Three-dimensional finite element analysis of translaminar lag screw designs for L5 bilateral spondylolysis.

PURPOSE: This finite element study aimed to quantitatively compare the biomechanical properties of translaminar lag screws with different diameters and structural designs for the surgical repair of L5 bilateral spondylolysis, to provide evidence for individualized screw selection. METHODS: An intact model of L4-S1 segment (Model A) and a L5 bilateral spondylolysis model (Model B) were established using Mimics, Geomagic, and SolidWorks. Additional models were constructed incorporating translaminar lag screws of varying diameters: 4.5 mm (solid, Model C1; cannulated, Model C2), 4.0 mm (solid, M

Direct pars repair techniques for lumbar spondylolysis: a comparative literature review.

BACKGROUND/AIM: Lumbar spondylolysis, a pars interarticularis defect affecting approximately 3%-6% of the general population, presents a significant clinical challenge in young adults and athletes. When conservative management fails, direct pars repair offers an attractive alternative to spinal fusion by preserving segmental motion and reducing the risk of adjacent segment degeneration. This narrative review compares seven principal direct pars repair techniques-Buck's technique, Scott's wiring method, Morscher's hook-screw system, pedicle screw-based constructs, the V-rod/U-rod (Gillet techniq

Countermovement jump force-time waveforms in male adolescent footballers with growth-related musculoskeletal pathology.

INTRODUCTION: Growth-related musculoskeletal pathology is common in adolescent athletes, although associated changes in countermovement jump force-time characteristics remain poorly understood. This exploratory repeated-measures biomechanical analysis examined whether adolescent footballers with current or historical growth-related pathology, including Osgood-Schlatter disease (OSD) and prior pars interarticularis fracture, displayed limb-specific differences in double-leg countermovement jump (DL-CMJ) force-time waveforms not evident using discrete performance metrics alone. METHODS: Five ma

Pedicle screw vs. pedicle screw-rod-hook direct repair for adolescent lumbar spondylolysis: a multicenter retrospective cohort study.

PURPOSE: To compare clinical outcomes, time to fusion, and post-fusion segmental ROM between pedicle screw (PS) direct repair and pedicle screw-rod-laminar hook (PSRH) repair in adolescents/young adults with non-spondylolisthetic lumbar spondylolysis. METHODS: We included patients aged 15-25 years treated from 2015 to 2022 at three centers. All patients had predominant low back pain, failed conservative treatment, no neurological symptoms, and no vertebral slip. The defect was decorticated and grafted with allograft. Patients underwent PS or PSRH repair. CT was obtained at 6, 12, 18, 24, and

Clinical and radiographic outcomes of robotic-assisted direct pars repairs for spondylolysis.

BACKGROUND: Spondylolysis is one of the most common identifiable causes of low back pain in young athletes. Direct pars repair offers a motion-preserving surgical option, and robotic-assisted minimally invasive surgery may reduce invasiveness and improve precision compared to traditional open techniques. METHODS: We performed a single-center retrospective review of patients with symptomatic spondylolysis who underwent robotic-assisted direct pars repair from 2023 to 2025. A reference array was placed on the spinous process of the affected vertebrae through a < 2 cm midline incision, and screw

The value of "Black-Bone" MR imaging in pediatric spondylolysis: impact on diagnostic confidence and accuracy in experienced and non-experienced readers.

INTRODUCTION: Lumbar spondylolysis is a common cause of low back pain in adolescents, yet early and confident diagnosis often requires CT, exposing patients to radiation. The Black-Bone (BB) MR sequence is a 3D low-flip-angle gradient-echo (GRE) acquisition designed to enhance bone-soft tissue contrast. This study assessed whether adding BB to a standard pediatric lumbar MRI protocol improves diagnostic accuracy, interreader-agreement, and confidence. METHODS: Forty-four adolescents (mean age, 12 years; 23 girls, 21 boys) with suspected spondylolysis underwent lumbar MRI including BB, with ra

Spine in Athletes.

Spinal lesions in athletes can be classified into overuse and acute injuries. Overuse injuries can be found after repetitive spinal load during sport activity; acute trauma injuries of the spine are common in contact sports. We present an overview of the wide spectrum of spinal injuries related to sport activity and review their imaging findings. Concerning overuse injuries, we focus on degenerative spine disease, spondylolysis, pedicle fractures, apophyseal ring injuries, Schmorl's nodes, and Scheuermann's disease. Acute spinal injuries are relatively frequently reported in athletes and may b

Isolated Orchialgia as the Sole Manifestation of an Occult L5 Pars Defect: A Case Report.

BACKGROUND: Type 2 diabetes is a major and growing public health challenge worldwide, contributing substantially to premature mortality and disability. Increasing evidence suggests that long-term exposure to air pollution, plays an important role in the development and progression of type 2 diabetes. Countries with high levels of ambient air pollution may therefore experience a disproportionate diabetes burden attributable to environmental exposure. In Iran, where both diabetes prevalence and air pollution levels are high, national- and subnational-level evidence on the burden of type 2 diabet

Unilateral High-Riding Vertebral Artery Associates With the Progression of Atlantoaxial Joint Degeneration in Patients With Rheumatoid Arthritis.

High-riding vertebral artery (HRVA) at C2 is more prevalent in patients with rheumatoid arthritis (RA) than in the general population, but its clinical significance remains unclear. Recent evidence in non-RA individuals suggests that unilateral HRVA may contribute to atlantoaxial joint degeneration (AAJD) through asymmetric mechanical stress. This study investigated whether unilateral HRVA is similarly associated with the progression of AAJD in RA. We retrospectively analyzed 336 RA patients from the single institutional cohort who underwent CT scans, including C1-C2, between 2011 and 2024. HR

[Sports and Spondylolysis: Chances and Limits].

Spondylolysis is a unilateral or bilateral defect of the interarticular region of a vertebra, most commonly in the lumbar spine. This pathology occurs frequently in young athletes and leads to spondylolisthesis in 75% of cases. Athletically active patients generally wish to continue their sports activity. This article aims to present literature-based recommendations and our personal experience regarding sports participation in children and adolescents suffering from spondylolysis and spondylolisthesis. We conclude that sports participation is possible despite spondylolysis in individuals of gr
