Pneumocystis jirovecii pneumonia-Immune reconstitution inflammatory syndrome: A review of published cases.

BACKGROUND: Immune reconstitution inflammatory syndrome (IRIS) can occur in patients with HIV after commencing antiretroviral therapy. Tuberculosis-IRIS is the most common, and Pneumocystis jirovecii pneumonia (PJP)-IRIS accounts for only 2.7%-4% of IRIS cases. The prognosis and management of IRIS is well studied in other opportunistic infections but is ill defined for PJP-IRIS, and no guidelines exist. We reviewed the literature to consolidate the available data for PJP-IRIS to formulate recommendations for the diagnosis and management of this condition. METHODS: We performed a literature re

Mortality of Pneumocystis jirovecii pneumonia in intensive care units: a post-hoc analysis of an international multicenter study by ESGCIP and EFISG.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is a life-threatening disease. In the intensive care unit (ICU), PJP is most frequently observed among patients with several conditions not related to the human immunodeficiency virus (HIV) infection. METHODS: The primary objective of the present post-hoc analysis of a multicenter, multinational, retrospective study was to assess factors impacting prognosis in ICU patients with PJP through univariable and multivariable analyses. RESULTS: A total of 107 patients were included; 28 had proven PJP (26.2%), and 79 had presumptive PJP (73.8%). The

Glucocorticoid-induced Pneumocystis pneumonia mice developed immune reconstitution inflammatory syndrome after glucocorticoid withdrawal.

Mortality from Pneumocystis pneumonia (PCP) continues to increase among non-human immunodeficiency virus (HIV) patients. Previous studies have reported that a considerable proportion of patients experience deterioration of their disease when glucocorticoids or other immunosuppressants are withdrawn. However, the underlying mechanisms responsible for this phenomenon remain poorly understood. Our findings, based on a comparative analysis of immune function in glucocorticoid-induced PCP mice at the time of glucocorticoid continuation and glucocorticoid withdrawal, suggested that the damage to lun

Diagnostic value of metagenomic next-generation sequencing in detecting Pneumocystis jirovecii pneumonia in HIV-infected patients.

INTRODUCTION: Accurate diagnosis of Pneumocystis jirovecii pneumonia (PJP) in HIV patients remains challenging. This study compares metagenomic next-generation sequencing (mNGS) with PCR, GMS staining, and serum β-D-glucan (BG) assays for PJP detection and co-infection identification. METHODS: BALF samples from 34 HIV-positive PJP patients and 50 non-PJP controls were analyzed. Diagnostic performance metrics (sensitivity, specificity, NPV, AUC) and co-pathogen profiles were evaluated for mNGS versus conventional methods. RESULTS: mNGS and PCR both achieved 100% sensitivity. mNGS showed highe

Metagenomic next-generation sequencing-based diagnosis of Pneumocystis jirovecii pneumonia in patients without human immunodeficiency virus infection: A dual-center retrospective propensity matched study.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP), caused by Pneumocystis jirovecii (PJ), is an opportunistic infection prevalent in clinical settings. However, large-scale studies on the efficacy of metagenomic next-generation sequencing (mNGS)-based diagnosis of PJP in patients without human immunodeficiency virus infection (HIV) are lacking. METHODS: The study included 168 patients diagnosed with either PJP (84) or other pneumonia types (non-PJP patients; 84) who underwent mNGS-mediated bronchoalveolar lavage fluid (BALF) analysis, Gomori methenamine silver (GMS) staining and peripheral b

A Case of Pneumocystis Pneumonia Caused by Long-Term Low-Dose Steroid Therapy.

Pneumocystis pneumonia (PCP) is an opportunistic infection caused by Pneumocystis jirovecii. It typically occurs in patients with advanced HIV infection and severely reduced CD4+ T cell counts, but it is also seen in other immunocompromised hosts such as those receiving long-term corticosteroids, immunosuppressive agents, or chemotherapy. Common symptoms include progressive dyspnea, dry cough, and low-grade fever. In general, patients at risk for PCP who are receiving the equivalent of 20 mg/day or more of prednisolone for at least eight weeks are recommended to receive prophylactic trimethopr

Efficacy and safety of echinocandins combined with Trimethoprim-Sulfamethoxazole as first-line treatment for Pneumocystis pneumonia in HIV-infected and non-HIV-infected patients.

INTRODUCTION: Pneumocystis pneumonia (PCP) is a life-threatening infection in both HIV- infected and non-HIV-infected immunocompromised patients. Although Trimethoprim-Sulfamethoxazole (TMP/SMX) combined with echinocandins has been used clinically, whether this combination is superior to TMP/SMX monotherapy as first-line treatment remains inconclusive. This systematic review and meta-analysis aimed to evaluate the efficacy and safety of TMP/SMX plus echinocandins versus TMP/SMX alone in treating PCP. The protocol of this study was registered at the PROSPERO, with registration number CRD4202510

Simultaneous detection of p24 antigen and antibody outside the acute phase of HIV infection: A case report.

We report a man in his thirties living with advanced HIV presenting with Pneumocystis pneumonia, cytomegalovirus colitis and HIV retinopathy. Initial HIV screening was positive, and immunochromatographic testing demonstrated simultaneous p24 antigen and antibody bands, an exceptionally rare finding outside the acute seroconversion phase. Despite profound immunosuppression (CD4 3 cells/µL), initiation of antiretroviral therapy led to rapid disappearance of the p24 antigen, while antibody positivity persisted. This case illustrates how severe cellular immune deficiency can allow concurrent antig
