Confined to Kambin's prism: MRI-based segmentation and prediction of the need for superior articular process removal in transforaminal endoscopic decompression.

OBJECTIVE: The endoscopic transforaminal lumbar approach offers reduced tissue disruption and faster recovery compared with other open and minimally invasive approaches but is limited by the narrow and variable anatomy of the transforaminal corridor. Degenerative changes such as superior articular process (SAP) hypertrophy can block safe access, forcing the surgeon to remove part of the SAP (foraminoplasty) either by reaming or drilling. This decision is often made subjectively or discovered intraoperatively. Recent advances in MRI-based nerve segmentation may provide an objective method to pr

Lower HLA-B27 positivity and heterogeneous late-onset case proportions across axial spondyloarthritis cohorts: a systematic review and meta-analysis.

To estimate the proportion of late-onset ankylosing spondylitis (AS)/axial spondyloarthritis (axSpA) cases defined by study-reported symptom, axial-symptom, or back-pain onset using > 45- or ≥ 45-year thresholds in clinical cohorts or registries, and to compare later- versus earlier-onset phenotypes. The nine original databases were searched through 20 August 2026, and DOAJ was added as a supplementary source. The prespecified primary study-family set was synthesised using a logistic-normal mixed model. Compatible phenotypes were pooled using REML random-effects models with Hartung-Knapp inter

Diffuse Bilateral Multifocal Bone Stress Injuries in a 19-Year-Old Male Military Trainee: A Case Report.

Bone stress injuries (BSIs) are common among high-performance athletes and military trainees. BSIs typically present with point tenderness, loading pain, and nighttime discomfort, and most involve only one or two bones, with clinical symptoms roughly proportional to the degree of osseous injury. We present the case of a 19-year-old male engaged in high-intensity Air Force Special Warfare training who initially presented with vague low back, thigh, and foot pain and was ultimately diagnosed with diffuse multifocal grade 3 and 4a BSIs involving numerous bones of the bilateral lower extremities.

Platelet-Rich Plasma Injections for Refractory Sacroiliac Joint Pain: A Two-Patient Case Series.

BACKGROUND: Sacroiliac joint (SIJ) pain is an important source of chronic axial low back and buttock pain. Initial treatment commonly includes activity modification, physical therapy, nonsteroidal anti-inflammatory drugs (NSAIDs), and other nonopioid medications. Interventional treatment options may include corticosteroid injections, radiofrequency ablation, and, in selected refractory cases, SIJ fusion. Platelet-rich plasma (PRP) is an autologous biologic injectate that has been investigated as an alternative treatment for low back pain disorders, although the available literature remains lim

Skeletal muscle-specific markers for paraspinal injury in full-endoscopic versus open microscopic lumbar spine surgery: a prospective observational study.

OBJECTIVE: Full-endoscopic lumbar surgery may minimize paraspinal muscle damage by avoiding extensive muscle dissection and retraction. However, objective biochemical comparisons of muscle injury between full-endoscopic and conventional open microscopic lumbar surgery remain limited. The purpose of this study was to compare postoperative changes in biochemical serum markers of skeletal muscle injury following open microscopic versus full-endoscopic lumbar decompression. METHODS: This prospective observational study included adult patients undergoing lumbar decompression surgery for degenerati

Effect Modifiers of the Association Between Physical Activities and Subsequent Low Back Pain Flares.

STUDY DESIGN: Prospective case-crossover study. OBJECTIVE: This study evaluated whether concurrent actions, work-relatedness, and analgesic use modify the associations between physical activities and subsequent LBP flares. SUMMARY OF BACKGROUND DATA: Prior longitudinal case-crossover studies found transient risks of various activities on LBP flares. These studies have focused on the main effects of exposures for LBP. However, little is known about contextual factors that could modify activity-flare relationships. METHODS: This was a secondary analysis of a 1-year prospective case-crossover

Preoperative Contralateral Joint And/Or Back Pain Impact on Achieving the Substantial Clinical Benefit for Hip Disability and Osteoarthritis Outcome Score-Joint Replacement.

BACKGROUND: The Centers for Medicare & Medicaid Services recently mandated reporting of patient-reported outcome measures following primary total hip arthroplasty (THA), with emphasis on the substantial clinical benefit (SCB) threshold for the Hip Disability and Osteoarthritis Outcome Score-Joint Replacement (HOOS-JR). The impact of preoperative back and/or contralateral hip pain on achieving SCB and other validated patient-reported outcome measure thresholds remains unclear. METHODS: We retrospectively analyzed a prospective cohort of 4,412 Medicare primary elective unilateral THAs (2016 to

Case 350: Retained Appendicolith.

History A 16-year-old girl presented to the emergency department with a 1-month history of right lower back pain. She was originally seen at an urgent care clinic, where a lumbar radiograph was reportedly normal, and she was prescribed naproxen. However, she came to the emergency department once pain had escalated to 10 out of 10. Over the past month, the patient's right flank had increased in size and "hardened" to touch. She reported no trauma or other inciting incident. She had a prior history of laparoscopic appendectomy for perforated appendicitis more than 6 years earlier at an outside h

Effectiveness of chiropractic care for low back pain: protocol for an updated systematic review and meta-analysis of randomized trials.

A 2016 review of randomized trials found that chiropractic care was as effective as physiotherapy for low back pain and was unable to establish the effectiveness of chiropractic care vs. exercise therapy or medical care; however, this review had several methodological limitations. We will conduct an updated systematic review to address these limitations and determine the effectiveness of chiropractic care compared to other usual care approaches for adults with non-specific low back pain. We will search for eligible trials indexed in MEDLINE, AMED, Embase, CINAHL, the Index to Chiropractic Lite

Comparison of Acupuncture and Lumbar Traction for Lumbar Disc Herniation and Subsequent Risk of Lumbar Fusion or Discectomy: A Retrospective Cohort Study.

BACKGROUND: Lumbar disc herniation (LDH) is commonly treated with conservative therapies, but comparative evidence on acupuncture versus lumbar traction and subsequent surgery is limited. We examined whether acupuncture was associated with lower 5-year risks of discectomy or spinal fusion than traction among adults with LDH. METHODS: We conducted a retrospective cohort study using deidentified electronic health records from the TriNetX US Collaborative Network from January 1, 2000, to December 31, 2023. Adults aged 20 years or older with LDH who received acupuncture or traction after diagnosi

Post-vertebroplasty lumbar spondylodiscitis due to Cutibacterium modestum.

Percutaneous vertebroplasty-associated spinal infection is uncommon but may cause progressive vertebral destruction and can mimic tuberculous spondylitis on imaging. The detection of low-virulence skin commensals such as Cutibacterium species in deep specimens also creates a diagnostic challenge because contamination and true infection must be distinguished. We report a 78-year-old man who developed recurrent progressive low back pain shortly after percutaneous vertebroplasty for an L2 compression fracture. Baseline computed tomography (CT) and contrast-enhanced magnetic resonance imaging (MRI

Vertebral Sarcoidosis Mimicking Metastatic Disease in a Patient With Prior Melanoma: A Diagnostic Challenge.

Sarcoidosis is a multisystemic granulomatous disease that most commonly affects the lungs and intrathoracic lymph nodes. Osseous involvement is uncommon, and vertebral disease is particularly rare, often mimicking metastatic malignancy and posing a significant diagnostic challenge. We report the case of a 51-year-old woman with a history of resected melanoma who was incidentally diagnosed with sarcoidosis following abnormal findings on screening mammography. Subsequent evaluation revealed pulmonary involvement. Months later, she developed acute back pain following minor trauma, and magnetic re

Calcific Patellar Tendinopathy. A Case Report Integrating Diagnostic Ultrasound Throughout a Multi-Modal Treatment Approach.

BACKGROUND: Calcific tendinopathy can be difficult to diagnose and manage from a traditional, non-invasive physical therapy perspective. However, there is preliminary evidence supporting the use of diagnostic ultrasound and dry needling interventions to evaluate and treat the pain and disability associated with this chronic condition. The purpose of this case report was to describe the integration of diagnostic ultrasound in the successful evaluation and multi-modal treatment of a patient with calcific patellar tendinopathy. CASE PRESENTATION: A 46-year-old active female with a six-week histo

Microneedle-Assisted Cell Delivery and Therapy.

STUDY DESIGN: Secondary analysis. OBJECTIVE: To investigate the Fear-Avoidance Beliefs Questionnaire (FABQ) for its ability to predict 6-month outcomes for patients with low back pain (LBP) participating in physical therapy clinical trials. BACKGROUND: Consistent evidence suggests that fear-avoidance beliefs are predictive of short-term outcomes for patients with LBP. However, proposed cut-off scores have not been widely investigated for longer-term outcomes in samples of patients receiving physical therapy. METHODS AND MEASURES: Subjects (n = 160) were participants in 2 separate randomized

Novel use of ultrasound-guided high-intensity focused ultrasound (HIFU) may enable the neuroablation of the sacroiliac joint in a swine model: a feasibility study.

BACKGROUND: Sacroiliac joint (SIJ) dysfunction accounts for the etiology of pain in 15%-30% of low back pain cases. Some patients with conservative treatment-refractory SIJ dysfunction undergo radiofrequency (RF) ablation of the SIJ for prolonged pain relief. This procedure involves placing up to 12 RF probes in what is an invasive, resource-intensive, and time-consuming process. High-intensity focused ultrasound is an alternative neuroablative technique that is non-invasive and potentially less cumbersome. MRI-guided high-intensity focused ultrasound (MRgHIFU) had previously been successfully

Chronic lower back pain effects of music on fluoroscopic guided (CLEF) interventional pain procedures: a randomized controlled trial.

INTRODUCTION: Music medicine has shown promise in reducing anxiety and pain during medical procedures, but its effects during interventional pain procedures without sedation remain unexplored. Although previous studies have examined music medicine with procedural sedation, to our knowledge, no published research has yet explored music medicine during procedures without sedation in the chronic pain population, underscoring the novelty of our investigation. This study aimed to evaluate the impact of music medicine on pain intensity, unpleasantness, and anxiety during lumbar medial branch blocks
