, SSST occurring in patients with rheumatoid arthritis (RA) receiving tumor necrosis factor-alpha (TNF-α) inhibitor therapy remains exceptionally rare and poorly described in the literature.
CASE PRESENTATION: We present a 59-year-old Caucasian female with RA treated with infliximab who developed extensive superior sagittal sinus thrombosis with concurrent intracranial and subarachnoid hemor

Autoantibody-mediated inflammation drives tissue injury in autoimmune diseases, yet how pathogenic antibodies evoke local immune activation remains poorly understood. Here, we show that spatially organized neutrophil-epithelial niches contribute to kidney injury in myeloperoxidase (MPO)-antineutrophil cytoplasmic antibody-associated glomerulonephritis. Single-cell-resolved spatial transcriptomics of treatment-naïve patient kidney biopsies identified early-phase inflammatory niches enriched with HAVCR1+ injured proximal tubular epithelial cells, neutrophils, lymphocytes, and TREM2+ macrophages.

Natural killer (NK) cells are innate lymphocytes, which are critical for the antitumor immunity response against tumour cells. However, the function of NK cells is frequently compromised in hepatocellular carcinoma (HCC). In this study, we found that CCAT1 expression level was reduced in peripheral blood (PB)-NK cells derived from HCC patients, compared to PB-NK cells derived from healthy controls. Reduced CCAT1 expression in NK cells was associated with poor disease-free survival and overall survival of HCC patients. Gain- and loss-of-function assays revealed that CCAT1 restored NK cell cytot

Rheumatoid arthritis is a chronic immune-mediated inflammatory disease, and therapeutic strategies predominantly rely on continuous immunomodulation. Despite major advances with biological and targeted synthetic disease-modifying antirheumatic drugs, some patients develop persistent, objectively active inflammatory disease that remains refractory to multiple therapeutic mechanisms. Durable drug-free remission is rare, highlighting a major unmet need. Emerging deep B-cell depleting therapies, including bispecific T-cell engagers and chimeric antigen receptor (CAR) T-cell-based therapies, challe

Adult-onset Still disease (AOSD) is a rare systemic autoinflammatory disorder characterized by quotidian fever, inflammatory arthritis, evanescent rash, neutrophilic leukocytosis, and hyperferritinemia. Because no pathognomonic diagnostic test exists, diagnosis relies on characteristic clinical features and exclusion of infectious, malignant, and autoimmune disorders. Viral infections, including cytomegalovirus (CMV), have been proposed as potential immunologic triggers in genetically susceptible individuals, although causality remains unproven. We report a 47-year-old Hispanic woman with hype

INTRODUCTION: Diagnosing amyloid arthropathy remains challenging because its manifestations often mimic those of other inflammatory musculoskeletal diseases. This review aimed to describe the clinical, biological, and imaging characteristics of musculoskeletal involvement in light-chain (AL) Amyloidosis.
METHODS: We conducted a review in accordance with the PRISMA guidelines. We searched SCOPUS and MEDLINE for case reports and series published between January 2000 and December 2023 that detailed musculoskeletal manifestations of AL amyloidosis. To minimize the risk of omitting relevant articl

OBJECTIVE: The "mucosal origins hypothesis" posits that mucosal sites, such as the gut, are initial sites where immune responses first develop that may, months to years later, result in clinical rheumatoid arthritis (RA). Our goal here was to conduct a discovery-based, hypothesis-generating study using samples from a relatively small number of patients to implicate previously unrecognized highly IgA-coated commensal flora that may have a role in RA pathogenesis (primary outcome). The secondary supporting outcomes were to determine whether HLA-DR-presented microbial peptides from such flora cou

Osteoarthritis (OA) is now widely discussed as a whole-joint immunometabolic disease involving cartilage, synovium, and subchondral bone, rather than as cartilage degeneration driven only by mechanical stress. Tissue injury, cell death, mitochondrial dysfunction, and extracellular matrix degradation can lead to the persistent release of damage-associated molecular patterns (DAMPs), which activate innate immune responses through pattern recognition receptors (PRRs). A key unresolved issue is why inflammation can persist or recur when there is no continuous external stimulus. Trained immunity pr

BACKGROUND: Clonal hematopoiesis (CH), characterized by clonal expansion of hematopoietic stem cells harboring preleukemic mutations, is an emerging cardiovascular disease risk factor. The American Heart Association's Life's Essential 8 (LE8) score is the contemporary standard for quantifying cardiovascular health. However, the interplay between this novel genetic risk factor and classical health metrics remains unknown.
OBJECTIVES: This study sought to examine the association between LE8 score and CH prevalence, and determine their joint effects on incident coronary artery disease (CAD).

Rheumatoid arthritis (RA) is an autoimmune disease marked by joint and systemic inflammation. Anticitrullinated protein antibodies identify an at-risk stage that precedes clinical RA, yet the mechanisms driving disease progression remain poorly understood. Here we show that anticitrullinated-protein-antibody-positive individuals who progressed to RA exhibited a marked expansion of CXCR5+CD69+-activated naive B cells (aNAVs) before disease onset. These cells displayed inflammatory and senescent-like transcriptional programs and expressed polyreactive, autoreactive IgM B cell receptors. The expa

Mesenchymal stromal cell (MSC) interactions with monocytes/macrophages are central to their therapeutic effects in knee osteoarthritis (KOA); however, mechanisms of these interactions are not fully understood. We hypothesize that MSC soluble factors, particularly interleukin-6 (IL-6) and C-C motif chemokine ligand (CCL2) modulate monocytes in KOA environment. Using healthy donor CD14+ monocytes exposed to KOA synovial fluid (SF) in the presence or absence of marrow-derived MSC(M) directly or conditioned medium (CM), we evaluated cell surface markers and signaling via signal transducer and acti

INTRODUCTION: Osteoarthritis (OA) is a prevalent chronic inflammatory disease in which elevated TNF-α and excessive reactive oxygen species (ROS) play central roles. These two pathological factors mutually reinforce each other-TNF-α induces ROS production while ROS upregulates TNF-α expression-forming a vicious cycle that propels disease progression. However, current therapies offer only symptomatic relief without halting this process. To address this, we developed polydopamine-poly(L-lysine) (PDA-PLL) nanoparticles as a dual-functional platform that combines PDA-mediated ROS scavenging with P

BACKGROUND: Although antigenemia is associated with poor outcomes in respiratory viral infections, the underlying mechanisms driving viral bloodstream dissemination remain not fully understood. Herein, we aimed to characterise SARS-CoV-2 nucleocapsid (N)-antigenemia kinetics in patients with severe COVID-19. We also sought to identify host and viral determinants associated with viral dissemination through a multiparametric integrated analysis and assess the associated risk of death.
METHODS: This observational study included hospitalised patients with severe COVID-19 across three independent

Pseudorabies virus (PRV), the causative agent of Aujeszky's disease in pigs, threatens swine production and can infect other mammals, with rare zoonotic cases reported. Effective antiviral agents against PRV remain lacking. Here, we constructed and purified a recombinant fusion protein combining porcine Interleukin-2 (IL-2), interferon α (IFN-α), and Thymosin α1 (Tα1). The fusion protein showed no cytotoxicity or induction of apoptosis in PK-15 cells. In vitro, it significantly enhanced peripheral blood mononuclear cell (PBMC) metabolic activity and upregulated expression of key interferon-sti

BACKGROUND: Continuous positive airway pressure (CPAP) is the primary treatment for obstructive sleep apnoea (OSA), although its effectiveness may vary across OSA subtypes. This joint European Respiratory Society (ERS)/European Sleep Research Society (ESRS) clinical practice guideline evaluates CPAP's impact on patient-centred and prognostically relevant outcomes, comparing it to no or sham treatment in different OSA subgroups.
METHODS: The joint ERS/ESRS Task Force consisted of experts in pulmonology, cardiology and respiratory sleep medicine, patient representatives and methodologists. Ques

BACKGROUND: Newly diagnosed multiple myeloma (NDMM) is characterized by a spatially heterogeneous tumor burden. Whole-body magnetic resonance imaging (WB-MRI)-derived apparent diffusion coefficient (ADC) histogram analysis may enable comprehensive characterization of tumor burden. This study aimed to investigate the feasibility of using whole-tumor ADC histogram parameters based on WB-MRI images to assess tumor burden and evaluate their prognostic value in NDMM patients.
METHODS: There were 95 NDMM patients categorized into focal lesion and whole-tumor groups in this retrospective study. Corr

BACKGROUND: Osteoarthritis (OA) is increasingly recognized as a chronic inflammatory disease, with dysregulated immune responses contributing to cartilage degeneration and disease progression. Immune dysregulation, including alterations in Th1-associated responses, may contribute to the chronic inflammatory environment observed in osteoarthritis. MSC-derived exosomes have been highlighted as potential cell-free therapeutic agents due to their potent immunomodulatory properties; however, their effects on Th1-associated immune responses in OA remain poorly understood.
