Complicated Seronegative Neuromyelitis Optica Longitudinally Extensive Transverse Myelitis in Elderly: A Diagnostic Dilemma.

A 69-year-old gentleman without any previous comorbidities presented with complaints of acute-onset low-grade fever, one episode of loss of consciousness, and weakness of both legs with urinary and stool retention for 1 day. He had an episode of generalized tonic-clonic seizure 3 hours after admission. On examination, he had tachycardia and hypotension; Glasgow Coma Scale (GCS) was E3V2M4, and neurological examination was suggestive of bilateral lower limb upper motor neuron lesion in the shock stage. Blood investigation revealed severe hyponatremia, and magnetic resonance imaging (MRI) with c

Protective Effects of Memantine Against Acute Stress-Induced Fear Memory and Synaptic Dysfunction via NMDA Receptor Antagonism.

INTRODUCTION: Post-traumatic stress disorder (PTSD) is a complex mental health condition that arises following exposure to severe psychological trauma. Dysregulation of excitatory transmission, especially involving N-methyl-D-aspartate (NMDA) receptors, has been increasingly linked to its development. This study aimed to evaluate the therapeutic potential of memantine, a noncompetitive antagonist of NMDA receptors, in a rodent model of PTSD, with a focus on its effects at the cellular and molecular levels. METHODS: Adult male Sprague Dawley rats were exposed to an unavoidable foot shock parad

Traumatic posterior mediastinal hematoma with left atrial compression: A case series of a rare, life-threatening entity.

BACKGROUND: Traumatic posterior mediastinal hematoma (TPMH) is a potentially life-threatening entity that, in rare instances, can lead to obstructive shock and sudden death secondary to left atrial compression (LAC). We describe the clinical course and outcomes of seven adult patients with TPMH and LAC. A brief literature review is presented. METHODS: Seven patients (ages spanning from the 20s to 80s) with TPMH and LAC were identified at a Level 1 trauma center from 2016 to 2023. The first 2 patients were identified following standard quality improvement case reviews for unexplained death. Th

Anterior Cervical Corpectomy and Reconstruction With Fibular Autograft for Bacterial Spondylodiscitis: A Case Report and Literature Review.

Bacterial spondylodiscitis of the cervical spine is an uncommon but potentially serious condition that may require surgical intervention in cases of progressive vertebral collapse or neurological deterioration. We report a 41-year-old man who presented with intermittent fever, neck pain, and quadriparesis and was diagnosed with C5-C7 spondylodiscitis complicated by epidural and prevertebral abscess caused by oxacillin-sensitive Staphylococcus aureus. Despite two months of appropriate antibiotic therapy, follow-up imaging demonstrated progressive collapse of the C6-C7 vertebral body. The patien

Morphological Analysis of the Intervertebral Foramen and Feasibility of Transforaminal Full-Endoscopic Spine Surgery.

BACKGROUND: Transforaminal full-endoscopic spine surgery (TF-FESS) is a minimally invasive surgical procedure for lumbar spinal disease; however, it is technically demanding in patients with foraminal stenosis because of anatomical problems. This procedure has unique surgery-related complications, such as exiting nerve root injury, causing postoperative dysesthesia or muscle weakness, resulting in a poor clinical outcome. Therefore, we have to avoid this complication. The objective of this study was to analyze the morphology of the intervertebral foramen and assess the feasibility of TF-FESS.

Complications of Elective Lumbar Spine Fusion in Patients With COVID-19.

INTRODUCTION: The COVID-19 pandemic led to widespread delays in elective surgery, and prior studies have shown increased perioperative risk when surgery is performed soon after infection. While recommendations exist for delaying elective arthroplasty following COVID-19, data guiding the timing of elective spine surgery remain limited. This study evaluates complication rates in patients undergoing elective lumbar fusion within 14 days of a COVID-19 diagnosis using a large national database. METHODS: The PearlDiver Mariner Database was queried to identify all adult patients who underwent primar

Cue-Dependent Fear Learning Drives Nucleus Accumbens Spine Plasticity.

Nucleus accumbens (NAc) dopamine 2 receptor expressing medium spiny neurons (D2-MSNs) are involved in stress and aversive learning, where repeated stress increases excitatory spine density. Whether this plasticity reflects cue-specific learning or generalized stress response remains unknown. Using Pavlovian fear conditioning in Tac1-Cre/Tdtomato mice, we dissociated associative plasticity from the effects of foot shock stress. Acute fear conditioning produced distinct physiological outcomes between stress in the presence or absence of a cue. Conditioning for 7 days consolidated cue learning an

Molecular sabotage of hippocampal synaptic plasticity by amyloid-β in Alzheimer's disease: A narrative review.

This narrative review aims to synthesize and critically evaluate the complex molecular mechanisms by which amyloid-β (Aβ) accumulation disrupts hippocampal synaptic plasticity, the cellular cornerstone of learning and memory in Alzheimer's disease (AD). AD is characterized by progressive hippocampus-dependent cognitive decline, strongly linked to impaired synaptic plasticity, the cellular basis of learning and memory. This review deciphers how Aβ accumulation orchestrates synaptic sabotage in the hippocampus. We detail the core molecular machinery of hippocampal synaptic plasticity, emphasizin

Disulfidptosis: A Metabolic Cell Death Mechanism with Therapeutic Potential in Cancer.

Disulfidptosis is a newly identified form of regulated cell death (RCD) first described in 2023, representing a significant advance in understanding programmed cell death pathways. This unique cell death modality is characterized by abnormal intracellular accumulation of disulfide bonds and disruption of redox homeostasis, leading to cytoskeletal collapse without caspase activation. Disulfidptosis is primarily triggered by glucose deprivation in cells with high expression of solute carrier family 7 member 11 (SLC7A11). Under these conditions, insufficient NADPH supply prevents the effective re

Exosome-mediated cholesterol flow drives scoliosis progression via promoting the spinal cartilage-bone positive feedback.

Adolescent Idiopathic Scoliosis (AIS) is the most common form of spinal deformity among adolescents. To explore its etiology of progression and scoliosis-modifying drugs, chondrocytic senescence was confirmed in AIS facet joint cartilage by analyzing clinical specimen. Furthermore, through 4D/480 label-free proteomics analysis, we identified an exosome-mediated positive feedback loop during scoliosis progression, which driving the elevation of cholesterol flow between spinal cartilage and vertebra. To further investigate the pathological significance of the loop in vivo, high-cholesterol flow

Electroconvulsive Shock Induces Greater Plasticity of Dentate Gyrus Neurons Born in Adulthood Than Those Born in Development.

Electroconvulsive therapy (ECT) is the most effective treatment for severe depression but uptake is hindered due to effects on memory and a poor understanding of its mechanisms of action. Hippocampal plasticity is one consequence of ECT that may be related to both the therapeutic and amnestic effects. In animals, electroconvulsive shock (ECS) dramatically increases adult neurogenesis in the dentate gyrus (DG). However, little is known about how ECS impacts the morphology of adult-born neurons. Moreover, it is unknown whether ECS differentially impacts DG neurons that are born in development ve

Sacral Insufficiency Fracture Following L4-S1 Circumferential Fusion in Corticosteroid-associated Bone Fragility: A Case Report and Biomechanical Considerations.

INTRODUCTION: Sacral insufficiency fracture (SIF) is an uncommon but increasingly recognized complication following lumbosacral fusion, particularly in patients with compromised bone quality. The biomechanical vulnerability of the L5-S1 junction, combined with altered load transfer after anterior and posterior reconstruction, may predispose to early sacral failure. CASE REPORT: A 55-year-old male with lytic spondylolisthesis at L5-S1, repeated corticosteroid exposure, and neurogenic claudication underwent staged circumferential L4-S1 fusion (L5-S1 anterior lumbar interbody fusion, L4-L5 obliq

Multifocal Osteonecrosis in the Hips and Shoulders Bilaterally after Severe COVID-19 Infection Misdiagnosed as Fibromyalgia: A Case Report.

INTRODUCTION: Multifocal osteonecrosis (MFON) (avascular necrosis [AVN] in more than three joints) is rare. It occurs when blood flow to multiple osseous sites is disrupted, and it is usually associated with systemic disease, coagulopathies, or high cumulative doses of steroids. The current report describes a case of bilateral femoral and humeral head AVN misdiagnosed as fibromyalgia several years after a severe COVID-19 infection with complications of asystole and multi-organ failure. CASE REPORT: A 37-year-old woman presented with joint pain that started 2 years and 4 months after a COVID-1

Influence of cage material, plating and vertebral bone quality on subsidence and long-term outcomes following anterior cervical discectomy and fusion-a retrospective analysis.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a surgical procedure used to treat degenerative conditions of the cervical spine. Cage subsidence is a known post-operative complication, resulting in loss of indirect decompression, deformity, and radicular symptoms. The objectives of this study are to: (I) investigate associations between cage-plate constructs and postoperative subsidence after single-level ACDF; (II) determine relationships between cervical vertebral bone quality (C-VBQ) scores, subsidence, and fusion construct; and (III) identify potential risk factors that may

The role of intracorporeal cancellous bone grafting in the biological augmentation of vertebral body fractures: a literature review, historical perspective, and update on imaging and bone healing outcomes.

BACKGROUND AND OBJECTIVE: Interior vertebral body augmentation enables minimally invasive reconstruction from within by filling the void created by trabecular collapse, restoring endplate alignment, and supporting the anterior and middle columns. Although traditional intracorporeal cancellous bone grafting has been associated with unfavorable outcomes due to insufficient early stability, its use in combination with modern stabilization techniques may represent an effective strategy for biological vertebral body repair. This study aims to provide an updated review of the scientific literature o

Disc collapse aggravates vertebral recollapse following percutaneous kyphoplasty: a clinical review and biomechanical study.

BACKGROUND: Promoting uniform load distribution is an essential biomechanical function of intervertebral discs (IVDs), but disc degeneration (DD) may compromise this and lead to localized stress concentration. However, whether DD can trigger stress concentration on the cement-augmented vertebral body and result in more severe augmented vertebral recollapse has yet to be determined. The aim of this study was to investigate clarify this issue and provide theoretical guidance regarding the mechanism underlying this complication. METHODS: Augmented vertebrae height was measured immediately after

Subacute post-traumatic ascending myelopathy after cervical spinal cord injury: a rare and fatal complication.

Subacute post-traumatic ascending myelopathy (SPAM) is a rare but devastating complication of spinal cord injury (SCI). It is character-ized by progressive neurological deterioration extending several segments above the primary lesion within days to weeks after trauma. The underlying pathophysiology remains uncertain, and treatment strategies are not standardized. A 38-year-old man sustained trau-matic C6-7 spondylolisthesis with bilateral facet dislocation following a motorcycle accident. Initial magnetic resonance imaging (MRI) demonstrated cord contusion and edema extending from C5 to C7. A
