Association of Spondyloarthritis With Chronic Opioid Use in Inflammatory Bowel Disease: A Nationwide Danish Cohort Study.

IMPORTANCE AND OBJECTIVE: Chronic opioid use may increase the risk of complications in patients with inflammatory bowel disease (IBD). Rheumatologic illness has been identified as a risk factor for chronic opioid use in IBD, but the specific influence of spondyloarthritis (SpA) on this risk is unclear. DESIGN, SETTING, AND PARTICIPANTS: This nationwide cohort study included adults diagnosed with Crohn's disease (CD) or ulcerative colitis (UC) between 1999 and 2021 in Denmark. EXPOSURES: We compared IBD patients with SpA diagnosed within 3 years before or after the IBD diagnosis (ie IBD+SpA)

Current Landscape of Molecular Diagnostic Tests and Emerging Tools for Tuberculosis and Drug Resistance.

This review synthesizes recent advances in WHO-endorsed NAATs for TB diagnosis and drug resistance detection. We examine the principles, genetic targets, diagnostic performance, implementation settings, and programmatic role of assays across the spectrum of technological complexity, from cartridge-based platforms to line probe assays and sequencing-based technologies. In addition, we highlight emerging molecular tools that show promise for future WHO endorsement and may further strengthen decentralized and near-patient testing. Low- and moderate-complexity assays such as Xpert® MTB/RIF, Xpert®

Intradiscal Injection of Adinizer-Processed Minimally Microfragmented Adipose Tissue for Degenerative Lumbar Spondylolisthesis: A Case Report With a Two-Year Follow-Up.

Degenerative lumbar spondylolisthesis is a common cause of chronic low back pain and functional limitation in older adults. Although conservative treatment is generally considered first-line management, some patients continue to experience persistent symptoms, and surgery may not always be feasible or acceptable. We report the two-year clinical course of a 72-year-old woman with symptomatic degenerative lumbar spondylolisthesis who underwent intradiscal injection of Adinizer-processed minimally microfragmented adipose tissue (MFAT). She had longstanding chronic low back pain for approximately

Evidence-Based Conservative Treatment Strategies for Lumbar Radiculopathy: A Systematic Review

Lumbar radiculopathy is a prevalent and debilitating spinal condition that causes severe pain, functional impairment, and diminished quality of life. Although conservative treatments are widely recommended as first-line management, establishing their comparative effectiveness remains challenging due to substantial heterogeneity in the literature. This systematic review synthesized and critically appraised the evidence evaluating non-surgical, conservative interventions for lumbar radiculopathy. Adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 gui

Does surgical treatment of grade 2 spondylolisthesis have sustained improvement in postoperative outcomes regardless of age? A Spine CORe™ analysis of QOD data.

OBJECTIVE: Surgical intervention for grade 2 lumbar spondylolisthesis is routinely performed, but outcomes in older patients, in whom the disease is most prevalent, remain poorly understood. The aim of this study was to compare patient-reported surgical outcomes between age groups (< 65 years of age vs ≥ 65 years of age) with 5 years of follow-up. The authors hypothesized that patients would have sustained improvement in outcomes in response to surgical treatment for grade 2 spondylolisthesis regardless of age. METHODS: The multicenter prospectively collected Quality Outcomes Database by the

Relationships among mechanisms in psychosocial treatments for chronic pain: mechanism to mechanism lagged effects and relationships with outcomes.

Results suggest that psychosocial treatments for chronic pain work via several mechanisms, and that they often do so to similar degrees and in similar ways. Extant research, however, has focused on individual and/or independent effects of mechanisms on outcomes. Whether successful outcomes are also partly because of sequential and meaningful relationships among and between mechanisms-mechanism-to-mechanism effects-has not been examined. Secondary analyses were conducted of an RCT that compared cognitive therapy, mindfulness-based stress reduction, and behavior therapy to treatment as usual in

En Bloc Spondylectomy for T11 Metastasis of Nasal Adenocarcinoma: A Case Report.

En bloc spondylectomy resection (EBR) has emerged as a potential option for achieving tumor-free margins and enhancing long-term outcomes in select cases of solitary spinal metastases, though the role of this treatment in metastatic spine disease remains debated. We present the case of a 49-year-old female with a prior history of adenoid cystic carcinoma of the nose who developed back pain two years later. Positron emission tomography-computed tomography (PET-CT) revealed a solitary T11 lesion, and a CT-guided biopsy confirmed metastasis. After meticulous surgical planning guided by the Weinst

Multivalent DNA vaccines induce potent cellular responses and prolong BCG-mediated control of Mycobacterium tuberculosis.

Mycobacterium tuberculosis (Mtb) remains a leading public health concern worldwide. While Bacillus Calmette-Guerin (BCG) vaccination is effective at protecting infants/children, efficacy wanes over time. Advancements in synthetic DNA technology have resulted in the delivery of structurally complex antigens that elicit robust, multifaceted cell-mediated immunity in humans. Using this platform, we evaluated the comparative immunogenicity and protective capacity of four multivalent plasmid DNA (pDNA) constructs that together encode a diverse set of 23 potential TB antigens. All cassettes were imm

Impact of Multiple Operative Levels on Radiographic Subsidence in Stand-Alone Lateral Lumbar Interbody Fusion.

OBJECTIVE: Lateral lumbar interbody fusion (LLIF) is a useful minimally invasive technique for arthrodesis, yet subsidence is still a significant postoperative concern. This study aims to evaluate the impact of the number of operative levels on subsidence rates following LLIF. METHODS: Patients who underwent LLIF procedures between 2014 and 2022 at 2 quaternary referral centers were reviewed. Collected variables included age, sex, body mass index, smoking status, number of operative levels, cage material, average operative level bone density (Hounsfield units), and length of imaging follow-up

Larger Lumbar Vertebral Body Osteophyte Volume Is Associated With Older Age and a More Caudal Vertebral Level in Individuals With Chronic Low Back Pain.

BACKGROUND: Chronic low back pain (cLBP) is often accompanied by changes in lumbar spine bony morphology, including osteophyte growth. Knowledge of the volume and location of osteophytes could provide insight into habitual loading that may induce symptomatic disc degeneration and serve to guide treatment. The aim of this study was to develop an automated method to calculate osteophyte volume in lumbar vertebral bodies and to evaluate the relationships between osteophyte volume and vertebral level, age, and sex. METHODS: Patients with cLBP received computed tomography (CT) scans of their entir

Rheumatoid arthritis susceptibility genes show pathotype-specific expression in synovial tissue of patients with early treatment-naïve RA.

OBJECTIVES: Rheumatoid arthritis (RA) exhibits clinical and biological heterogeneity, with synovial tissue stratified into histological pathotypes: lympho-myeloid, diffuse-myeloid, and pauci-immune fibroid. Although genome-wide association studies (GWAS) have uncovered RA risk loci, how genetic risk relates to synovial immunopathology remains unclear. To better understand how genetic predisposition may shape divergent early disease mechanisms, we characterised the expression patterns of GWAS-identified RA susceptibility genes and related rheumatic diseases across the synovial pathotypes. Long TB: Framing post-acute sequelae of tuberculosis as a distinct clinical entity.

Tuberculosis (TB) has historically been declared cured at the point of microbiological clearance. Yet emerging evidence from systematic reviews and cohort studies demonstrates that between 30% and 80% of pulmonary TB survivors experience persistent symptoms, organ damage or functional impairment after treatment completion; a burden that existing outcome metrics fail to capture. We propose the term Long TB, with the formal designation Post-Acute Sequelae of Tuberculosis (PAST-TB), as a conceptual and clinical framework to define, study and address this under-recognised morbidity. Drawing explic

Influence of cage material, plating and vertebral bone quality on subsidence and long-term outcomes following anterior cervical discectomy and fusion-a retrospective analysis.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a surgical procedure used to treat degenerative conditions of the cervical spine. Cage subsidence is a known post-operative complication, resulting in loss of indirect decompression, deformity, and radicular symptoms. The objectives of this study are to: (I) investigate associations between cage-plate constructs and postoperative subsidence after single-level ACDF; (II) determine relationships between cervical vertebral bone quality (C-VBQ) scores, subsidence, and fusion construct; and (III) identify potential risk factors that may
