Persistent occipital headache after cervical arthrodesis is a disabling complication, often due to iatrogenic injury to the greater or lesser occipital nerves. Clinical overlap with primary headaches may delay diagnosis. High-resolution ultrasound (HRUS) has emerged as a reliable tool for evaluating superficial peripheral nerves and postoperative changes. We report a case illustrating its diagnostic value in occipital nerve injury, with a focused review of sonoanatomy. A 45-year-old woman developed refractory bilateral occipital pain consistent with occipital neuralgia after cervical arthrodes

Transoral laser microsurgery (TOLMS) is an established organ-preserving treatment for early-stage and selected intermediate-stage laryngeal squamous cell carcinoma (SCC). Initially indicated for Tis-T1 lesions, its use has progressively expanded to carefully selected T2 and T3 tumors, providing oncologic outcomes comparable to open surgery and (chemo)radiotherapy [(C)RT] while offering favorable functional results. The CO₂ (carbon dioxide) laser is the preferred device for TOLMS due to its precision and minimal thermal injury, allowing accurate tumor resection with optimal margin control. Succ

Diagnosing cervical spine instability with flexion-extension radiographs is challenging, as current guidelines are based on limited cadaver studies and do not adequately account for level, vertebral size, or patient effort. There is a need for automated cervical instability metrics anchored to normative reference data, accompanied by evidence on how often abnormal findings occur in real clinical populations and which soft-tissue injury patterns they can detect. We developed and evaluated fully automated, radiographic-based cervical intervertebral motion (IVM) metrics-adapted from prior lumbar

BACKGROUND: Temporomandibular disorders (TMD) present with a wide spectrum of symptoms, and many of these are linked to structural changes within the temporomandibular joint (TMJ). Degenerative changes of the TMJ disc, including thinning, perforation, and positional changes, are common in symptomatic patients. MRI is the gold standard for evaluating soft tissues, but the relationship between clinical findings and specific MRI-detected disc degenerations remains unclear. This study aims to review the clinical symptoms reported by patients with TMD and evaluate if these symptoms are linked to MR

BACKGROUND: Anterior disc displacement with reduction (ADDwR) most frequently occurs in temporomandibular joint disorder (TMD) and poses a major clinical challenge. Both botulinum toxin (BTX) and hypertonic dextrose (HD) injections have been reported for the treatment of ADDwR. However, the clinical confirmation of precise injection into the lateral pterygoid muscle and bilaminar zone is difficult. This study evaluated ultrasound-guided injection of BTX and HD for the treatment of ADDwR. METHODS: Thirty patients with ADDwR received real-time ultrasound-guided injection of 30 units of BTX and 0

Spontaneous resolution represents an ideal therapeutic outcome for intervertebral disc herniation, a prevalent clinical disorder. Drawing on our prior cervical spine research, we have demonstrated that herniated disc tissue is absorbable, a process we term the induced resorption of herniated nucleus pulposus (iRHNP). We hypothesized that a synergistic microenvironment-combining mechanical force withdrawal (decompression) and the biological activity of bone healing (release of bioactive factors)-artificially drives iRHNP. This study employed integrated in vitro and in vivo models, including rat

BACKGROUND CONTEXT: Preventing cervical adjacent segment disease (ASD) is a priority in spine surgery, because its onset can result in recurrent symptoms and undermine the durability of surgical intervention. However, the biomechanical effects of craniocervical junction (CCJ) fusion on the adjacent annulus and facet joint forces (FJFs) that contribute to the risk of ASD after surgery are poorly understood.
PURPOSE: To elucidate the biomechanical effects of FJFs and annulus fibrosus (AF) stresses on the subaxial cervical spine caused by fusion in the CCJ.
STUDY DESIGN/SETTING: A finite elemen

BACKGROUND: Brucellosis, historically endemic to northern China, has recently shown a southward spread into previously nonendemic regions. Although brucellar spondylitis represents one of the most disabling complications of the disease, its clinical and epidemiologic characteristics in these emerging southern regions remain poorly defined, warranting further investigation.
QUESTIONS/PURPOSES: Among patients diagnosed with brucellar spondylitis from nonendemic regions of southern China, we asked: (1) What are the predominant clinical and laboratory findings? (2) For focal lesion aspiration cul

AIMS: Wrong-level spinal surgery (WLSS) is a rare but serious event. Previous studies have been constrained by small sample sizes and voluntary survey methodologies, resulting in variable incidence rates and insufficient data. This study aimed, first, to determine accurately the incidence of WLSS in different regions of the spine; second, to identify high-risk situations and causative mechanisms; and third, to analyze event characteristics in detail, including the timing of recognition, corrective management, and patient-surgeon conflicts.
METHODS: This study was a retrospective analysis of a

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) with plate and screw fixation is a standard treatment for degenerative cervical disc disease. However, the advent of 3D-printed porous titanium cages (3D-PTCs) has prompted ongoing attempts to integrate this technology into ACDF procedures.
OBJECTIVE: To compare the 2-year clinical and radiologic outcomes of single-level ACDF using a stand-alone 3D-PTC versus conventional ACDF with allograft and anterior plating.
METHODS AND MATERIALS: In this retrospective, propensity-matched cohort study, 30 patients per group were analyzed. Clinic

Spinal arthroplasty aims to preserve or reconstruct the normal biomechanical functions of the spine with motion-preserving implants. Although fusion is a proven technique to stabilize the spine, it is nonphysiologic with known limitations such as loss of mobility and risk of adjacent segment disease. This paper focuses on the main areas of spinal arthroplasty including cervical disc replacement, lumbar disc replacement, and lumbar facet arthroplasty. We review the biomechanics, history, outcomes, and future directions for each of these over the last 30 years. Although today spinal arthroplasty

Study DesignSystematic review.ObjectiveLumbar disc replacement (LDR) is a motion-preserving treatment for degenerative disc disease. However, questions remain regarding the durability of segmental range of motion (ROM) over time and its relationship to patient-reported outcome measures (PROMs) and adjacent segment disease (ASD). The purpose of this analysis is to evaluate current clinical evidence regarding the long-term preservation of segmental ROM after LDR, identify predictors of preserved motion, and assess associations with clinical outcomes.MethodsA systematic review was conducted accor

INTRODUCTION AND IMPORTANCE: Cervical diffuse idiopathic skeletal hyperostosis (DISH) can cause stiffness, reducing quality of life as the result of functional impairment and progressive pain. The standard treatment depends on patient's problem which often involves anterior cervical discectomy and function, but this approach can lead to ASD. Cervical disc arthroplasty (CDA) using a metal-on-metal (MoM) prosthesis may offer an alternative treatment to preserve segmental motion while alleviating symptoms.
CASE PRESENTATION: A 55-year-old man presented with progressive cervical stiffness and pai
