Defining and interpreting the clinical relevance of intervention effects for patient-reported outcomes: the smallest worthwhile effect construct.

In clinical research, statistically significant effects do not necessarily indicate that an intervention provides benefits that are meaningful to patients. This is particularly important for patient-reported outcomes, where thresholds used to interpret clinical relevance are often derived from within-person changes, such as the minimal clinically important difference, and then inappropriately applied to between-group effects in randomised trials and meta-analyses. This article clarifies the conceptual distinction between within-group change and between-group effects, and argues that the latter

Successful Treatment of Hamstring Tendinopathy in a Nonathlete with Ultrasound-Guided Injection to the Ischial Tuberosity: A Case Report.

Background and Clinical Significance: Proximal hamstring tendinopathy can occur not only in athletes but also in nonathletes when daily activities impose repetitive tensile or compressive loading at the ischial tuberosity. Because symptoms often resemble piriformis syndrome or lumbar pathology, diagnosis may be delayed; Case Presentation: A woman in her twenties developed buttock pain during desk work. Lumbar MRI was normal, and piriformis blocks provided only temporary relief. Localized tenderness at the ischial tuberosity, pain provocation during sitting, and positive provocation tests sugge

Brain imaging-wide exploration of site-specific pain: genetic correlation and Mendelian randomization study.

BACKGROUND: Observational studies have suggested that brain imaging-derived phenotypes (IDPs) may serve as specific markers of pain-related phenotypes and severity. However, the shared genetic architecture between pain and brain IDPs and their potential causal relationships remains unclear. METHODS: We applied linkage disequilibrium score regression and Mendelian randomization (MR) analyses to uncover genetic correlations and potential causal links of brain structural (33,224 UK Biobank participants) and functional (47,276 UK Biobank participants) changes with site-specific pain phenotypes (a

A genomic structural equation modelling analysis of the shared genetic architecture of the aging spine.

BACKGROUND: Clinical manifestations of aging spine, such as lumbar spinal stenosis, intervertebral disc degeneration, osteoporosis and sciatica, frequently co-occur, yet their shared genetic basis remains unclear. METHODS: We assembled large-scale GWAS summary statistics for telomere length, osteoporosis, intervertebral disc degeneration, lumbar spinal stenosis and sciatica, and applied Genomic structural equation modelling to model their SNP-based heritability and genetic covariance structure. A latent aging spine factor was fitted to capture common genetic liability, followed by mvGWAS of t

Identifying the presence of disc herniations in lumbar spine MRI using Gemini 3.1 Pro.

PURPOSE: Lumbar disc herniation is associated with substantial morbidity, including low back pain, radicular leg pain (sciatica), sensory disturbance, and motor deficit. Magnetic resonance imaging (MRI) is central to confirming the diagnosis in symptomatic patients and to planning surgical or interventional management. Recent advances in artificial intelligence (AI) raise the possibility of automating aspects of image interpretation to improve consistency and reduce radiologist workload. This study evaluates a general-purpose large multimodal model, Gemini 3.1 Pro, for identifying disc herniat

Revision Lumbar Spine Surgery for Failed Back Surgery Syndrome: A Retrospective Study of Causes, Complications, and Outcomes.

BACKGROUND: Failed back surgery syndrome (FBSS) describes persistent, recurrent, or new-onset low back pain and/or radicular symptoms following lumbar spine surgery. Revision lumbar surgery remains challenging because of altered anatomy, epidural scarring, unclear tissue planes, and a higher risk of complications. Careful identification of a surgically correctable pathology is essential to improve outcomes. AIM: The aim of this study was to evaluate the causes of failed previous lumbar surgery, indications for revision surgery, operative procedures performed, postoperative complications, and

Breast Cancer Metastases Presenting as Musculoskeletal Complaints.

Breast cancer often metastasizes to the bone, leading to musculoskeletal symptoms that may mimic benign conditions. Patients with undiagnosed or recurrent malignancies may initially present to first-contact providers, including chiropractors, with non-specific spinal or musculoskeletal pain. Here, we extend the series with two additional cases, highlighting risks of delayed diagnosis, the role of imaging, and chiropractic care. In Case 1, a 62-year-old woman with a history of breast carcinoma presented with progressive left-sided sciatica-like symptoms (pain radiating from the sacroiliac regio

Differentiating Sciatica from Hip Osteoarthritis: Diagnostic Challenges and the Role of the Athena Sign.

Background/Objectives: Sciatica and hip osteoarthritis (OA) are common causes of lower extremity pain and functional impairment. Because both conditions may produce overlapping symptoms, distinguishing them can be challenging. Misdiagnosis may lead to inappropriate treatment, delayed intervention and unnecessary procedures. In addition, the frequent coexistence of hip and lumbar degenerative pathology, commonly referred to as hip-spine syndrome, further complicates diagnostic evaluation. This review aims to examine current diagnostic strategies used to differentiate lumbar radiculopathy from h

Immune Cell-Specific Regulatory Gene Prioritization in Low Back Pain Using Single-Cell eQTL-Informed Mendelian Randomization and Colocalization.

OBJECTIVE: Low back pain (LBP) is a heterogeneous clinical pain condition, but the contribution of inherited immune cell-specific regulatory variation to LBP susceptibility remains poorly understood. We aimed to prioritize peripheral immune cell-specific regulatory genes associated with LBP by integrating single-cell eQTL-informed Mendelian randomization (MR), Bayesian colocalization, consistency analyses across related spinal phenotypes, and transcript-level inflammatory-response assessment. METHODS: Single-cell eQTL data from the OneK1K resource across 14 peripheral immune cell types were i

Perioperative Hydrogen Inhalation and Postoperative Pain After Endoscopic Discectomy: A Pilot Non-Randomized Clinical Study with Exploratory Murine Data.

Background and Objectives: Postoperative pain after endoscopic discectomy may reflect acute surgical nociception superimposed on pre-existing radicular symptoms. Molecular hydrogen (H2) has antioxidant and anti-inflammatory properties, but clinical evidence for perioperative analgesia is limited. We evaluated the association between perioperative H2 inhalation and postoperative pain and analgesic requirements. Materials and Methods: This open-label, non-randomized pilot study enrolled 37 patients in two consecutive periods (H2, n = 25; control, n = 12). Because original outcome records for 12
