Incidental durotomy is a complication of spinal surgery that carries a risk of persistent CSF leak. One major concern with persistent CSF leakage is the increased risk of infection. Fifty-six consecutive patients who underwent spine surgery complicated by incidental durotomy between 2017 and 2022. We compared 28 patients who developed a postoperative infection (IG) and 28 patients who did not (CG). The median age was 40.6 years in the IG and 57.7 years in the CG (p = 0.04). The index procedures were decompression for stenosis or discectomy for herniated disc; 18 patients had associated instrum

Dual-energy X-ray absorptiometry (DXA) is the gold standard for diagnosing osteoporosis, but degenerative spinal changes and vascular calcifications can result in artefactual overestimation of bone mineral density (BMD) (3-5). Extreme elevations in T-scores in the absence of metabolic bone disease are exceptionally uncommon. We report a 95-year-old male with multiple chronic comorbidities who presented with poor oral intake and a urinary tract infection. DXA revealed a lumbar spine T-score of + 11.0. Laboratory and imaging evaluation showed no evidence of osteopetrosis, Paget's disease, second

To identify potential red flag diagnoses in a cohort of patients seeking care for LBP, compare prevalence rates based on the setting of initial diagnosis, and identify demographic risk factors. A longitudinal cohort study using US Military Health System Data Repository data, identifying all patients seeking care for LBP in military clinics between January 2015 and December 2019. LBP cases and red-flag conditions were identified using diagnostic codes in electronic medical records. The setting of initial LBP diagnosis was also identified (immediate care: emergency/urgent care clinics, versus al

INTRODUCTION: [18F]FDG PET/CT scan is sometimes performed as part of the diagnostic evaluation of non-compressive myelopathies, but the spinal cord lesion metabolism is not systematically evaluated. We aimed to investigate the diagnostic value of spinal cord lesion metabolism on [18F]FDG PET/CT for differentiating inflammatory myelitis from neurosarcoidosis and tumoral lesions.
METHODS: We conducted a retrospective cohort study of patients presenting with spinal cord lesions who underwent [18F]FDG PET/CT. Patients were classified as primary inflammatory myelitis, neurosarcoidosis, tumoral les

Respiratory diseases, including bacterial pneumonia, viral infections and allergic asthma, are leading causes of hospitalization, yet current therapies often fall short. The lower airways are densely innervated by pain-transmitting sensory neurons (nociceptors) that arise from the nodose-jugular ganglia of the vagus nerve, with additional contributions from the spinal dorsal root ganglia. Converging evidence indicates that reciprocal neuroimmune signalling between lung-innervating sensory neurons and immune cells lies at the centre of pulmonary defence, inflammation and tissue repair. Among se

Background Vertebral osteomyelitis (VO) is an uncommon but potentially debilitating spinal infection associated with delayed diagnosis, prolonged hospitalization, and significant morbidity. General international consensus indicates that the incidence of VO is increasing, which likely reflects an aging population and an increase in use of advanced imaging. This paper aims to describe the demographic characteristics, risk factors, microbiology, and short-term outcomes associated with VO in Malta. Methods We performed a retrospective single-centre cohort study of adult patients (>18 years old) di

BACKGROUND: The incidence of the spinal infection pyogenic spondylodiscitis is rising, particularly in the aging, multimorbid segment of the population. The goal of this study was to analyze nationwide trends in the epidemiology, pathogen spectrum, and antimicrobial resistance profile of this disease in Germany over the period 2005–2023.
METHODS: All hospital discharges were analyzed retrospectively using nationwide German Diagnosis Related Groups (G-DRG) data. Hospitalizations with a primary ICD-10-GM diagnosis of spinal osteomyelitis, pyogenic intervertebral disc infection, or discitis, not

BACKGROUND: Pestivirus scrofae (previously atypical porcine pestivirus, APPV) is a member of the genus Pestivirus within the family Flaviviridae. This virus has been established as the etiological agent of congenital tremor (CT) type AII, a neurological disorder in newborn piglets characterized by generalized tremors. Despite this association, the epidemiological characteristics of the virus, including its transmission dynamics and patterns of spread between herds, remain poorly understood. It has already been confirmed that, similarly to other flaviviruses, APPV can be shed by semen. Previous

Schwannomas are benign neurogenic tumors commonly arising in the posterior mediastinum. Although typically asymptomatic, lesions with intraspinal extension may lead to neurological compromise. We report a 47-year-old male who presented with symptoms suggestive of a lower respiratory tract infection and was incidentally found to have a thoracic mass on chest radiography. Further imaging revealed a large, left paravertebral tumor with intrathoracic and intraspinal components extending through the left T7-T8 intervertebral foramina, consistent with a dumbbell schwannoma causing spinal cord compre

BACKGROUND: Granular parakeratosis (GP) is a rare benign inflammatory dermatosis that preferentially involves the axillae and other intertriginous, friction-prone skin sites and typically presents with pruritic erythema and scaling. Cases associated with benzalkonium chloride (BAC) exposure have rarely been systematically summarized, and further clinical evidence is needed. Herein, we report a case of generalized GP potentially associated with long-term exposure to a BAC-containing laundry sanitizer, with the aim of expanding the clinical spectrum of cutaneous adverse reactions associated with

Misdiagnosis is common in cases of spinal dural arteriovenous fistula (SDAVF), and immune-related myelopathies constitute one of the most frequent diagnostic pitfalls. This study aims to identify the clinical, radiological, and cerebrospinal fluid features that distinguish idiopathic transverse myelitis, seropositive neuromyelitis optica spectrum disorder (NMOSD), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD)-the immune-mediated myelopathies most often confused with SDAVF. The study included 15 patients with SDAVF, 18 with idiopathic transverse myelitis, 12 seropo

RATIONALE: Disease-modifying anti-rheumatic drugs (DMARDs) are the cornerstone of pharmacologic treatment for inflammatory arthritis (rheumatoid arthritis, psoriatic arthritis and axial spondyloarthritis). DMARDs are immunomodulatory drugs which may increase the risk of infection, including surgical site infections; thus, some surgery guidelines recommend continuing some DMARDs and withholding others prior to surgery. On the other hand, discontinuation may result in worsening of the symptoms of the underlying inflammatory arthritis. Little is known about the optimal use of DMARDs during electi

BACKGROUND: Influenza remains a major global health threat and, akin to "long COVID," has been linked to prolonged multi-organ comorbidities ("long flu"). Cardiovascular diseases (CVDs) are increasingly recognized in the pathogenesis of influenza, yet most prior work emphasizes acute or short-term outcomes and rarely contrasts multiple endpoints over extended horizons.
METHODS: This study conducted a disease-wide association study (DWAS), assembling individuals with clinically coded influenza without pneumonia (n = 5136) and population comparators (n = 314,673). Using age- and sex-adjusted Co

BACKGROUND: FIREFISH was a multicentre, open-label, two-part, phase 2 trial that assessed the safety, tolerability, pharmacokinetics, pharmacodynamics, and efficacy of risdiplam in children with type 1 spinal muscular atrophy and two SMN2 copies, aged 1-7 months at enrolment. Part 1 assessed safety and determined the dose for part 2. In part 2, treatment with risdiplam for 24 months resulted in continual improvements in motor function and achievement of developmental motor milestones. The aim of the 3-year open-label extension reported here was to assess long-term safety and efficacy of risdip

Human T-lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) is a chronic neuroinflammatory disorder often leading to demyelination of the spinal cord. Progression to HAM/TSP is closely associated with the high proviral load and the presence of virally infected CD4+ T cells that release extracellular vesicles (EVs). Exosomes, an EV subtype released by many cell types, transport proteins and nucleic acids that regulate intercellular communication and have been implicated in the progression of cancer and neuroinflammatory diseases. Herein, we have studi

Spinal echinococcosis is a rare manifestation of hydatid disease with potentially debilitating neurological consequences. We present two cases of spinal echinococcosis: one depicting residual disease postsurgical decompression and the other representing recurrence after prior spinal hydatid surgery. Case 1, an early 60s male, underwent posterior decompression and laminectomy for cauda equina compression due to multicompartment extradural cysts. Despite surgical clearance, a single residual cyst was noted on follow-up MRI and managed medically. Case 2, a mid-20s female, presented 3 years after
