Pedicle subtraction osteotomy: a comprehensive analysis in 104 patients. Does the cause of deformity influence the outcome?

OBJECTIVE The clinical outcomes and complications of patients who underwent pedicle subtraction osteotomy (PSO) for various diagnoses were compared. More specifically, the purpose was to identify if outcomes differed between patients with flat-back syndrome after lumbar fusion (FBS-LF) versus patients who underwent surgery for adult spinal deformity (ASD). METHODS A retrospective analysis of 104 patients who underwent a PSO for sagittal plane imbalance was performed. There were 28 patients with FBS-LF and 76 patients with various forms of ASD. Outcome was measured using visual analog scale (VA

Can Posterior Lumbar Instrumentation and Fusion Be Overpowered by Anterior Lumbar Fusion With Hyperlordotic Cages? A Cadaveric Study.

STUDY DESIGN: Technical report on cadavers. OBJECTIVE: To evaluate preliminary feasibility and safety of lumbar sagittal alignment correction with anterior hyperlordotic cages used to overpower previous posterior spinal instrumentation. METHODS: Hyperlordotic 30° anterior lumbar interbody fusion (ALIF) cages were inserted in collapsed L5-S1 disc space of 2 cadavers to overpower prior posterior L5-S1 pedicle screws and rod constructs. A distinct technique of opening up the disc space and creation of intersegmental lordosis was employed using a large endplate distractor and transforaminal lumb

Case Presentation of Sagittal Balance.

Sagittal balance is an important concept in spinal coronal and kyphotic deformity correction. Sagittal alignment/lumbar lordosis is increasingly recognized and discussed in terms of surgical outcomes. Positive sagittal balance can lead to development of iatrogenic kyphosis, flatback syndrome, adjacent level degeneration, and junctional kyphosis. This concept is no longer associated only with deformity. Spine surgeons have learned that sagittal alignment/lumbar lordosis is very important even in 1-level or 2-level interbody fusion procedures, and that it can be attained, maintained, or lost dur

Flatback Revisited: Reciprocal Loss of Lumbar Lordosis Following Selective Thoracic Fusion in the Setting of Adolescent Idiopathic Scoliosis.

STUDY DESIGN: Retrospective review of prospective multicenter adolescent idiopathic scoliosis (AIS) database. OBJECTIVE: To investigate the relationship between iatrogenic loss of thoracic kyphosis (TK) after selective thoracic posterior spinal instrumentation and fusion (PSIF) for AIS with straightening of lumbar lordosis (LL). SUMMARY OF BACKGROUND DATA: Segmental PSIF has become the standard of care for surgical treatment of severe AIS. Studies show that adults with flattening of TK and LL can develop pain and dysfunction associated with flatback syndrome. Analysis of post-fusion sagittal

The influence of sagittal spinal deformity on anteversion of the acetabular component in total hip arthroplasty.

The interaction between the lumbosacral spine and the pelvis is dynamically related to positional change, and may be complicated by co-existing pathology. This review summarises the current literature examining the effect of sagittal spinal deformity on pelvic and acetabular orientation during total hip arthroplasty (THA) and provides recommendations to aid in placement of the acetabular component for patients with co-existing spinal pathology or long spinal fusions. Pre-operatively, patients can be divided into four categories based on the flexibility and sagittal balance of the spine. Using

[Evaluation of the sagittal profile in patients with thoracic adolescent idiopathic scoliosis Lenke type 1 following posterior correction].

INTRODUCTION: The principle philosophy of posterior spinal instrumentation and fusion (PSIF) for the treatment of adolescent idiopathic scoliosis (AIS) has changed during recent decades. In the past the treatment of AIS mainly focused on correction of the major curve in the frontal plane while the sagittal profile and balancing were only of inferior interest in treatment planning. Various long-term outcome studies have demonstrated that many AIS patients developed a flatback syndrome (decrease of thoracic kyphosis and lumbar lordosis) associated with pain. It was concluded that treatment of AI

Biomechanical analysis of a disc prosthesis distal to a scoliosis model.

STUDY DESIGN: Biomechanical study of bovine spines. OBJECTIVE: The purpose of this study was to perform a biomechanical test to analyze intervertebral deflections following placement of both 1 and 2 semiconstrained TDRs in the subjacent segments of a long fusion. SUMMARY OF BACKGROUND DATA: Long-term sequela of long lumbar fusion for scoliosis include adjacent segment disease and flatback syndrome. Total disc replacement (TDR) is a viable option for the treatment of these conditions. Little data has been published regarding the placement of a TDR distal to a scoliosis fusion. METHODS: Six t

Flatback syndrome.

Flatback syndrome is characterized by loss of normal lumbar lordosis, resulting in forward tilt of the trunk, inability to stand erect, back pain, and thigh pain from chronic hip flexion and knee bending. The usual etiology is iatrogenic, through previous fusions or with extension instrumentation. Surgical treatments described include extension osteotomy (Smith-Petersen), pedicle subtraction osteotomy, and polysegmental osteotomies.

Prevention and management of iatrogenic flatback deformity.

The most common cause of iatrogenic flatback syndrome is Harrington distraction instrumentation extending into the lower lumbar spine. Other common causes and exacerbating factors include failure to enhance regional lordosis during lumbar fusion for degenerative spondylosis, development of pseudarthrosis or postoperative loss of correction, development of kyphosis at the thoracolumbar junction, development of degeneration and decompensation cephalad or caudad to a prior fusion, and hip flexion contractures. Prevention of flatback syndrome involves preoperative assessment of sagittal balance, a

Treatment of symptomatic flatback after spinal fusion.

Fifty-five patients who had loss of lumbar lordosis after spinal fusion and subsequently had corrective osteotomies were studied. When they were first seen, fifty-two patients (95 per cent) were unable to stand erect and forty-nine (89 per cent) had back pain. The previous use of distraction instrumentation with a hook placed at the level of the lower lumbar spine or the sacrum was the factor that was most frequently identified as leading to the development of the flatback syndrome. Sixty-six extension osteotomies were performed in these fifty-five patients. Nineteen patients (35 per cent) had
