What determines patient satisfaction after surgery for grade 2 lumbar spondylolisthesis? A Spine CORe™ analysis of QOD data.

OBJECTIVE: The objective of this study was to evaluate predictors of patient satisfaction following surgical treatment of Meyerding grade 2 lumbar spondylolisthesis. The authors hypothesized that postoperative improvements in patient-reported outcomes (PROs) would be the primary determinants of satisfaction. METHODS: Patients with grade 2 lumbar spondylolisthesis were identified from the Spine CORe™ study group of the Quality Outcomes Database, a multicenter consortium of 14 participating sites. This cohort comprises 328 patients with a 60-month follow-up rate of 81%. Demographic, clinical, a

What is the average time frame of clinically meaningful improvement in surgical decompression for cervical spondylotic myelopathy? A Spine CORe™ analysis of QOD data: 5-year follow-up.

OBJECTIVE: The objective of this study was to determine the time frame of clinical improvement in patient-reported outcomes (PROs) following surgical decompression for cervical spondylotic myelopathy (CSM). Based on previously published 12-month data from this group, the authors hypothesized that the average time to minimal clinically important difference (MCID) improvement would primarily occur by 3 months postoperatively regardless of preoperative myelopathy severity. They also hypothesized that there would be minimal additional improvement between 3 months and 5 years after surgery. METHOD

Is baseline Neck Disability Index a predictor of 5-year postoperative satisfaction in cervical spondylotic myelopathy? A Spine CORe™ analysis of QOD data.

OBJECTIVE: Patient satisfaction is an important outcome to measure quality of care. The hypothesis of this study was that more severe baseline neck disability (Neck Disability Index [NDI] scores 70-100) is associated with less frequent postoperative satisfaction in cervical spondylotic myelopathy. METHODS: This study used the 14-site Spine CORe™ study group's cervical dataset module from the Quality Outcomes Database, which included 1085 patients. Baseline demographics, clinical variables, and surgical parameters were collected. Patient-reported outcomes (PROs) collected include EQ-5D, NDI, a

What factors predict return to work after surgery for grade 1 lumbar spondylolisthesis? A 5-year Spine CORe™ analysis of QOD data.

OBJECTIVE: Unemployment following surgery incurs significant societal costs. The authors aimed to identify predictors of return to work (RTW) following surgery for patients with grade 1 lumbar spondylolisthesis. METHODS: This Spine CORe™ study is a post hoc analysis of prospectively collected data from the Quality Outcomes Database (QOD) grade 1 lumbar spondylolisthesis module. Patients were divided into 2 groups: employed preoperatively and unemployed preoperatively. Univariate and multivariate instruments were used to identify predictors of RTW/employment within 5 years postoperatively. RE

Should BMI influence anterior versus posterior approach surgery in patients with CSM? A 5-year Spine CORe™ analysis of QOD data.

OBJECTIVE: Cervical spondylotic myelopathy (CSM) is a common cause of spinal cord dysfunction worldwide and can be treated through anterior or posterior approaches. Both strategies achieve acceptable results, but the growing prevalence of obesity poses unique challenges. Data directly comparing outcomes across body mass index (BMI) strata are limited. Here, the authors examined rates of achieving minimal clinically important differences (MCIDs) in patient-reported outcomes (PROs) between anterior and posterior approaches relative to BMI. METHODS: This was a post hoc analysis of prospectively

What are minimal clinically important differences for patient-reported outcome measures after surgery for cervical spondylotic myelopathy? A 5-year Spine CORe™ analysis of QOD data.

OBJECTIVE: The minimal clinically important difference (MCID) is widely used to interpret patient-reported outcome measures (PROMs) in cervical spondylotic myelopathy (CSM). However, consensus on its definition is lacking, and its long-term consistency remains unknown. The objective of this study was to determine if MCID thresholds for PROMs at 5 years after surgery for CSM remained consistent when compared to previously established 2-year values. METHODS: The Spine CORe™ study group performed a post hoc analysis of the prospective Quality Outcomes Database. Eight established anchor- and dist

Giant cervical facet joint osteophyte causing myelopathy: illustrative case.

BACKGROUND: Cervical spondylosis frequently leads to osteophyte formation to stabilize degenerative spinal segments. While osteophytes presenting with cervical myelopathy typically arise from the vertebral body, a cervical facet joint osteophyte causing spinal cord compression has not been described. OBSERVATIONS: This report details the case of a 57-year-old right-handed male waiter with progressive myelopathy. Imaging confirmed a 2-cm calcified extradural mass at the C5 level causing significant spinal cord compression. Surgery revealed a giant osteophyte originating from the right C5-6 fac

Evaluation of a Novel Flexible Cage System for C5-C6 Fixation: A Finite Element Study Against Conventional ACDF Implants.

Cervical spondylosis is a common cause of spinal cord dysfunction, and anterior cervical discectomy and fusion (ACDF) is widely employed when conservative treatment fails. Conventional implant systems such as the cervical cage with plate (CCP) and zero-profile stand-alone cage (ZPSC) are commonly used to enhance spinal stability and promote fusion, but they are associated with complications including dysphagia and adjacent segment degeneration. To address these limitations, a novel flexible plate cage system (FPCS) has been developed to optimize biomechanical performance while minimizing surgi

Does surgical approach affect headache relief in patients undergoing cervical fusion for subaxial cervical spondylotic myelopathy? A Spine CORe™ analysis of QOD data.

OBJECTIVE: Headache is a common complaint associated with cervical spondylotic myelopathy (CSM), presenting in more than one-third of patients. Surgical treatment of CSM has been linked to improved headache symptoms. The etiology of headache associated with subaxial cervical spine disorders is not well understood, nor is the mechanism of surgery in relieving symptom intensity. The authors hypothesized that surgical treatment of CSM has a positive impact on patients with headache symptoms, and that anterior cervical discectomy and fusion (ACDF) provides simpler access to foraminal decompression
