The use of spinal instrumentation as an adjunct to fusion for the treatment of degenerative disorders of the lumbar spine is controversial. Instrumented lumbar fusions, in specific instances, may improve patient outcomes. For patients undergoing single level primary lumbar arthrodesis, the available data do not conclusively support the efficacy of spinal instrumentation. However, in the setting of previous failed lumbar surgery, iatrogenic or degenerative lumbar spondylolisthesis, spinal instrumentation may be useful as an adjunct to fusion. Possible advantages associated with the use of instr

We describe a rare case of iatrogenic injury to the upper abdominal aorta during spine surgery. The case is reviewed, and diagnosis as well as management of this injury is discussed. No similar previous reports of iatrogenic injury to the upper abdominal/lower thoracic aorta were identified in the literature. Reports of iatrogenic injury to the lower abdominal aorta during spinal surgery have been frequent. With more aggressive operative treatment of spinal disorders, injury to the upper abdominal/lower thoracic aorta may become more common. Timely diagnosis and treatment are essential to prev

This retrospective study analyzed the survivorship of DKS instrumentation and the clinical outcomes in 185 patients with spondylolisthesis. These patients were treated with Zielke DKS instrumentation for a mean followup period of 3.5 years. Eight (4.3%) patients had late removal of implants, 25 (14%) had rod breakage, three (1.7%) had screw breakage, and 16 (8.7%) had nut loosening. The survivor rate of DKS instrumentation was 96% within 3 months after operation, 80% at 2 years, and 61% at 5 years after surgery. One hundred sixty-three (88%) patients had solid posterolateral fusion, and 167 (9

Attainment of successful lumbar fusion in adults with spondylolisthesis has historically been unpredictable. Recent results and conclusions have been conflicting regarding the role of instrumentation in improving the fusion rate and clinical outcome in this patient population. In a retrospective multicenter clinical study, we assessed the outcome of 42 adults with spondylolisthesis who underwent posterolateral lumbar fusion by using pedicular instrumentation with AO DC plates. No attempt was made to reduce slippage. Follow-up clinical outcome was obtained from a patient questionnaire administe

Stabilization for the treatment of a pars defect frequently involves fusion with sacrifice of a motion segment. Intrasegmental stabilization has been described, however, with preservation of the motion segment by using various constructs. We describe a method of obtaining rigid fixation across a pars defect without sacrificing a motion segment.

120 consecutive patients with painful spondylolisthesis underwent combined anterior and posterior fusion. Patients were divided retrospectively into two groups. In group I, 46 patients were treated with monosegmental fusion. In group II, 74 had polysegmental fusion. The mean follow-up time was 3 (2-7) years. Anterior posterior (AP)-translation in the first disc superior to fusion was measured according to Wiltse and Winter (1983). In group I, 3 and in group II, 10 patients developed an AP-translation of more then 3 mm in the disc adjacent to the fusion. AP-translation correlated with the numbe

Between March 1988 and March 1990, 45 patients underwent a spondylodesis using transpedicular screws and plates of the "Variable Screw Placement Spinal Fixation System". The indications for operation were spondylolisthesis (13), spondylolisthesis plus discopathy at the adjacent level (4), degenerative discopathy (13), pseudarthrosis after interbody fusion (7), disc herniations (4) and disc herniations plus degenerative discopathy of the adjacent segment (4). In 1992, 43 patients were available for follow-up. The mean follow-up was 3.85 years. Side effects or complications of a more permanent c

The results of Cotrel-Dubousset instrumentation in 50 patients with idiopathic scoliosis were evaluated. The patients were followed for 24 to 108 months with a mean follow-up of 5 years. Their average age was 15 years and 3 months. The scoliotic curves were classified according to King et al. There were 4 type I, 20 type II, 10 type III, 10 type IV and 1 type V. Five curves could not be included in this classification: 2 double lumbar, 1 right lumbar and 2 left thoracic curves. Coronal plane analysis showed an average postoperative correction of 56% for thoracic curves and 57% for lumbar curve

In a retrospective study, we evaluated 180 patients treated for painful spondylolisthesis with combined anterior and posterior fusion. Group I included 90 patients treated by anterior fusion with the transperitoneal approach. Group II included 90 patients operated on with a retroperitoneal approach. Group II showed a higher incidence of pseudarthrosis L5/S1 (4%), tear of the common iliac vein (1%), postsympathectomy syndrome (4%) and reversible L4 nerve-root lesion (3%). On the basis of our findings, we recommend the transperitoneal approach for anterior interbody fusion L5 to S1 or L4 to S1.

STUDY DESIGN: Radiographic analysis of a pediatric population with spondylolisthesis was performed to examine sagittal plane pelvic rotation and degree of slip over time.
OBJECTIVES: To determine whether the degree of standing sagittal offset of L5 with respect to the acetabulum correlated with slip progression and symptoms.
SUMMARY OF BACKGROUND DATA: The natural history of isthmic spondylolisthesis remains unclear. Attempts to predict slip progression in the clinical setting, and thus the possible need for eventual surgical intervention, remain imprecise. Predicting slip progression based

Forty-two neurologically intact adults in whom non-operative treatment of grade-I or grade-II isthmic spondylolisthesis of the most caudad lumbar segment had failed were entered into a prospective study of the results of operative treatment. Twenty patients who smoked were managed with a posterolateral arthrodesis with instrumentation (transpedicular fixation), and twenty-two patients who did not smoke were managed with a posterolateral arthrodesis without instrumentation. Of the patients who were managed with instrumentation, eight were randomized to treatment with a decompressive laminectomy

PURPOSE: To study the relationship of lumbar facet joint tropism to degeneration of the cartilage and subcortical bone in the facet joints and the effect of tropism in intervertebral disk degeneration.
METHODS: The orientation of 104 cadaveric lumbar facet joints with respect to sagittal plane was measured on CT scans, and the joints were classified as having no tropism, mild tropism, or severe tropism. On MR images, the severity of cartilage degeneration and bony sclerosis was measured. The correlation between tropism and degeneration was calculated, as was the relationship among age, spinal

STUDY DESIGN: Prospective observational study.
OBJECTIVES: To examine associations between radiographic parameters (scoliosis and olisthesis) and outcomes of surgery for degenerative spinal stenosis.
SUMMARY OF BACKGROUND DATA: Preoperative degenerative scoliosis generally is thought to be associated with an unfavorable outcome of surgery for spinal stenosis. Data on the relationship between an increase in olisthesis after laminectomy for spinal stenosis and outcomes of surgery are sparse and conflicting.
METHODS: Radiographs were obtained before surgery and at least 6 months after surgery

STUDY DESIGN: A clinical and radiologic retrospective follow-up examination of patients treated surgically for severe juvenile spondylolisthesis.
OBJECTIVE: To compare two different surgical techniques in the management of severe (degree of slip > 30%) juvenile spondylolisthesis (anterior spondylodesis in situ versus combined anterior spondylodesis and posterior transpedicular instrumentation including reduction of the slipping vertebra) to determine if the advantages of the repositioning of the slipping vertebra and a decreased number of pseudarthroses because of the transpedicular instrumen
