Quantitative analysis of the correlation between extraforaminal structures of lumbar spondylolisthesis and L5 nerve injury.

OBJECTIVE: This study aims to analyze the relevant factors leading to extraforaminal L5 nerve root injury (efL5NRI) in patients with lumbar spondylolisthesis based on commonly used radiological data and to develop a diagnostic model. METHODS: In this study, patients with lumbar spondylolisthesis accompanied by efL5NRI (positive group) were compared to those in a control group (patients with lumbar disc herniation or central canal stenosis at the L5 level). Participants were randomly assigned to a training set and a validation set in a 7:3 ratio. Relevant risk factors were then analyzed using

Common Injuries and Their Management Among Youth to Elite Artistic Male and Female Gymnasts.

Artistic gymnastics is a complex and challenging sport. The specific demands of gymnastics place athletes at unique risk of injuries to the spine, as well as upper and lower extremities. Changes to the rules and scoring system since the 2004 Olympics have encouraged athletes to perform more challenging skills, adding to the risk of injury. Single-sport specialization occurs at a young age among competitive gymnasts, introducing an increased risk of overuse injuries. Common injuries include spondylolysis, medial-sided elbow injuries, gymnast's wrist, ligamentous injuries of the knee, and Achill

Analysis of the efficacy of Endo-TLIF and OLIF-360 assisted by optical navigation in the treatment of degenerative lumbar spondylolisthesis lesions: a retrospective cohort study.

BACKGROUND CONTEXT: Endoscopic transforaminal lumbar interbody fusion (Endo-TLIF) and oblique lumbar interbody fusion with percutaneous fixation (OLIF-360) are both minimally invasive fusion techniques for degenerative lumbar spondylolisthesis (DLS). Endo-TLIF provides direct decompression under endoscopic visualization, whereas OLIF-360 restores disc height and lumbar lordosis through indirect decompression. However, their relative advantages remain uncertain, particularly regarding perioperative outcomes and radiographic efficacy. PURPOSE: To compare the clinical and radiographic outcomes o

Understanding the Impact of L5 Spondylolysis on the Correlation between Pelvic Incidence and Sagittal Acetabular Orientation.

PURPOSE: Surgeons are both interested in the relationship between spinopelvic parameters and acetabular orientation since concomitant diseases of the spine and hip are common. Spondylolysis of L5 is common in 4% to 8% of the population. L5 spondylolysis could potentially affect acetabular orientation since it is associated with alterations in the sacral and pelvic parameters. In our study, we aimed to assess the influence of L5 spondylolysis on acetabular orientation. MATERIALS AND METHODS: In 23 patients with L5 spondylolysis and 46 control patients, we assessed the correlation between the p

Anterior lumbar interbody fusion augmented with anterior screw fixation for the treatment of L5/S1 spondylolisthesis: a case report.

BACKGROUND: Various procedures, including anterior and posterior approaches, have been shown to be effective in the treatment of lumbar spondylolisthesis. Anterior approaches, such as stand-alone anterior lumbar interbody fusion (ALIF) and ALIF augmented with posterior screw fixation (ALIF + PSF), are considered options for spondylolisthesis at the L5/S1 segment. However, stand-alone ALIF is associated with concerns of stability, and ALIF + PSF is associated with postoperative low back pain because of its impact on posterior elements. The concept of anterior screw fixation (ASF) is an innovati

[Research progress on the biomechanical effects of lumbar spondylolysis on adjacent segment intervertebral discs and treatment strategies].

This study systematically reviews the biomechanical effects of lumbar spondylolysis on adjacent intervertebral discs and surgical intervention strategies. By integrating biomechanical experiments, finite element analyses, and clinical data,we found that lumbar spondylolysis significantly increased the biomechanical burden on adjacent discs, leading to reduced stiffness, abnormal stress distribution, and expanded range of motion. Comparative analysis of surgical methods revealed that direct repair techniques effectively reduced adjacent segment stress, while fusion surgery stabilized the affected seg

Relative comparison of spinal deformity between pediatric high-impact and geriatric low-impact fractures.

BACKGROUND: Spinal stress fractures are common among young athletes due to high-intensity training and repetitive spinal loading. Elderly individuals also exhibit high rates of spinal fractures, primarily due to age-related bone degeneration. This study aimed to compare fracture severity between pediatric patients experiencing high-energy mechanical injuries and geriatric patients sustaining low-energy injuries. METHODS: This 10-year retrospective review included patients from a single center. Pediatric patients (<18 years) were included if they sustained injuries from exercise or motor vehic

Posterior lumbar interbody fusion (PLIF) combined with pars repair using a U-shaped rod technique for two-level lumbar spondylolysis: case report and technical note.

BACKGROUND: Multi-level lumber spondylolysis is a rare clinical condition. In a surgical case, a surgical strategy gets complicated and needs to be tailored to each. We report a case of L4 and L5 spondylolysis with L4-5 spondylolisthesis successfully treated using the U-shaped rod technique. CASE DESCRIPTION: The patient experienced low back and right lower leg pain. Neurological imaging revealed L4-5 spondylolytic spondylolisthesis, resulting in right foraminal stenosis and L4 root compression. L5 spondylolysis without slipping was observed. We performed L4-5 posterior interbody fusion and L

Initial follow-up results in patients undergoing uniportal full endoscopic cervical unilateral laminotomy for bilateral decompression.

OBJECTIVE: Cervical spondylosis affects 5%-20% of the population and is commonly managed with posterior decompression. However, early studies of laminectomy without fusion revealed a significant risk of postoperative kyphosis, leading to the adoption of laminectomy with posterior fixation or laminoplasty as the standard of care. The aim of this study was to assess whether posterior decompression performed via a uniportal full endoscopic approach can achieve adequate decompression of the cervical spinal cord while preserving postoperative alignment. METHODS: This single-surgeon single-institut
