Growth, infection, and humoral immunity in children who are HIV exposed and uninfected.

BACKGROUND: Children who are HIV-exposed uninfected (HEU) show greater morbidity and mortality than HIV-unexposed children (HUU). In this study we investigate sex differences in growth, infection rates and antibody response among HEU and HUU infants. METHODS: The study enrolled 107 pregnant women with HIV and 103 pregnant women without HIV with follow-up of their infants from birth to 12 months of age. Study measures assessed included growth parameters, the prevalence of children with overt disease symptoms as reported by the mother, PCR-based assessment of infections (cytomegalovirus (CMV),

Metagenomic next-generation sequencing of cerebrospinal fluid reveals pathogen spectrum and mortality predictors among patients with advanced HIV-1 disease at a tertiary hospital in China.

BACKGROUND: Central nervous system (CNS) infections remain the major causes of morbidity and mortality among people living with HIV-1 (PLWH), particularly in resource-limited settings. However, the clinical characteristics and prognostic indicators of PLWH with suspected CNS infections are not well defined. In this study, we aim to characterize the spectrum of CNS pathogens, clinical characteristics, in-hospital mortality, and factors associated with death among people with advanced HIV-1 disease (AHD) in Guangxi, China. METHODS: Metagenomic next-generation sequencing (mNGS) was performed to

Screening of Rubella Virus, Cytomegalovirus, Hepatitis B Virus, Hepatitis C Virus, HIV, Syphilis, and Toxoplasma gondii Antibodies in Pregnant Women.

TORCH pathogens are often asymptomatic in healthy adults but can cause foetal death when transmitted during pregnancy; therefore, accurate regional data are essential for screening. This study aimed to determine first-trimester TORCH seropositivity and to inform the development of hospital-based and national screening algorithms. This study analysed test results from 7481 pregnant women aged 15-49 years who participated between January 2020 and December 2024. TORCH serological results obtained using the MAGLUMI X3/X6 system (Snibe, Shenzhen, China) were analysed with Statistical Package for th

Congenital Infections as Risk Factor For Adverse Birth Outcomes Among HIV-Exposed Neonates in Uganda.

BACKGROUND: Prematurity and low birth weight (LBW) are leading risk factors for morbidity among HIV-exposed uninfected (HEU) neonates. The IMPAACT PROMISE 1077BF trial found high rates of adverse pregnancy outcomes among women living with HIV (WLHIV) randomized to triple antiretroviral treatment compared to zidovudine alone. We sought to elucidate the possible association of select congenital infections with preterm delivery (PTD) and low birth weight (LBW) among Women Living with HIV (WLHIV) on ART. METHODS: This study was designed as 1:2 nested case-control study of Ugandan mother/infant pa

Commensal bacteria inhibit viral infections via a tryptophan metabolite.

Clinical outcomes following viral exposures exhibit substantial interindividual variability. Although developing evidence suggests commensal bacteria modulate viral infections, the specific bacteria and mechanisms remain underexplored. Here, we define a pathway by which viral infections are inhibited by specific tryptophan-catabolizing bacteria. Using HIV as a model, we bioinformatically associated and experimentally validated several bacterial species that inhibited viral replication. This activity required the aromatic amino acid aminotransferase (ArAT) to metabolize tryptophan into 3-indole

A Clinical Review of Congenital and Perinatal Infections: From Womb to Follow-Up.

The acronym "TORCH" was originally introduced in 1974 by Dr Nahmias to collectively describe the most common pathogens responsible for congenital and perinatal infections, including Toxoplasma gondii, others, rubella virus, cytomegalovirus (CMV), and herpes simplex virus. With advances in diagnostic capabilities and expanded epidemiologic surveillance, the list of recognized congenital pathogens has grown to include syphilis, varicella-zoster virus, Zika virus, HIV, parvovirus B19, and lymphocytic choriomeningitis virus. While the TORCH acronym continues to be widely used in clinical and educa

Socio-Demographic and Prenatal Care Factors Associated with TORCH Screening During Pregnancy in Romania: A Cross-Sectional Study.

Background: Congenital infections included in the TORCH complex remain an important cause of fetal and neonatal morbidity and mortality, being associated with miscarriage, intrauterine growth restriction, congenital malformations, neurological impairment, and long-term developmental sequelae. Prenatal serological screening may contribute to the early identification of maternal infections and facilitate preventive and therapeutic interventions. However, data regarding the utilization of TORCH screening and associated socio-demographic determinants in Romania remain limited. Objective: This stud

The impact of cytomegalovirus coinfection on tuberculosis in the mouse model.

BACKGROUND: Tuberculosis (TB) remains a major global health challenge, with 10.6 million new cases and 1.3 million deaths annually. HIV coinfection is a well-known risk factor in high-burden regions, but emerging evidence suggests that other viral infections, including cytomegalovirus (CMV), may also impact TB pathogenesis. CMV is a ubiquitous herpesvirus capable of lifelong persistence, and its potential role in modulating TB progression remains unclear. METHODS: In this study, we established a murine coinfection model in C57BL/6 mice to investigate the effects of latent murine CMV (MCMV) on

Pathogenic characteristics of central nervous system infections in AIDS individuals: a retrospective cohort study based on immune status.

BACKGROUND: Human immunodeficiency virus (HIV) infection frequently leads to central nervous system (CNS) complications, especially as immunity declines. While antiretroviral therapy (ART) has transformed HIV into a manageable condition, in clinical practice, a significant subset of individuals still present with advanced immunosuppression (CD4 < 200 cells/μL) due to delayed diagnosis, treatment failure, or poor adherence, leading to AIDS-defining conditions. This study investigated how immune status affects CNS infection characteristics and outcomes specifically within this immunocompromised

Maribavir is a novel benzimidazole riboside compound that is currently in clinical development with ViroPharma for the prevention of cytomegalovirus (CMV) infections in transplant patients. Maribavir is thought to inhibit viral DNA assembly and inhibit egress of viral capsids from infected cell nuclei. This mechanism is different to other available CMV treatments that inhibit CMV DNA polymerase. Maribavir, originally developed by GlaxoSmithKline, was licensed to ViroPharma in August 2003. Glaxo was conducting phase I/II clinical studies in the EU and the US in 2001, but discontinued developmen
