Three-Dimensional Analysis of Disc and Vertebral Height and Their Role in Kyphosis Creation in Adolescent Idiopathic Scoliosis.

STUDY DESIGN: Retrospective. OBJECTIVE: Describe three-dimensional (3D) changes after adolescent idiopathic scoliosis (AIS) reconstruction, with attention to anterior column (AC) shortening and middle column (MC) lengthening. SUMMARY OF BACKGROUND DATA: Relative elongation of the AC, particularly the disc, is a common feature of AIS. 3D correction of deformity requires creation of thoracic kyphosis (TK). METHODS: An AIS registry was queried for patients treated with posterior-only instrumented fusion, with pre/postoperative biplanar radiographs and 3D spine models. MATLAB script calculated

Assessing radiological factors: the role of pelvic tilt in surgery for geriatric patients with degenerative spinal disorders.

BACKGROUND: Adult Degenerative Spinal Disorders (ADSD) pose significant clinical challenges, especially among elderly population. Currently, the decision-making process regarding surgical intervention primarily relies on the severity of clinical symptoms. However, there is no well-established standard for determining which patients are appropriate candidates for surgery. In addition, there is a noticeable lack of research focusing on the radiological assessment of patients to determine the necessity for surgical intervention. Hence, this study aims to explore potential influencing factors for

Anterior release is not needed to restore kyphosis in moderate AIS with hypokyphosis.

PURPOSE: The purpose of this study is to evaluate if AR offers improved 3D kyphosis restoration during PSF for hypokyphosis in moderate AIS (< 70° coronal cobb), where the decision for AR is likely driven by sagittal concerns. METHODS: A multicenter pediatric spine registry was queried for hypokyphotic (< 10°) Lenke 1-4 AIS patients aged < 20 years with > 2-year surgical follow-up. Coronal curves were limited to < 70°. A linear mixed model was created to predict 2-year 3D kyphosis by treatment and pre-op 3D kyphosis, while controlling for age, sex, thoracic coronal deformity and flexibility,

Multicenter validation of a surgical planning tool for lumbar vertebral body tethering simulating growth modulation over 2 years.

PURPOSE: Vertebral body tethering (VBT) for lumbar curves may have wider application than for thoracic curves due to greater growth potential than thoracic spine and benefits of preserved flexibility. Predicting long-term correction remains challenging, with high revision rates and complications (14-32%) including under-/over-correction, tether breakage, adding-on. This study aimed to validate a planning tool for lumbar VBT using a patient-specific finite element model (FEM) integrating mechanobiological growth modulation as a function of preoperative skeletal maturity. METHODS: Thirty-five r

Emerging MRI-based spine scoring techniques targeting bone quality to assess osteoporosis, vertebral fracture risk, other spinal degenerative diseases, and post-surgical outcomes.

Osteoporosis is a metabolic skeletal disease defined by reduced bone mass and a higher risk of vertebral fractures. Due to the increasing elderly population worldwide, it is considered a major healthcare challenge for now and the future. Magnetic resonance imaging (MRI), a non-radiation modality, is emerging as an opportunistic tool for assessing osteoporosis using T1-weighted images. While extensive research has been conducted in this area, a unified and comprehensive review encompassing current knowledge, methodologies, diagnostic efficacy, and prospective research direction is still needed.

Rod options to outcomes: comparing sagittal correction in pediatric posterior spinal fusion by rod size and material.

PURPOSE: Our study aims to evaluate the impact of rod diameter and material on sagittal plane correction and reoperation. We hypothesize larger diameter rods would improve the sagittal plane alignment without increasing complications or progression of proximal junctional kyphosis (PJK). METHODS: Data were reviewed from consecutive series of Lenke 1-4 AIS patients enrolled in a multicenter registry between 2010 and 2019 with minimum 2-year follow-up. Patients who underwent PSF with 5 common rod types (5.5 or 6.0 cobalt chrome, 5.5 stainless steel, 5.5 or 6.0 titanium rods) were evaluated. Othe

Whole Blood Multi-OMIC Analysis Is Effective in Clinical Interpretation of Splicing Aberrations in PLOD1 -Related Kyphoscoliotic Ehlers-Danlos Syndrome.

Kyphoscoliotic Ehlers-Danlos syndrome (kEDS) Type 1 is a rare hereditary connective tissue disorder due to biallelic deleterious variants in PLOD1 and is mainly characterized by hypotonia, congenital kyphoscoliosis, eye fragility, hyperextensible skin, Marfanoid habitus, and joint hypermobility. In PLOD1-kEDS, deleterious variants are typically loss-of-function alleles. Therefore, the identification of private non-canonical splicing variants might deserve functional studies to refine their clinical interpretation. In a 7-year-old boy with congenital hypotonia, kyphoscoliosis, joint hypermobili

Percutaneous Reduction and Fixation for Traumatic Thoracolumbar Vertebral Fracture Using a Monoaxial Screw System.

BACKGROUND AND AIM: The development of percutaneous pedicle screws (PPSs) has led to the development of minimally invasive spine stabilization (MISt) procedures, which has decreased the invasiveness of spine surgery. Conventionally, PPSs were available as polyaxial screws only, making it technically challenging to perform percutaneous repair of thoracolumbar vertebral fractures with implants. This study aimed to evaluate percutaneous posterior fixation for thoracolumbar vertebral fractures using a monoaxial screw system and determine imaging and clinical outcomes until after implant removal.

Abdominal wall tension after spinal deformity correction compromises postoperative biomechanics and may contribute to proximal junctional kyphosis.

PURPOSE: Proximal junctional kyphosis (PJK) is a common complication after fusion surgery for adult spinal deformity (ASD). Tissue adaptation to deformity may cause abdominal wall shortening/stiffening. This study evaluated, using musculoskeletal modeling, the effect of these adaptations and sarcopenia on trunk muscle effort required to maintain postoperative alignment in PJK patients versus controls. METHODS: ASD patient data was grouped by mechanical complication status: PJK (N=44), other (N=56), none (N=260). Spinopelvic landmarks were annotated in pre-op, post-op, and follow-up X-ray imag
