Brain mechanisms supporting movement-evoked chronic low back pain: an observational brain imaging study.

BACKGROUND: Chronic low back pain (cLBP) is the most prevalent chronic pain condition and the leading cause of disability worldwide. Acute movement-evoked pain strongly predicts cLBP exacerbation, reflecting maladaptive amplification of nociceptive input, yet the neural mechanisms supporting movement-evoked cLBP flares remain poorly characterised. METHODS: A total of 114 participants with cLBP performed the cLBP-evoking leg raise test during perfusion-based arterial spin labelling functional MRI. Leg raise test-based pain intensity and unpleasantness ratings were assessed with an 11-point vis

Previously fused, sagittally malaligned patients undergoing a pedicle subtraction osteotomy: early neurologic deficit recovery kinetics and their impact on patient-reported outcomes.

PURPOSE: To describe the incidence and recovery of motor weakness after pedicle subtraction osteotomy (PSO) for fixed sagittal imbalance and determine whether early postoperative weakness is associated with patient-reported outcomes (PROs) at latest available follow-up. METHODS: We retrospectively reviewed consecutive adults undergoing revision posterior fusion with PSO (Schwab grade 3-4) for fixed sagittal imbalance from December 2015 to June 2022. Lower-extremity strength was graded 0-5, and weakness was defined as 3/5 or less in any distribution. PROs (SRS-22r, ODI, VAS back, and leg pain)

Comparative effectiveness of slow-stroke back massage and hot stone massage for postoperative pain after orthopedic surgery: A randomized controlled trial.

OBJECTIVE: Postoperative pain represents a complex interplay of neurophysiological and psychosocial processes involving nociception, autonomic regulation, and stress responses. This study compared the effectiveness of slow-stroke back massage and hot stone massage, alongside standard care, in reducing acute postoperative pain following orthopedic surgery. METHODS: This three-arm randomized controlled trial included 99 male orthopedic surgery patients assigned to slow-stroke back massage (n = 33), hot stone massage (n = 33), or routine care (n = 33). Pain intensity was assessed using the Visua

Prevalence and associated factors of work-related musculoskeletal symptoms among sonographers in Singapore: An exploratory cross-sectional study.

INTRODUCTION: Work-related musculoskeletal disorders (WMSDs) are widely reported among sonographers and are associated with pain, reduced productivity and work disability. This exploratory study examined the prevalence of self-reported WMSDs among sonographers in Singapore and their unadjusted associations with demographic factors and ergonomic awareness. METHODS: A cross-sectional online survey was conducted with sonographers working across 24 healthcare institutions in Singapore. The survey collected demographic and work-related characteristics, ergonomic awareness items and discomfort acro

VERIDAH: solving enumeration anomaly-aware vertebra labeling across imaging sequences.

OBJECTIVES: Although the identification of enumeration anomalies has potential clinical implications for chronic back pain and operation planning, the thoracolumbar junction is poorly assessed and rarely described in clinical reports. Additionally, even though multiple deep-learning-based vertebra labeling algorithms exist, there is a lack of methods to automatically label enumeration anomalies. MATERIALS AND METHODS: We introduce "Vertebra Identification with Anomaly Handling" (VERIDAH), a novel vertebra labeling algorithm based on multiple classification heads combined with a weighted verte

Can Pillow Use Prevent Postdural Puncture Headache? A Critical Review.

Postdural puncture headache (PDPH) is a recognized complication of neuraxial anesthesia that follows unintentional dural puncture during epidural techniques or intentional dural puncture during spinal anesthesia and other neuraxial procedures. It has traditionally been managed with a period of mandatory supine bed rest and pillow restriction, a practice grounded in the historical belief that recumbency limits ongoing cerebrospinal fluid (CSF) leakage, thereby preventing headaches. This "no-pillow" instruction remains widely taught and practiced, including among clinicians, despite an absence o

Duplex ultrasound as an initial imaging clue to spontaneous isolated celiac artery dissection: A case report.

Spontaneous isolated celiac artery dissection is uncommon and is usually confirmed with computed tomography angiography (CTA). Duplex ultrasound may provide an initial clue when the lesion and its flow abnormalities are directly visualized. A 56-year-old woman with untreated hypertension presented with acute persistent epigastric pain radiating to the back. Color Doppler ultrasound at the celiac artery origin demonstrated an intimal flap separating the true and false lumens, with an adjacent hypoechoic component without detectable flow. Spectral Doppler sampling within the dissected segment sh

Self-reported musculoskeletal symptoms among airport employees across multiple occupational groups: a European cross-sectional study.

Objectives. Work-related musculoskeletal disorders (WMSDs) are a significant challenge for companies around the globe, including airport environments. Airport studies are limited and have mostly focused on specific occupational groups, particularly baggage handlers, without considering the whole airport community. The aim of this study was to investigate self-reported musculoskeletal symptoms among airport employees across multiple occupational groups. Methods. This cross-sectional study utilized an adapted version of the Nordic musculoskeletal questionnaire (NMQ) with a pain intensity scale.

Canadian evidence-based guidelines on spinal procedures for chronic pain.

INTRODUCTION: Interventional procedures are commonly used to diagnose and treat chronic pain of spinal origin but guidance on: their indications; frequency; technical conduct; requirements for image guidance; outcome measurement; and management of complications is lacking. These guidelines were developed by a Canadian group of clinicians, researchers, patient partners and representatives from provincial healthcare regulatory and funding bodies. METHODS: Reviews of published literature were undertaken for commonly performed spinal procedures for pain, and the evidence was synthesised qualitati

Infiltration between the popliteal artery and the capsule of the knee (IPACK) vs. selective tibial nerve block for analgesia after total knee arthroplasty: a triple-blinded randomised controlled trial.

INTRODUCTION: Infiltration between the popliteal artery and the capsule of the knee (IPACK) and the selective tibial nerve block provide analgesia for the posterior aspect of the knee after total knee arthroplasty. This randomised controlled triple-blinded trial tested the hypothesis that IPACK provides superior analgesia to selective tibial nerve block after total knee arthroplasty. METHODS: Patients having total knee arthroplasty under spinal anaesthesia with a femoral triangle block were allocated randomly to receive either IPACK with 19 ml ropivacaine 0.2% or a selective tibial nerve bloc

Over-the-counter topical diclofenac in musculoskeletal pain: clinical value, access and responsible use.

Topical non-steroidal anti-inflammatory drugs (NSAIDs), including diclofenac, are established options for localized musculoskeletal pain, with guidelines supporting their use in knee osteoarthritis, broader osteoarthritis management and acute non-low-back musculoskeletal injuries. For selected indications, topical diclofenac can be highly effective and is generally well tolerated when used as indicated. It provides local analgesic and anti-inflammatory activity with substantially lower systemic exposure than oral NSAIDs, which may be particularly relevant for older adults and people with gastr

Case Report: Clinical characteristics and management strategies of intraspinal tophaceous gout: a case series and literature review.

BACKGROUND: Intraspinal tophaceous gout, defined as monosodium urate crystal deposition within the effective spinal canal causing neural compression, is a rare subtype of spinal gout with no unified diagnostic standard. Despite documented gout history, it is frequently misdiagnosed preoperatively. METHODS: We retrospectively analyzed five male patients with pathologically confirmed intraspinal tophaceous gout (median age 25 years, range 21-48) admitted to two tertiary hospitals between January 2018 and May 2026, alongside a systematic literature review of strictly defined cases. RESULTS: The

Atrophic Dermatofibroma: A Rare Variant With Distinct Clinical and Histopathological Features.

Dermatofibroma (DF) is a common benign cutaneous soft-tissue tumor that typically affects females between the third and fifth decades of life. While its pathogenesis is not fully understood, DF has been associated with minor local trauma and insect bites. We present a case of atrophic DF in a 35-year-old Saudi female who noticed a solitary, hyperpigmented, depressed plaque on her back that had been present for five years. The lesion initially appeared as a small papule but gradually increased in size while remaining asymptomatic, without pain, itching, or systemic complaints. Physical examinat

Chondromyxoid Fibroma of the Rib on 99mTc-MDP SPECT/CT.

A 37-year-old woman with persistent right-sided back pain. 99mTc-MDP SPECT/CT demonstrated a solitary focus of intense radiotracer uptake corresponding to the rib lesion. The patient declined percutaneous biopsy. The rib was excised because malignancy was still a concern. Histopathologic examination confirmed chondromyxoid fibroma (CMF). CMF rarely involves the ribs but can show intense ⁹⁹ᵐTc-MDP uptake. A solitary MDP-avid bone lesion on SPECT/CT should therefore be interpreted cautiously, as uptake intensity alone cannot distinguish benign from malignant disease.

Association between body mass index and musculoskeletal disorders among agricultural workers: a systematic review.

BACKGROUND: Increasing Body Mass Index (BMI) is a global health concern associated with adverse health outcomes. Musculoskeletal disorders (MSDs) are prevalent among agricultural workers and substantially impair quality of life and work productivity. However, evidence on the association between BMI and MSDs in farming populations remains limited. This systematic review examines the relationship between BMI and MSDs among farmers to inform targeted occupational health strategies. METHODS: A systematic review was conducted across three electronic databases (PubMed, Embase, and SCOPUS) from thei

Assessment of Task-Specific Gait and Balance Deficits in Chronic Low Back Pain Using Multi-Sensor Inertial Measurement Units.

Chronic low back pain (cLBP) compromises daily function, but associated balance deficits remain difficult to quantify using traditional scales. We recruited 50 participants with CLBP (age: 65.68 ± 11.84) and 50 age-matched controls (age: 65.26 ± 10.73) wearing multiple inertial measurement units (IMUs) to evaluate kinematics: average absolute angular velocity (Jω) and linear acceleration (Jα) across static (single-leg stance), perturbed (balance board), locomotor (walking, tandem gait), and transitional (five times sit-to-stand) tasks. Group differences were assessed using multivariate analysi
