Comparing the predictions of CT-based subject-specific finite element models of human metastatic vertebrae with digital volume correlation measurements.

Several conditions can increase the incidence of vertebral fragility fractures, including metastatic bone disease. Computational tools could help clinicians estimate the risk of vertebral fracture in these patients; however, comparison with in vitro data is mandatory before using them in clinical practice. Nine spine segments were tested under compression and imaged with micro-computed tomography (µCT). The displacement field was calculated for each vertebra using a global digital volume correlation (DVC) approach. Subject-specific homogenised finite element models of each vertebra were built

Surgical factors that contribute to tibial periprosthetic fracture after cementless Oxford Unicompartmental Knee Replacement: a finite element analysis.

BACKGROUND: Tibial periprosthetic fracture (TPF) is a severe complication of cementless Oxford Unicompartmental Knee Replacement (OUKR) with patient risk factors including small tibial size and tibia vara with an overhanging medial tibial condyle. Surgical factors also influence fracture but remain poorly defined. This finite element (FE) analysis study identified surgical risk factors for TPF after OUKR and determined the optimal tibial component positioning to minimise fracture risk. METHODS: Knees in two very high-risk, small, bilateral OUKR patients who had a TPF in one knee and a good re

An investigation into the biomechanical effects of tibial vertical cutting errors on the proximal tibia after unicompartmental knee arthroplasty and the improvement of cutting planes.

OBJECTIVE: Unicompartmental knee arthroplasty (UKA) has shown significant clinical effectiveness in treating medial compartment knee degeneration, but postoperative periprosthetic fractures and persistent pain remain common and challenging complications. Tibial vertical cutting errors are considered an important factor influencing postoperative biomechanics. This study aims to investigate the biomechanical effects of tibial vertical cutting errors(referring to the deviation between the actual vertical cutting plane and the ideal vertical resection plane during UKA)on the proximal tibia after U

Acromion and scapular spine fractures in reverse total shoulder arthroplasty: an extended finite element model study.

BACKGROUND: Reverse total shoulder arthroplasty is widely used to restore shoulder function and alleviate pain. In approximately 1%-4% of patients, acromion and scapular spine fractures (ASF) occur and present a clinical challenge for orthopedic surgeons. While impact trauma, impingement, and excessive muscle forces are proposed causes, the relationship between these loading conditions and ASF types remains unclear, hindering targeted prevention strategies. The objective of this study is to identify the biomechanical relationship between loading conditions and ASF types, with the goal of infor

Early experience with percutaneous photodynamic nails for sacral metastatic disease and insufficiency fractures: a retrospective cohort analysis of functionality and pain relief.

BACKGROUND: Treatment of impending pathologic fractures and insufficiency fractures of the sacrum due to metastatic disease and radiation is challenging. The use of traditional hardware is limited by poor bone quality and presence of non-contained defects. The role of minimally invasive photodynamic nails (PDN) for treatment of these fractures remains poorly understood. METHODS: Patients with symptomatic impending pathologic fractures of the sacrum due to metastatic bone disease, multiple myeloma, or insufficiency fractures from radiation osteitis who underwent PDN stabilization were identifi

Biomechanical effects of cement neck and interspinous process device on locking lumbar interbody cementation.

BACKGROUND: Locking lumbar interbody cementation is a surgical option in patients with osteoporosis and low mobility. It can quickly stabilize the spine construct and prevent cage subsidence. However, establishing a stable bridging neck cement between the vertebrae and disc is a key procedure. METHODS: The validated lumbosacral model analyzed the stress cracking risks for five cement neck diameters under flexion, extension, bending, and twisting. The key indices included disc mobility and neck stress. The biomechanical impact of the interspinous process device was evaluated in high-stress fra

Influence of the magnitude and orientation of forces on the odontoid fracture: A finite element model analyses.

BACKGROUND: Fractures of the odontoid, prevalent among the elderly but affecting diverse demographics, pose significant risks ranging from mild discomfort to severe disability or fatality. These fractures, often stemming from trauma, are particularly frequent in the cervical spine. While commonly attributed to high-impact events like traffic accidents in adults, even low-energy incidents such as falls can precipitate these fractures in the elderly. Previous studies have explored loading conditions and treatment effects; however, a comprehensive investigation into the influence of the magnitude

Unilateral sacral fragility fractures: a comparative study of unilateral vs. bilateral minimally invasive osteosynthesis.

PURPOSE: Unilateral sacral fragility fractures are frequently managed with unilateral percutaneous iliosacral screw fixation, which may increase stress on the contralateral sacrum and lead to secondary fractures. This study compared complication rates between unilateral and bilateral screw osteosynthesis and evaluated risk factors for contralateral fracture following unilateral treatment. METHODS: This retrospective study included patients with unilateral sacral fragility fractures treated between 2018 and 2024. Patients were categorized into two groups: Group 1 (unilateral fixation) and Grou

Prediction of joint deformity after MIPPO with double reverse traction repositor based on Hounsfield units derived from CT scans.

BACKGROUND: Tibial plateau fractures (TPFs) account for approximately 1% of all fractures and notably 8% of fractures among the elderly. While both surgical and non-surgical interventions can yield promising short-term results, the long-term implications, particularly the risk of post-traumatic osteoarthritis (PTOA), necessitate further investigation. Although osteoarthritis (OA) and osteoporosis (OP) share common risk factors, including age and gender, yet the interaction between PTOA and OP remains unclear, thereby warranting in-depth exploration. METHODS: This study involved a cohort of 10

Does spinopelvic alignment affect fixation stability in pelvic ring fractures?: A finite element study.

BACKGROUND: The pelvic ring plays a pivotal role in maintaining biomechanical stability during upright posture. While lumbopelvic fixation is effective for stabilizing unstable pelvic fractures, the influence of spinopelvic alignment, particularly sacral slope on fixation mechanics and stress on adjacent joints, has not been adequately investigated. METHODS: A finite element model of the lumbar spine, pelvis, and femur was used to simulate a pelvic ring fracture. Three spinopelvic configurations were analyzed (sacral slope = 20°, 26°, 32°), each stabilized with lumbopelvic fixation, with or w

Biomechanical evaluation of percutaneous cement discoplasty combined with percutaneous vertebroplasty: a finite element analysis.

INTRODUCTION: Since the introduction of percutaneous cement discoplasty (PCD), numerous studies have confirmed its clinical efficacy in elderly patients. However, PCD is also associated with risks such as bone cement leakage and vertebral fractures. The purpose of this study was to present a biomechanical evaluation of two modified versions of PCD performed in combination with percutaneous vertebroplasty (PVP). METHODS: Data from a CT scan of a healthy male's lumbosacral region were used to establish finite element (FE) models of nonsurgical treatment, PCD, L4/5PCD + L4L5PVP (modified techniq
