Biomechanical evaluation of four internal fixation systems for two-segment anterior cervical corpectomy and fusion: a finite element analysis.

BACKGROUND: Anterior cervical corpectomy and fusion (ACCF) is a key surgical intervention for cervical spine pathologies, but multi-segment ACCF is associated with high risks of instability, implant failure, and adjacent segment degeneration (ASD). Conventional internal fixation system, vertebral body screw (VBS), titanium plate (TP), combine with titanium mesh cage (TMC) have limitations such as insufficient three-column fixation. Anterior transpedicular screws (ATPS) and 3D-printed artificial vertebral bodies (3D-AVB) have shown potential to improve biomechanical performance, but direct biom

Use of Anterior Plates in Anterior Cervical Discectomy and Fusion After Traumatic Fracture Dislocations Lead to Early Adjacent-Level Degeneration.

Objectives Adjacent-level disc degeneration after anterior cervical discectomy and fusion (ACDF) is established in degenerative cervical disease. Plate-to-disc distance (PDD) <5 mm is a known cause of this complication. Our aim was to assess the likelihood of disc degeneration of adjacent levels in patients with traumatic fracture dislocation within two years of surgery. Methods We retrospectively reviewed scans of patients who underwent ACDF surgery using an anterior plate for fracture dislocation of the cervical spine in the Royal London Hospital, London, UK. ACDF for indications other than

Computed Tomographic Evaluation of the Morphometry of the Intervertebral Foramina and Intervertebral Discs of the Cervical Spine.

BACKGROUND: The cervical spine comprises vertebrae and intervertebral discs that facilitate neck mobility and protect neurovascular structures. Intervertebral foramina serve as conduits for spinal nerves and vessels and may undergo morphological changes with aging and degeneration, potentially leading to neurological symptoms. The objectives of this study included performing a CT analysis of the morphometry of intervertebral foramina and the height of the intervertebral disc across different levels of the cervical spine, with comparisons based on age, vertebral level, gender, and laterality.

A Mendelian randomization study to reveal gut-disc axis: causal associations between gut microbiota with intervertebral disc diseases.

PURPOSE: Emerging evidence suggests a link between gut microbiota and intervertebral disc diseases (IDDs); however, the causal relationships remain unclear. This study aimed to evaluate the causal effects of gut microbiota on the risk of cervical disc disorders (CDD), other intervertebral disc disorders (OIDD), pyogenic intervertebral disc infections, and discitis, shedding light on the potential "gut-disc axis". METHODS: Genetic variation data for 202 gut microbiota taxa were obtained from the Dutch Microbiome Project, and disease outcome data were sourced from the FinnGen consortium. A Mend

Musculoskeletal Manifestations of Alkaptonuria: A Case-Based Review of Literature.

Alkaptonuria is a rare metabolic disorder caused by a mutation in the homogentisate 1,2 dioxygenase (HGD) gene, which encodes the HGD enzyme. It is inherited in an autosomal recessive pattern. This leads to the accumulation of homogentisic acid (HGA) and benzoquinone acetic acid (BQA), which is an oxidised product of HGA. Both these compounds form polymerised deposits, which cause bluish black discolouration of the cartilage. They also cause inflammation, degeneration and calcification of ligaments, large joints, tendons and intervertebral discs. A fragmented cartilage forms, which attaches to

[Cervical spondylosis: innovative understanding from traditional Chinese medicine and treatment by classic formulas].

As one of the chronic diseases with high incidence in contemporary society, cervical spondylosis has increasing patient groups who gradually present a low age, and it seriously affects social and public health. Although modern medicine has made great progress in the pathological research and clinical treatment of cervical spondylosis, patients still face gastrointestinal side effects of nonsteroidal anti-inflammatory drugs(NSAIDs), neck pain, limited mobility, upper limb numbness, and other symptoms after conservative or surgical treatment. In the theory of traditional Chinese medicine(TCM), c

[Efficacy of the far lateral Key-hole technique in the treatment of central cervical disc herniation].

OBJECTIVES: In the early stage of central cervical disc herniation, clinical symptoms may be mild. However, as the spinal cord becomes compressed by herniated nucleus pulposus tissue, progressive edema and degeneration may occur, resulting in more severe clinical manifestations, including limb weakness, bladder and bowel dysfunction, spastic paraplegia of the lower extremities, and even respiratory difficulty. The spinal endoscopic Key-hole technique is widely applied in treating radiculopathic cervical spondylosis and has demonstrated good clinical outcomes. This study aims to analyze and sum

Adoptive NK cell transfer confers neuroprotection by attenuating neuroinflammation and alpha-synuclein pathology in a mouse model of synucleinopathy.

UNLABELLED: Clevidipine (Cleviprex), a late-generation dihydropyridine calcium channel antagonist available as a lipid emulsion for intravenous infusion, is approved in the US for the reduction of blood pressure (BP) when oral therapy is not feasible or desirable. Intravenous clevidipine is effective in the treatment of both acute preoperative and postoperative hypertension in adult cardiac surgery patients, and with a rapid onset and short duration of action the drug can be easily titrated for predictable BP control. Moreover, in terms of controlling acutely elevated BP in this patient popula

Magnetic resonance imaging-based classification of the myodural bridge complex and its influencing factors.

Cerebrospinal fluid (CSF) circulation is considered the third circulation of the human body. Recently, some scholars have proposed the myodural bridge (MDB) as a novel power source for CSF flow. Moreover, the suboccipital muscles can exert a driving force on the CSF via the MDB. This hypothesis is directly supported by head rotation and nodding movements, which can affect CSF circulation. The MDB has been validated as a normal structure in humans and mammals. In addition, the fusion of MDB fibers of different origins that act in concert with each other forms the MDB complex (MDBC). The MDBC ma

Influence of cervical total disc replacement on motion in the target and adjacent segments.

BACKGROUND CONTEXT: The motion limitation after cervical discectomy and fusion alters the spine´s kinematics. Unphysiological strains may be the result and possible explanation for adjacent segment degeneration. Alterations to cervical kinematics due to cervical total disc replacement (TDR), especially two-level, are still under investigated. PURPOSE: To investigate cervical motion including coupled motions after one-level and two-level TDR in the treated and also the adjacent segments. STUDY DESIGN: An in-vitro study using pure moment loading of human donor spines. METHODS: Seven fresh fro

Practice variation in surgical treatment for lumbar degenerative disc disease: exploring regional and hospital factors influencing surgical rates.

The presence of significant, unwarranted variation in treatment suggests that clinical decision making also depends on where patients live instead of what they need and prefer. Historically, high practice variation in surgical treatment for lumbar degenerative disc disease (LDDD) has been documented. This study aimed to investigate current regional variation in surgical treatment for sciatica resulting from LDDD. We conducted a retrospective, cross-sectional analysis of all Dutch adults (>18 years) between 2016 and 2019. Demographic data from Statistics Netherlands were merged with a nationwid

A comprehensive review of cell transplantation and platelet-rich plasma therapy for the treatment of disc degeneration-related back and neck pain: A systematic evidence-based analysis.

Low back pain (LBP) and neck pain predominate as the primary causes of disability. Cell- and platelet-rich plasma (PRP) products are potential therapies with clinical trials and reviews promoting their efficacy. Nonetheless, they frequently disregard the clinical significance of reported improvements. In this systematic review, the effectuated improvements in pain, disability, quality of life (QoL), and radiographic images are comprehensively described and scored on their clinical significance. An electronic database literature search was conducted on July 2023 for in-human assessment of cell

Anterior cervical hybrid constructs reduce superior adjacent segment burden compared to multilevel anterior cervical discectomy and fusion.

BACKGROUND: Traditional surgical treatment for symptomatic cervical degenerative disc disease is anterior cervical discectomy and fusion (ACDF), yet the increased risk of adjacent segment degeneration (ASD) requiring additional surgery exists and may result in limiting long-term surgical success when it occurs. Disc arthroplasty can preserve or restore physiologic range of motion (ROM), decreasing adjacent level stress and subsequent surgery. For patients with multilevel pathology requiring at least a 1-level fusion, interest is growing in anterior cervical hybrid (ACH) surgery as a partial mo
