A mixed effect similarity matrix regression model (SMRmix) for integrating multiple microbiome datasets at the community level.

Recent studies have highlighted the importance of the human microbiota in health and disease. However, in many areas of research, individual microbiome studies often provide inconsistent results due to limited sample sizes and the heterogeneity in study populations and experimental procedures. This inconsistency underscores the need for integrative analysis of multiple microbiome datasets. Despite the critical need, statistical methods that incorporate multiple microbiome datasets and account for study heterogeneity are not available in the literature. To address this, we propose a mixed effec

Baseline clinical and laboratory profiles at breast cancer diagnosis in Tanzania: differences by HIV status.

BACKGROUND: Women living with HIV (WLHIV) may present with distinct clinical and laboratory characteristics at the time of breast cancer diagnosis. While previous studies, including multi-country cohorts such as the ABC-DO study, have described baseline patient and tumor features, data on pre-treatment laboratory parameters in routine clinical settings remain limited. This study aimed to characterize baseline clinical and laboratory factors at diagnosis and assess differences by HIV status. METHODS: We analyzed data from women newly diagnosed with breast cancer at three tertiary hospitals in

Incidence of frailty-related fracture among Medicaid beneficiaries living with HIV and cancer: A cohort study.

BACKGROUND: People living with HIV (PLWH) are at increased risk for frailty-related fracture. Limited evidence suggests recent diagnosis of non-AIDS defining cancer (NADC) is a risk factor for fracture among PLWH. We evaluated frailty-related fracture by HIV and cancer status to inform the burden of fracture among PLWH. METHODS: We included 14,554,711 beneficiaries without HIV and 159,188 beneficiaries with HIV who were 30-64 years old enrolled in Medicaid between 2001-2015 in 14 states. HIV, NADC, and fracture diagnoses were identified from inpatient and other non-prescription claims. We cal

Extended Genotyping as Triage of HPV Positive Screened Women in Low-and-Middle Income Countries.

Cervical cancer screening coverage in low- and middle-income countries is low, and many screened women are lost to follow-up. Point-of-care testing that allows same-day treatment could reduce the incidence of this cancer, but same-day triage testing is required to prevent overtreatment and minimise loss-to-follow-up before treatment. Built-in extended genotyping with the Xpert® human papilloma virus (HPV) assay offers point-of-care triage, but treatment thresholds differ. We calculated test performance of the Xpert® HPV assay for HIV-positive and HIV-negative women. Extended HPV-genotyping was

Geospatial Access to CAR-T Clinical Trials for Non-Hodgkin Lymphoma for Persons With HIV.

IMPORTANCE: Chimeric antigen receptor T-cell (CAR-T) therapy is a paradigm-changing therapy in treating non-Hodgkin lymphoma (NHL). Although NHL is a leading cause of cancer-attributable deaths for persons living with HIV (PWH) in high-income countries, PWH are frequently excluded from CAR-T clinical trials. OBJECTIVE: To explore access to CAR-T trials for the general population and PWH. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study queried ClinicalTrials.gov for all interventional CAR-T clinical trials for the treatment of NHL. Zip codes for each trial site were extracted, a

Primary bile duct CD20-negative B cell lymphoma presenting as cholestatic jaundice in a patient with HIV.

Primary bile duct lymphoma is an exceptionally rare cause of biliary obstruction and may mimic cholangiocarcinoma or AIDS cholangiopathy on imaging particularly in immunocompromised patients. We report a man in his 30s with a newly diagnosed HIV infection (CD4 count 153 cells/µL) who presented with progressive jaundice, weight loss, fever and abdominal pain. Initial laboratory investigations revealed a cholestatic pattern of liver injury. Imaging demonstrated diffuse biliary strictures, infiltrative lesions and a hypovascular mass, initially suggestive of cholangiocarcinoma or sclerosing chola

Cervical cancer screening among HIV-positive women in conflict-affected and resource limited settings: the case of Tigray region, Ethiopia.

BACKGROUND: Cervical cancer remains a significant public health concern, particularly among women living with HIV/AIDS, who are at increased risk due to compromised immunity. Despite the high burden of cervical cancer in sub-Saharan Africa, including Tigray, Ethiopia, the magnitude of cervical lesions within screening programs and its associated determinants remain inadequately addressed. This study assesses the magnitude and risk factors of precancerous cervical lesion among HIV-infected women in conflict-prone and resource- limited settings of Tigray, Ethiopia. METHODS: We conducted a facil

Early antiretroviral therapy and long-term cancer risk in HIV: 9-year outcomes from the START randomized trial.

BACKGROUND: Antiretroviral therapy (ART) reduces cancer risk in people with HIV (PWH) but the long-term impact of immediate ART initiation on infection-related and infection-unrelated cancers remains unclear. METHODS: In the Strategic Timing of Antiretroviral Treatment (START) trial, 4684 ART-naïve adult PWH with CD4 + T-cell count above 500 cells/mm3 were randomized to immediate or deferred ART initiation arms (i.e., until CD4 + T-cells below 350 cells/mm3). In May 2015, unblinding revealed a 74% reduction in infection-related cancer risk in the immediate arm. Post-2016, participants in the

Diagnosis of Kaposi's sarcoma via gastric biopsy leading to the detection of an AIDS case in a report.

Kaposi's sarcoma is a vascular low-grade malignant tumor that is frequently observed in immunocompromised patients, particularly those with AIDS, and is caused by Human Herpesvirus 8. This case report presents a case of gastric Kaposi's sarcoma identified through gastric mucosa biopsy, which eventually resulted in an AIDS diagnosis. The Digestive Endoscopy Center at Jilin received a male patient in his mid-30s in 2024 with a chief complaint of "abdominal and periumbilical pain for two weeks." Esophagogastroduodenoscopy demonstrated dispersed white patches throughout the esophagus mucosa, along

Epidemiology of pre-cancerous cervical lesions among women in Adama, Ethiopia.

BACKGROUND: Cervical pre-cancer is a distinct change in the epithelial cells of the transformation zone of the cervix; the cells begin developing in an abnormal fashion in the presence of persistent or long-term high-risk oncogenic human papillomavirus infection. It is estimated that out of women infected with human papillomavirus, 10% will develop pre-cancerous changes in their cervical tissue. About 8% of the women who develop these changes will develop pre-cancer limited to the outer layers of the cervical cells, and about 1.6% will develop invasive cancer. The main purpose of the study was

Primary nasopharyngeal Kaposi's sarcoma without cutaneous manifestations: a case report.

BACKGROUND: Kaposi's sarcoma (KS) is a neoplastic lymphovascular disorder. It typically appears on the skin of the upper and lower extremities, but can also occur on the mucosal surfaces of the head and neck. The most common site reported for head and neck injuries is within the oral cavity, particularly on the palate, and other mucosal sites, such as the nasopharyngeal area. Moreover, involvement of the nasopharynx without skin involvement is even rarer. CASE PRESENTATION: Here, we present a rare case of HIV associated/epidemic KS in a 30-year-old man of African origin, where the primary man

False-positive HIV-1 RNA detection after CAR-T cell therapy in a multiple myeloma patient: multi-assay comparison confirms dual-target cross-reactivity.

Chimeric antigen receptor T-cell (CAR-T) therapies, employing lentiviral vectors, may cause false-positive HIV-1 RNA results due to amplification of vector-derived sequences. We describe a case of a patient with multiple myeloma undergoing CAR-T therapy tested with two dual-target assays testing positive at low copies for HIV-1 RNA. A single-target assay consistently tested negative. HIV serological tests remained non-reactive. This case highlights the importance of assay target design in interpreting HIV RNA results after CAR-T therapy and supports the importance of serological tests and alte

From Awareness to Action Study: Improving Human Papillomavirus Knowledge, Screening and Vaccine Uptake Among Mother-Adolescent Pairs in the HOMINY Study in Nigeria: A Longitudinal Study.

INTRODUCTION: Persistent cervical infection with high-risk human papillomavirus (HPV) in women is a leading cause of cervical cancer, and its co-infection among people living with HIV increases the risk of other HPV-associated cancers, including oropharyngeal and anogenital cancers. In sub-Saharan Africa, awareness of HPV is limited, screening and vaccine adoption are critically low, undermining efforts towards sexually transmitted infection (STI) elimination. METHODS: From Awareness to ACTion (FACT) study assessed HPV knowledge, screening and vaccine uptake among adolescent-mother pairs part
