Multi-pathogen serosurveillance reveals correlated vaccination performance, waning tetanus immunity, and diphtheria boosting in Zambian children.

BACKGROUND: Vaccination coverage estimates and case-based surveillance have limitations in evaluating immunization programs. Serosurveillance offers a complementary approach by directly measuring population immunity. We assessed whether serologic analyses across multiple antigens (i.e., measles, diphtheria, tetanus) could provide additional insights into vaccination program performance. METHODS: We conducted a matched case-control study among children aged 2- to 10-years-old (n = 1286) in Zambia using specimens from the 2016 ZAMPHIA survey. Using previously generated data on measles serostatu

Incidence and Outcome of Hepatitis D Virus Infection in People With HIV and Hepatitis B Virus Coinfection in the Era of Tenofovir-Containing Antiretroviral Therapy.

BACKGROUND: Tenofovir-containing antiretroviral therapy (ART) improves survival in hepatitis B virus (HBV)-coinfected people with HIV (PWH). We investigated the incidence of hepatitis D virus (HDV) infection and its clinical impact in HBV-coinfected PWH in the era of tenofovir-containing ART. METHODS: Between 2011 and 2022, HBV-coinfected PWH were included and followed until December 2023. Anti-HDV antibody screening was performed using sequentially archived blood samples. Timing of incident HDV infection was estimated as the midpoint between the last timepoint of anti-HDV-negative samples an

Hepatitis C virus transmission among people who inject drugs in rural United States: mathematical modeling study using stochastic agent-based network simulation.

People who inject drugs (PWID) account for the majority of hepatitis C virus (HCV) infections in the United States. The injection-equipment-sharing network likely plays an important role in shaping the dynamics of HCV transmission. Recognizing the emerging HCV epidemic in rural communities, we developed an agent-based network simulation model of HCV transmission via injection equipment sharing and used data on rural PWID networks to inform model parameterization and calibration. We then simulated an array of networks that varied key network properties to understand their impact on the magnitud

Factors Affecting Risk of Cardiovascular Disease (CVD) Events in a Global CVD Prevention Cohort of People With HIV.

BACKGROUND: Persons with HIV face increased risks of major adverse cardiovascular events (MACE), partially mitigated by statin therapy. METHODS: We characterized factors associated with MACE and MACE subcomponents among PWH enrolled in REPRIEVE, globally. Our primary outcome measure was time-to-first primary MACE. Secondary outcome measures included time-to-first (1) hard MACE (cardiovascular disease [CVD] death, myocardial infarction [MI], or stroke), (2) MI, or (3) stroke. For each outcome, Cox proportional hazards models were used to estimate the hazard of baseline risk factors. RESULTS:

Effect of Pitavastatin on Epigenetic Aging Biomarkers in People With HIV: Pilot Substudy of the REPRIEVE Trial.

BACKGROUND: People with human immunodeficiency virus (HIV, PWH) exhibit increased cardiovascular disease (CVD) risk and accelerated biological aging. REPRIEVE demonstrated that pitavastatin reduced major adverse cardiovascular events (MACE) in antiretroviral therapy (ART)-treated PWH with low-to-moderate traditional cardiovascular risk. It remains unknown whether statin therapy can modulate epigenetic aging in PWH. METHODS: We assessed epigenetic aging biomarkers using DNA methylation profiles from peripheral blood mononuclear cells (PBMCs) in a subset of 99 randomly selected US REPRIEVE part

Phase 2 Trial of Long-Acting Cabotegravir and VRC07-523LS for Viral Suppression in Adults With HIV-1: ACTG A5357.

BACKGROUND: Long-acting regimens are needed to expand antiretroviral therapy (ART) options for people with human immunodeficiency virus type 1 (HIV-1). Combining broadly neutralizing antibodies (bNAbs) with long-acting small-molecule antiretrovirals may offer an alternative to daily oral therapy. METHODS: We conducted a phase 2, open-label, single-arm trial at AIDS Clinical Trials Group (ACTG) sites across the United States. Eligible adults had HIV-1 virologically suppressed on ART for ≥2 years, CD4 count ≥350 cells/μL, and susceptibility to VRC07-523LS (half-maximal inhibitory concentration

Protracted SARS-CoV-2 Infection in B-cell Depleted Patients: Immunologic and Viral Characteristics and Response to Dual and Extended Antiviral Therapy.

BACKGROUND: Immunocompromised patients remain at risk for protracted severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections with persistent viral shedding that could pose a wider public health risk. The optimal therapeutic strategy remains unknown. METHODS: We describe a sequential case series of immunocompromised adults with protracted SARS-CoV-2 infection who received dual/extended antiviral therapy (median 23d nirmatrelvir/r; 8d remdesivir). Protracted infection was defined as persistent viral shedding and prolonged symptoms unresponsive to antiviral monotherapy in B-cell-

Low Trauma Fractures in People With HIV: Longitudinal Time Trends in the Swiss HIV Cohort Study, 2009-2022.

BACKGROUND: People with human immunodeficiency virus (HIV) are at increased risk of low trauma fractures (LTFs). Published data on LTF incidence trends over time have not been uniform. This study sought to analyze LTF time trends in the Swiss HIV Cohort Study (SHCS) over the time period 2009-2022. METHODS: Fractures are prospectively captured in the SHCS. Since 2008, using a standardized form, each fracture and its low trauma nature was validated by the treating HIV physician and the main investigators. Applying negative binomial regression, we estimated the LTF incidence rate ratio per calen

Clinical Consequences of Delaying Implementation of Long-Acting Antiretroviral Therapy for People With HIV and Persistent Viremia in the United States.

BACKGROUND: People with HIV (PWH) with persistent viremia and adherence challenges to oral antiretroviral therapy (ART) can achieve viral suppression (VS) with long-acting cabotegravir/rilpivirine (LA-CAB/RPV). The US guidelines, however, recommend CAB/RPV only in limited situations. We projected the impact of delaying LA-CAB/RPV implementation while awaiting trial data. METHODS: Using a microsimulation model, we considered 2 approaches for PWH with persistent viremia and intermittent care engagement: daily first-line oral ART or LA-CAB/RPV, both with intensive-support-services (ISS) to maxim

In the Mismatch: Viral Suppression Despite Low Tenofovir-Diphosphate in Dried Blood Spots in the Multicenter AIDS Cohort Study/Women's Interagency HIV Study Combined Cohort.

BACKGROUND: Tenofovir-diphosphate (TFV-DP) in dried blood spots (DBS) is an objective drug concentration measure predictive of future viremia in virally suppressed persons with human immunodeficiency virus (PWH) on TFV-based antiretroviral therapy (ART). Viral load (VL):TFV-DP mismatch (suppressed VL despite low TFV-DP concentrations) may increase future viremia risk. Previously, Black race, male sex, and higher body mass index were associated with lower TFV-DP concentrations among PWH receiving tenofovir disoproxil fumarate. This study assessed mismatch rates in PWH on tenofovir alafenamide (

Fostemsavir Decreases the Levels of Anti-gp120 CD4-Induced Antibodies in Heavily Treatment-Experienced People With HIV.

BACKGROUND: Heavily treatment-experienced (HTE) people with HIV (PWH) have limited antiretroviral therapy (ART) treatment options, putting them at greater risk of unfavorable human immunodeficiency virus 1 (HIV-1) disease progression. Fostemsavir (BMS-663068) efficiently suppressed viremia and increased CD4 count when combined with other antiretrovirals in HTE PWH. Its active metabolite, temsavir (BMS-626529, GSK2616713), binds a conserved pocket within the envelope glycoprotein (Env) CD4 binding site and prevents CD4-induced conformational changes. Temsavir effect on Env conformation profound

Safety and Immunogenicity of a Severe Acute Respiratory Syndrome Coronavirus 2 Spike Subunit Vaccine Stabilized in the Prefusion Conformation by a Second-Generation Molecular Clamp and Evaluated in Adults Aged 18-55 Years: A Randomized, Double-Blind, Active Comparator, Phase I Trial.

BACKGROUND: A coronavirus disease 2019 (COVID-19) subunit vaccine developed with the molecular clamp platform was among the first to enter clinical trials in 2020. While the vaccine was well suited to large-scale manufacture and demonstrated a favorable safety and immunogenicity profile, it did not progress into further clinical testing due to the human immunodeficiency virus 1 (HIV-1)-derived sequence in the molecular clamp, interfering with certain point-of-care HIV-1 diagnostic tests. A second-generation molecular clamp (MC2) has since been developed. Here, we describe a phase I clinical tr
