Endoscopic treatment of sacrococcygeal pilonidal sinus (EPSIT): results in 104 patients.

INTRODUCTION: Sacrococcygeal pilonidal sinus (PS) is a common condition in young adults, characterised by an inflammatory response due to hair impaction in the gluteal cleft. Optimal treatment remains controversial because of the morbidity and recurrence associated with conventional techniques. The endoscopic EPSIT technique has emerged as a minimally invasive alternative. OBJECTIVE: To evaluate the recurrence rate of EPSIT and, secondarily, postoperative complications, healing time, and return to daily activities. MATERIALS AND METHODS: Observational study including 104 patients with primar

Clinical outcomes associated with abnormal middle cerebral artery Doppler velocities in fetal sacrococcygeal teratoma.

BACKGROUND: Sacrococcygeal teratoma is a rare fetal tumor that may cause progressive hemodynamic compromise, fetal anemia, hydrops, and perinatal death. Although middle cerebral artery peak systolic velocity is widely used to assess fetal anemia, its prognostic value in pregnancies complicated by sacrococcygeal teratoma is not well established. OBJECTIVE: To evaluate the association between abnormal fetal middle cerebral artery (MCA) Doppler velocities and neonatal outcomes in pregnancies complicated by fetal sacrococcygeal teratoma (SCT). STUDY DESIGN: We conducted a retrospective cohort st

The vitamin D receptor story: discovery, control, and genomic reach.

OBJECTIVE: A subset of fetuses with sacrococcygeal teratomas (SCT) develops hydrops caused by high-output heart failure. Identification of fetuses at risk for hydrops is important because surgical intervention may reverse the pathophysiology of the disease. The aim of this study was to evaluate sonographic prognostic factors regarding tumor morphology and vascularity associated with the development of hydrops in utero. METHODS: Over a 10-year period, we identified 7 fetuses with SCT diagnosed antenatally and managed at the University of Mainz. We retrospectively reviewed the charts of mothers

Undiagnosed Sacrococcygeal Fistulas: A Case Report.

The clinical presentation of fistulas can include persistent drainage, pain or swelling. In the perianal, gluteal and sacrococcygeal areas, differential diagnosis may include perianal fistula, malignancy, inflammatory bowel disease or pilonidal disease. Lack of treatment can lead to disease progression. Patient appraisal of symptoms plays an important role in clinical outcomes, a role possibly shared with the shame of exposing symptoms. A behavior of avoidance and withholding of information can directly impact clinical outcomes by not seeking medical help and delays in treatment. This case des

Long-Term Outcomes of Sinus Laser-Assisted Closure in Primary Versus Recurrent Sacrococcygeal Pilonidal Sinus Disease: A Retrospective Cohort Study.

BACKGROUND: Sinus laser-assisted closure (SiLaC) demonstrates favorable outcomes for primary sacrococcygeal pilonidal sinus disease (SPSD), yet its efficacy in recurrent disease remains undefined. This study compared long-term outcomes following SiLaC in primary versus recurrent SPSD, hypothesizing that recurrent disease would predict inferior results. METHODS: This retrospective cohort study analyzed 267 consecutive patients undergoing SiLaC at a single center (July 2019-August 2022), stratified by disease status: primary (n = 214) versus recurrent (n = 53). The primary outcome, disease recu

Negative pressure wound therapy in primary closure surgeries for pilonidal disease: a meta-analysis.

Primary closure with negative pressure wound therapy (NPWT) has been investigated in patients with sacrococcygeal pilonidal sinus to enhance healing and reduce recurrence. To address existing controversies, we conducted a systematic review and meta-analysis comparing primary closure with NPWT versus non-NPWT (control) for treatment of pilonidal disease. PubMed, Embase, and Cochrane Library databases were searched from inception to December 2024 to identify studies comparing NPWT versus control in primary closure surgeries in pilonidal sinus. Statistical analyses were performed using R Software

Pilonidal Sinus: Open Surgery or PEPSiT? Our Preliminary Experience in Adolescents.

BACKGROUND: Pilonidal sinus disease (PSD) is a chronic inflammatory condition commonly affecting the sacrococcygeal region, particularly in adolescents and young adults. Traditional open surgical approaches are associated with prolonged recovery, high complication rates, and recurrence. The advent of endoscopic techniques, such as Pediatric Endoscopic Pilonidal Sinus Treatment (PEPSiT), offers a promising alternative, reducing discomfort and potentially improving outcomes. The aims of the study were to compare the effectiveness and safety of PEPSiT versus traditional open excision in the treat

Suspected organophosphate delayed neurotoxicity in sheep associated with widespread lobed needlegrass (Nassella charruana)-induced skin perforations.

Organophosphate-induced delayed neuropathy (OPIDN) is a rare, chronic manifestation of organophosphate (OP) poisoning that typically emerges days after exposure and affects humans and other animals. Here, we describe the epidemiologic, clinical, and pathologic features of 2 suspected outbreaks of OPIDN in Australian Merino sheep in Uruguay following cutaneous exposure to OP. The outbreaks occurred in yearling sheep at 21 and 44 d after a single exposure to pirimiphos-methyl and diazinon, respectively, involving several hundred animals in each case. Affected animals had massive skin perforation

Segmental Infantile Hemangiomas That Involve the Midline Define Risk for LUMBAR Syndrome.

A systematic review of clinical photographs from 91 patients with LUMBAR syndrome demonstrated that infantile hemangiomas in affected individuals were consistently segmental in morphology and involved the anatomic midline of the lumbosacral, sacrococcygeal, or pelvic regions. No cases were "partial segmental" or spared the midline. The hemangiomas were transmedian in all patients except one, in whom the hemangioma was unilateral and abutted the midline, and thus paramedian. These findings support the conclusion that segmental infantile hemangiomas involving the anatomic midline confer risk for

Open Pilonidal Excision as a Translational Human Model for Wound Healing and Skin Regeneration Research.

Background/Objectives: Wound healing is a complex biological process involving coordinated interactions among inflammatory cells, growth factors, extracellular matrix components, and resident tissue cells. Despite significant advances in experimental research, translation of these findings into clinical practice remains limited, partly due to the lack of reproducible and ethically accessible human wound models. Pilonidal disease, a chronic inflammatory condition of the sacrococcygeal region, is frequently treated by surgical excision with healing by secondary intention. The resulting open woun

Exploratory pilot study of minimally invasive therapy: laser ablation combined with acellular dermal matrix implantation for recurrent sacrococcygeal pilonidal disease.

PURPOSE: Recurrent sacrococcygeal pilonidal disease (SPD) poses significant therapeutic challenges. Recently, minimally invasive techniques, notably laser ablation, have emerged as promising alternatives to conventional surgery. Concurrently, the application of acellular dermal matrix (ADM) has shown potential in facilitating wound healing and tissue regeneration. This small retrospective exploratory pilot study aims to evaluate the efficacy and safety of a combined therapeutic approach utilizing laser ablation and ADM implantation for the management of recurrent SPD. METHODS: We retrospectiv

[Research on deep learning-based pathological image classification of pilonidal sinus].

Pilonidal sinus disease (PSD) is a suppurative inflammatory condition characterized by a sinus tract in the sacrococcygeal gluteal cleft and surrounding skin. Surgical resection is an important treatment for PSD; however, postoperative recurrence occurs in some patients, severely affecting their quality of life. Due to the lack of objective clinical indicators for predicting recurrence, developing an artificial intelligence-based prediction model is an effective approach. In this study, postoperative hematoxylin and eosin (H&E) stained pathological sections from PSD patients were used. The hor
