Electroacupuncture-augmented breaststroke vs. breaststroke alone for mild AIS: superior correction in a retrospective cohort.

BACKGROUND: In China, acupuncture is commonly used for adolescent idiopathic scoliosis (AIS), but it requires combination with other therapies. In clinical practice, orthopedic surgeons often recommend swimming for mild AIS patients with Cobb angle <25°. However, owing to the paucity of high-quality evidence and standardized protocols, the efficacy of swimming for AIS remains controversial. The objective of this study was to evaluate the efficacy and safety of breaststroke combined with electroacupuncture vs. breaststroke alone in skeletally immature patients with mild AIS. METHODS: This was

Redefining Proximal Junctional Kyphosis After Posterior Spinal Fusion for Idiopathic Scoliosis: PJA >12 Is Associated With Increased Pain and Decreased Mobility, But Not Reoperation.

INTRODUCTION: Proximal junctional kyphosis (PJK) occurs at moderate rates following posterior spinal fusion (PSF) in idiopathic scoliosis (IS). However, the impact of preoperative sagittal alignment and the clinical consequences of PJK, particularly effects on Patient-Reported Outcome Measurement Information System (PROMIS) scores, remain underreported. METHODS: A query of an institutional pediatric spinal deformity database identified patients undergoing primary PSF for IS with a minimum 2-year follow-up. Preoperative and latest follow-up radiographs, as well as PROMIS scores, were analyzed.

Outcomes after surgical correction of severe scoliosis in patients with osteogenesis imperfecta: a prospective, 2-year minimum follow-up study with radiographic and patient-reported outcomes.

PURPOSE: To evaluate both radiographic and patient-reported outcomes after surgical correction of severe scoliosis in osteogenesis imperfecta (OI) patients with minimum 2-year follow-up. The main research question was whether modern multimodal surgical approaches translate into meaningful improvements in both deformity correction and quality of life measures. METHODS: This retrospective analysis of prospectively collected data included 35 OI patients (mean age 12.2 ± 3.7 years) who underwent posterior spinal fusion for scoliosis at a single institution between 2013 and 2022. OI type 3 was mos

Minimum 5-Year Clinical and Radiographic Results of Double-Sided Vertebral Body Tethering Applied to Double Adolescent Idiopathic Scoliosis Curves of the First 13 Patients: Important Lessons Learned.

BACKGROUND: Double-sided VBT (dsVBT) (bilateral application to thoracic and lumbar major curves) was reported to provide successful thoracic and thoracolumbar curve corrections in skeletally immature adolescent idiopathic scoliosis (AIS) patients when applied under strict inclusion criteria in the short term. The present prospective cohort study aimed to present at least 5 years' results from the same study population to question the reliability of dsVBT in terms of correctional efficacy, growth modulation, and long-term functional outcomes. METHODS: Thirteen consecutive skeletally immature (

Case reports: Pulmonary destructive lesions caused by ankylosing spondylitis.

BACKGROUND: Ankylosing spondylitis (AS) frequently produces thoracic-cage deformities-kyphosis and rib fusion-that restrict chest expansion and impair ventilation, occasionally progressing to atelectasis or pulmonary fibrosis. Such pulmonary manifestations are often misdiagnosed as tuberculosis or fungal disease. CASE PRESENTATION: We describe two AS patients with bilateral upper-lobe destruction whose initial chest CT scans suggested tuberculosis. After thorough microbiological work-up remained negative, both were diagnosed with AS-associated lung injury (ASLI) and referred to rheumatology.

The use of hooks in upper instrumented vertebra to prevent proximal junctional complications: a systematic review and meta-analysis.

BACKGROUND: Proximal junctional kyphosis (PJK) and proximal junctional failure (PJF) are leading complications after adult spinal deformity (ASD) surgery. Proximal hooks at the upper instrumented vertebra (UIV) may reduce stiffness transition and PJK/PJF risk, but comparative evidence is limited. This systematic review and meta-analysis evaluates the association between hook use and PJK/PJF incidence. METHODS: A comprehensive literature search was conducted using PubMed, Cochrane Library, and SpringerLink from 2000 to 2025. Studies related to the use of hooks versus other fixation methods (e.g

Lateral armrest support improves postural alignment, respiratory mechanics, and shoulder girdle loading during seated eating in healthy young adults: a multidimensional biomechanical evaluation.

BACKGROUND: Postural collapse and increased reliance on accessory respiratory muscles during meals can compromise ventilatory efficiency, particularly in individuals with respiratory impairment. Forward elbow-supported sitting is commonly used to unload the upper limbs; however, this posture often induces trunk flexion and cervical extension, which may adversely affect respiratory mechanics. To date, the effects of lateral armrest support on posture and respiratory function during seated eating have not been quantitatively investigated. OBJECTIVE: To examine the effects of lateral armrest sup

Frailty, Bone Health, and Radiographic Predictors of Complications After Long-Segment Fusion for Adult Spinal Deformity.

BACKGROUND: Surgery for adult spinal deformity (ASD) can result in complications such as proximal junctional kyphosis (PJK), pseudoarthrosis, hardware failure, and surgical site infection. This study sought to identify specific risk factors for these complications in ASD patients. METHODS: Patients undergoing long-segment thoracolumbar fusion (>4 levels) for ASD between 2016 and 2021 were included. Univariate and multivariable analyses assessed preoperative risk factors for complications. A subgroup analysis of patients undergoing ≥7-level fusion constructs was also performed. RESULTS: Among

Cervical Paraspinal Muscle Morphometry and Mechanical Failure: A Propensity-Matched Analysis.

OBJECTIVES: Hardware-related complications are rare but important issues after posterior cervical fusion. Current risk stratification relies on bone quality parameters, but other variables may also contribute to mechanical complications. In lumbar spine surgery, paraspinal muscle morphometry has emerged as a predictor of mechanical failure, though cervical-specific evidence is limited. This study evaluates preoperative cervical paraspinal muscle morphometry in predicting mechanical failure following posterior cervical fusion. METHODS: This retrospective propensity score-matched study included

Comparison of Surgical Outcomes Between Vertebral Body Stenting (VBS) and Balloon Kyphoplasty (BKP)-Multicenter Cohort Study.

Background/Objectives: Vertebral body stenting (VBS) and balloon kyphoplasty (BKP) are widely used for the treatment of osteoporotic vertebral fractures (OVFs). However, it remains unclear whether the theoretical biomechanical advantages of VBS translate to superior clinical or radiographic outcomes. This study aimed to compare VBS and BKP with respect to clinical outcomes, radiographic parameters, and complications. Methods: In this multicenter retrospective comparative cohort study, 123 patients with OVF treated with VBS (n = 24) or BKP (n = 99) were analyzed. VBS was indicated for complex f

Paraspinal muscle degeneration patterns in degenerative spinal deformity: a histological, morphological, and muscle functional comparison.

BACKGROUND: Degenerative spinal deformity (DSD) is an umbrella term for three-dimensional (3D) spine malalignment. Paraspinal muscle (PSM) degeneration has been widely implicated in the pathology and progression of DSD. However, little is known about the differences in the degeneration patterns of PSM in deformity subtypes. This study aimed to explore the degeneration patterns of PSM in DSD subtypes. METHODS: A total of 65 patients with degenerative lumbar scoliosis (DLS), 57 with degenerative lumbar kyphosis (DLK), and 62 patients with lumbar spinal stenosis (LSS) were included. Hematoxylin

The correlation between loss of cervical lordosis and the severity of ligament ossification in patients with cervical ossification of posterior longitudinal ligament: a cross-sectional observational study.

BACKGROUND: Both genetic and environmental (e.g., mechanical stress) factors have been considered multifactorial components contributing to the development and progression of ossification of posterior longitudinal ligament (OPLL). Importantly, loss of cervical lordosis may change the mechanical stress on posterior longitudinal ligament. However, few studies have discussed the relationship between the cervical sagittal alignment and OPLL. The aim of this study is to quantify the cervical sagittal alignment and severity of ligament ossification in patients with cervical OPLL and to analyze their

Expandable corpectomy cage with articulating endcap for correction of severe cervical kyphotic deformity: a case report.

BACKGROUND: The treatment of cervical kyphotic deformities is challenging. In select cases, anterior corpectomy is required to obtain adequate correction. Most contemporary anterior column reconstruction involves titanium mesh cages or fixed-angle expandable systems, but there are limitations to those techniques. In this case, we present the use of an expandable titanium cage with an articulating endcap to provide controlled correction of a severe cervical kyphotic deformity. CASE DESCRIPTION: A 63-year-old male with a history of C4-C7 decompression and fusion done elsewhere presented with se

VEPTR treatment of early onset scoliosis in children without rib abnormalities: 10-year follow-up from a prospective multicenter study.

PURPOSE: In 2007, a prospective study on VEPTR treatment of EOS in children without rib abnormalities was initiated. Two-year and 5-year follow-up results from this cohort have previously demonstrated that scoliosis is controlled, and spinal growth continues. This study examines whether, at long-term follow-up, VEPTR continues to control scoliosis and allow spinal growth. METHODS: Prospective, multicenter cohort of participants with EOS without rib abnormalities who underwent VEPTR. Pre-implantation and last available images were compared, regardless of whether VEPTR remained in vivo. RESULT
