Comparative effects of non-specific exercise therapies on adolescent idiopathic scoliosis: a systematic review and network meta-analysis.

OBJECTIVE: Non-specific exercise therapies are widely used in the conservative management of adolescent idiopathic scoliosis (AIS) because of their accessibility and broad applicability. However, the comparative effectiveness of different non-specific exercise modalities remains unclear. This study aimed to systematically review the available evidence and compare the effects of different non-specific exercise therapies on AIS using network meta-analysis. METHODS: PubMed, Embase, Web of Science, the Cochrane Library, CNKI, Wanfang, and VIP databases were searched from inception to September 20

Learning from the unseen: synthetic data for scoliosis AI segmentation.

PURPOSE: Severe scoliosis remains challenging for 3D vertebral segmentation because annotated pathological computed tomography (CT) data are scarce. METHODS: We synthesize scoliosis-like CT volumes by jointly warping public healthy spine scans and labels and then train nnU-Net CNN and PRIMUS models under matched compute. We evaluate on in-house out-of-distribution (OOD) adolescent scoliosis CTs and the public Spine1K dataset. RESULTS: On 18 in-house adolescent scoliosis CTs, the best CNN setting increased mean Dice from 0.765 to 0.842 and vertebra success rate from 73.2 to 81.6%. In 55 surge

The LL-TK index as a regional marker of sagittal balance and disability in adults with chronic low back pain.

PURPOSE: Population aging has increased both the prevalence and clinical impact of adult spinal deformity (ASD). Sagittal and regional parameters (e.g., SVA, PI-LL, LL-TK) guide assessment and treatment planning, but it remains unclear which indices are most closely associated with sagittal imbalance and functional disability. This study therefore evaluated the associations between regional and global spinopelvic parameters and clinical status in an adult outpatient cohort with chronic low back pain, and compared the ability of the LL-TK and PI-LL indices to discriminate concurrent sagittal im

Axis fractures: historical systems and advancements with the AO spine classification.

STUDY DESIGN: Systematic Review. BACKGROUND: A variety of C2 (axis) and C2/3 historical classification systems have been proposed but there still remains a need for a rational hierarchical means of characterizing these traumatic spinal injuries which involve the axis. We evaluate the utility of existing classification systems and discuss the evolution of the validated AO Spine Upper Cervical Injury Classification (UCIC) System. METHODS: A comprehensive search of MEDLINE, EMBASE and Cochrane Databases was performed in keeping with the Preferred Reporting Items for Systematic reviews and Meta-

Sagittal alignment following a modified Jackson technique in the surgical management of high-grade spondylolisthesis.

PURPOSE: The optimal surgical management of paediatric high-grade spondylolisthesis (HGS) remains controversial. Instrumented fusion in situ (IFIS) is considered low risk for neurological complications at the expense of limited correction of spinopelvic imbalance. This study evaluated radiographic outcomes of IFIS using a modified Jackson technique, with a focus on sagittal alignment and the evolution of spinopelvic balance over time. METHODS: A retrospective review of 19 patients (< 18 years, mean age 13.7) with ≥ grade 3 HGS treated with IFIS using a modified Jackson technique between 2006

Single Prone Position, Dual Surgeon Simultaneous Navigated Prone Lateral Interbody Placement with Endoscopic Laminectomy for Symptomatic Immobile Spondylolisthesis.

Symptomatic immobile spondylolisthesis with severe circumferential stenosis consisting of foraminal, lateral recess, and central canal narrowing secondary to disc height loss and facet arthropathy typically may require direct decompression followed by fixation given the need for complete facetectomies to adequately release exiting nerve roots. Recent evidence suggests that standalone grafts may offer a favorable risk profile for indirect decompression in cases with appropriate spinal alignment and normal bone density. A 54-year-old female presented with a one-year history of neurogenic claudic

Rethinking sagittal alignment: the value of evaluating spinal shape in conjunction with T4-L1 pelvic angle measurement to prevent proximal junctional failure.

OBJECTIVE: The T4-L1 pelvic angle (T4-L1PA) has been proposed as a novel alignment target associated with mechanical complications in long-segment adult spinal deformity (ASD) surgery. However, its relationship to proximal junctional failure (PJF) and its relevance in shorter thoracolumbar constructs remain unclear. The aim of this study was to evaluate whether T4-L1PA and L1 pelvic angle (L1PA) guidelines predict PJF in ASD surgery and to examine how early proximal reciprocal changes influence this relationship. METHODS: A prospective ASD database was reviewed for patients who underwent fusi

O-arm Navigation-assisted Extreme Lateral Interbody Fusion Utilizing SureTrak Device.

Currently, intraoperative guidance during conventional extreme lateral interbody fusion relies primarily on fluoroscopy, which can expose both surgical staff and the patient to radiation. The use of O-arm navigation during extreme lateral interbody fusion and percutaneous pedicle screw fixation is anticipated to enhance accuracy and substantially reduce radiation exposure. To date, there have been no published studies investigating the application of O-arm navigation in extreme lateral interbody fusion procedures. Thirty-three extreme lateral interbody fusion and percutaneous pedicle screw fix
