Osteoporosis Screening Gaps Among Patients Undergoing Lumbar Fusion Surgery.

BACKGROUND: Osteoporosis is associated with elevated complication rates after lumbar fusion surgery. Despite this, osteoporosis remains both underscreened and undertreated in the United States. Currently, rates of dual-energy radiograph absorptiometry (DEXA) screening before lumbar fusion surgery are unknown, as is the proportion of patients who, after being diagnosed with osteoporosis, receive appropriate treatment. Therefore, the aim of this study was to assess osteoporosis screening and treatment rates among patients undergoing lumbar fusion surgery. METHODS: Patients older than 50 years u

Avoidance of vascular injury during trans-psoas approaches; awareness of venous anatomy on MRI.

PURPOSE: Iliolumbar venous anatomy shows many variations between individuals, which can potentially be hazardous during lumbar spine surgery. The iliolumbar veins and any large variants can be observed on routine pre-operative lumbar spine MRI. Knowledge of the venous anatomy is important during lumbar spinal surgery, to avoid any potential haemorrhagic complications. This is particularly relevant for anterior lumbar surgery and lateral trans-psoas surgery. METHODS: In this study, we assessed the venous anatomy on the pre-operative MRI of 144 patients who underwent trans-psoas lateral access

Compact Navigation-Assisted Transpedicular Approach Using Unilateral Biportal Endoscopy for Caudally Migrated Lumbar Disc Herniation.

INTRODUCTION: Severely migrated lumbar disc herniations are challenging to access using conventional interlaminar or transforaminal approaches, especially in revision cases. A transpedicular route provides direct access but raises concerns regarding accuracy, safety, and radiation exposure when fluoroscopy is required. In this Technical Note, we describe a transpedicular unilateral biportal endoscopic (UBE) approach assisted by a compact navigation system for a caudally migrated disc herniation, highlighting its feasibility and potential advantages. TECHNICAL NOTE: A 73-year-old man with a hi

Age at Onset Predicts Motor and Non-motor Severity in Cervical Dystonia.

BACKGROUND: Cervical dystonia (CD) is the most common focal dystonia and shows significant heterogeneity in both motor and non-motor symptoms. Although motor symptoms typically prompt clinical presentation, non-motor features are the principal determinants of quality of life. Previous efforts to classify CD have focused on selected motor and non-motor clinical characteristics, suggesting distinct subtypes with potential prognostic implications. OBJECTIVE: We employed cluster analysis to identify phenotypic subgroups within a large CD population with detailed motor and non-motor assessments.

Predicting Failure to Rescue After Surgery for Degenerative Cervical Myelopathy: An ACS-NSQIP Analysis (2015-2020).

BACKGROUND CONTEXT: Failure to rescue (FTR), defined as 30-day mortality among patients who experienced a major complication, has emerged as a quality metric in spine surgery. Recent FTR analyses report high discrimination using prediction models combining pre- and postoperative variables into a single model. Whether this performance reflects true preoperative risk stratification or the inclusion of peri-mortality events as predictors remains unknown. PURPOSE: To identify predictors of FTR in patients undergoing surgery for degenerative cervical myelopathy (DCM) and compare the discrimination

Regional Variations in Surgical Approach and Perioperative Outcomes in Adult Cervical Instrumentation Surgery: A Nationwide Registry Study from the JSIS-DB.

INTRODUCTION: Cervical spinal surgery is performed for various pathologies, including degenerative disease, deformity, trauma, tumor, and infection. However, the surgical approach (anterior vs. posterior) and perioperative safety may vary among regions due to differences in patient characteristics, surgeon experience, and healthcare infrastructure. This study aimed to clarify regional variations in approach selection and perioperative outcomes for cervical surgery in Japan using the nationwide registry of the Japanese Spinal Instrumentation Society Database (JSIS-DB). METHODS: This retrospect

Impact of Age at Surgery on Perioperative Outcomes After Posterior Spinal Fusion for Neuromuscular Scoliosis.

BACKGROUND: While surgical outcomes for posterior spinal fusion (PSF) for pediatric patients with neuromuscular scoliosis (NMS) have been well described, less is known about how perioperative outcomes differ when surgery is performed later, during young adulthood. The purpose of this study was to compare perioperative outcomes between adolescents and young adults undergoing PSF for NMS using a large, multi-institutional clinical registry. METHODS: A retrospective cohort study was performed using the National Surgical Quality Improvement Program (NSQIP) and NSQIP-Pediatric databases from 2016

Open Lumbar Spine Fracture-Dislocation in a Rural Setting Associated With a Fall From Height: A Case Report.

Open spinal fractures are rare and highly complex injuries associated with a high risk of infection and neurological impairment. We report a case of a previously healthy 25-year-old male who sustained a fall from approximately four meters in a rural setting, with direct impact of the lumbar region against a toilet, resulting in an extensive wound contaminated with feces, urine, soil, and ceramic fragments. Upon admission, he presented with an approximately 15-cm lacerated and contused wound, disruption of the paravertebral fascia, and exposure of the posterior spinal elements. Computed tomogra

Endoscopic Decompression for Lateral Recess Stenosis Due to Vertebral Compression Fracture: A Case Report.

STUDY DESIGN: Case presentation. OBJECTIVE: The interlaminar endoscopic lateral recess decompression (IE-LRD) represents a novel surgical alternative to consider in carefully selected cases, particularly when patients do not respond to conservative treatment. BACKGROUND: Lumbar spinal stenosis (LSS) constitutes one of the most prevalent spinal conditions in the elderly population around the world. A myriad of causes have been elucidated, and although infrequent, vertebral compression fractures (VCF) may also cause LSS. Conservative treatment might be the first option; however, patients often

Mind the gap: discrepancies between planned and achieved screw trajectories in 3D-navigated spine surgery.

OBJECTIVE: Intraoperative 3D image-guided navigation has improved the accuracy of posterior screw placement in spine surgery. However, discrepancies between planned and achieved screw trajectories remain underexplored. In this study, authors assessed these deviations across spinal regions and their potential correlation with screw misplacement. METHODS: They prospectively analyzed patients undergoing posterior instrumented fusion surgery between 2021 and 2023. Screw placement accuracy was graded using the 2-mm increment basis of the Gertzbein and Robbins (GR) classification. Additionally, ang

Quantitative Assessment of Postoperative Lumbar Morphology Using Geometric Morphometrics: Within-Subject Changes Following TLIF and Decompression.

Background: Conventional radiographic assessment of postoperative lumbar alignment relies predominantly on Cobb angle measurements, which incompletely characterize global spinal morphology. We evaluated whether geometric morphometrics (GM) provides complementary quantitative information for assessing postoperative lumbar morphology after TLIF and decompression. Methods: Ninety-nine patients (TLIF n = 70; decompression n = 29) underwent landmark-based GM analysis of pre- and post-surgical lateral lumbar radiographs (48 landmarks, L1-S1). Because TLIF and decompression patients were imaged at ma

Determinants of Thoracic Reciprocal Change and Its Relationship to Proximal Junctional Kyphosis Following Lower Thoracic Fusion for Lumbar Flatback Deformity.

BACKGROUND: Lumbar flatback deformity (LFD) is characterized by the loss of lumbar lordosis (LL) and global sagittal malalignment, often necessitating thoracolumbar corrective surgery. Lumbar correction induces adaptive changes in unfused thoracic segments, known as thoracic reciprocal change (RC); however, its independent clinical significance remains debatable. This study aims to identify factors associated with thoracic RC after lower thoracic fusion for LFD and to evaluate its independent effect on the development of proximal junctional kyphosis (PJK). METHODS: This retrospective study in
