Intraosseous foreign body granuloma in rotator cuff repair with bioabsorbable suture anchor.

Biodegradable implants lead to problems such as cyst formation, soft-tissue inflammation, loose implant fragments or local osteolysis. This report represents the first published case of an intraosseous foreign body granuloma in the humeral head after arthroscopic rotator cuff tear fixation with a poly-L: -lactide (PLLA) suture anchor. A 48-year-old female patient presented with pain in her right shoulder. A refixation of her right supraspinatus tendon with a biodegradable suture anchor was performed 11 months ago at an external hospital. Laboratory tests showed normal values for C-reactive pro

Complications after polyacrylamide hydrogel injection for soft-tissue augmentation.

BACKGROUND: Polyacrylamide hydrogel has gained international attention as a new injectable permanent filler that appears to be ideal for soft-tissue augmentation. However, studies on the safety of polyacrylamide hydrogel injection are limited and inconsistent. In the authors' experience, polyacrylamide hydrogel injections can induce complications. METHODS: The authors retrospectively reviewed all cases where patients injected with polyacrylamide hydrogel for cosmetic purposes between 2004 and 2007 at other clinics suffered complications that led them to come to the authors' facility. The auth

F-18 FDG PET of foreign body granuloma: pathologic correlation with imaging features in 3 cases.

PURPOSE: Retained textiles with foreign body granulomatous reaction can form mass-mimicking soft tissue tumors, which may cause diagnostic, clinical, and medicolegal problems. In this study, we present 3 cases of foreign body granuloma by retained textiles, and demonstrate FDG PET findings with detailed pathologic correlation. We also provide a review of previous literatures. MATERIALS AND METHODS: Three patients (all females; age range, 48-61 years) with histopathologic diagnosis of mass-forming foreign body granuloma caused by retained textiles were retrospectively studied. They underwent a

Periorbital lipogranuloma: a previously unknown complication of autologous fat injections for facial augmentation.

BACKGROUND: Autologous fat injection into the face is a commonly used technique in plastic surgery. Autologous fat is claimed to be a safe soft-tissue filler without a foreign body reaction. This article reports on several cases of periorbital lipogranuloma after autologous fat injection for facial augmentation. METHODS: This was a retrospective, non-comparative and interventional case series. The authors identified nine patients with periorbital lipogranuloma. All patients had a history of previous autologous fat injection into the face by different plastic surgeons. A medical record review

Factors associated with incomplete insertion of electrodes in cochlear implant surgery: a histopathologic study.

OBJECTIVES: Atraumatic and complete insertion of the electrode array is a stated objective of cochlear implant surgery. However, it is known that obstructions within the cochlea such as new bone formation, cochlear otosclerosis, temporal bone fracture, and cochlear anomalies may limit the depth of insertion of the electrode array. In addition, even among patients without obvious clinical or radiographic indicators of obstruction, incomplete insertion may occur. The current study is a histopathologic evaluation of possible sources of resistance to insertion of the electrode array using the temp

Imaging features of foreign body granuloma in the lower extremities mimicking a soft tissue neoplasm.

Foreign body granuloma is a tissue reaction for retained foreign bodies after skin-penetrating trauma. Detection of retained foreign bodies can be extremely difficult when the patients present with non-specific symptoms such as pain and/or swelling without recognizing a previous trauma. We report three patients of foreign body granulomas in the lower extremities with emphasis placed on their unique clinical and radiological features. The involved sites were the foot, posterior thigh, and posterior lower leg, with wooden splinters in two patients and a fragment of tile in one. Plain radiographs

Silicone implant for chin augmentation mimicking a low-grade liposarcoma.

The silicone implant has been widely used in facial aesthetic surgery for chin augmentation. A case of a 71-year-old woman with a lesion in the anterior mandibular region detected by radiographies and suggestive of benign soft tissue tumor is presented. The patient denied previous surgical intervention in the mental area. A biopsy was performed and initially interpreted as low-grade liposarcoma. After an immunohistochemistry study showing CD-68-positive cells and complementary information regarding a silicone implant for chin augmentation, the definitive diagnosis was silicone granuloma. This

Huge cholesterol granuloma of the middle ear extending to middle cranial fossa.

Cholesterol granuloma (CG) may erode into the middle ear, the mastoid bone and the petrous apex. However, aggressive erosion into the cranial cavity is extremely rare. Here we report a case of huge CG extending to the middle cranial fossa. Temporal bone computerized tomography showed a soft tissue mass which destroyed the bony plate of the posterior and middle cranial fossa. On magnetic resonance imaging, the mass revealed a high signal on both T1 and T2-weighted images. The mass compressed the middle cranial fossa without invasion into the brain. The CG was removed by extended cortical mastoi

Cholesterol granuloma of the maxillary sinus encountered during floor augmentation procedure: A case report.

BACKGROUND: Cholesterol granuloma (CG) is a foreign-body reaction to the deposition of cholesterol crystals. Its occurrence in the paranasal sinuses is very rare. PURPOSE: This report describes a new case of maxillary sinus CG discovered incidentally during sinus-floor augmentation for dental implant placement in a 60-year-old female patient. MATERIALS AND METHODS: The preoperative clinical and radiological examinations revealed a normal maxillary antrum with no evidence of sinus pathology. After lateral osteotomy, a dark-green, viscous soft tissue mass appeared through the thin mucous membr

Silicone migration and granuloma formation.

The use of liquid injectable silicone for soft tissue augmentation is a controversial practice within the medical community. Injectable silicone has been illegally obtained, adulterated, and abused by nonmedical practitioners for the last five decades. Injection of silicone can result in catastrophic consequences including death, when grossly abused. Opponents of this practice cite the many serious adverse events that have occurred with its use both legally and illegally. Proponents argue that employment of a stringent regimen of use as well as a highly purified medical grade product allow for

The role of ultrasonography in detection and localization of radiolucent foreign body in soft tissues of extremities.

INTRODUCTION: Retained non-radiopaque foreign body inside soft tissue can be a cause of prolonged morbidity. Detection and localization is difficult task with conventional radiography. Ultrasonography, CT and MRI are other modes of evaluation but both of CT and MRI are expensive and not easily available. METHODS: Twenty three patients were evaluated with ultrasonography (8 MHz linear probe) and X-ray for clinically suspicious non-radiopaque foreign body in soft tissue of extremities. Clinical presentation, duration of symptoms, anatomical location and foreign bodies retrieved after surgical e

Outcome and complications of treatment of ankle diastasis with tightrope fixation.

Fixation of ankle syndesmosis injury with a fibre-wire tightrope has previously been reported. Early clinical results indicate that it can remain in situ indefinitely without complications. We are the first to report complications with the use of this device. Six patients were treated for ankle diastasis using the tightrope. These included four Weber C fractures, one Maisonneuve fracture and one isolated diastasis without fracture. Fractures were treated according to AO-ASIF principles and the tightrope applied through a plate in three cases and directly through the fibula in three cases. In t

Detection of bacteria by fluorescence in situ hybridization in culture-negative soft tissue filler lesions.

BACKGROUND: Adverse reactions to polyacrylamide gel occur as swellings or nodules, and controversy exists whether these are due to bacterial infection or an autoimmune reaction to the filler. OBJECTIVES: Biopsies from culture-negative long-lasting nodules after injection with different types of polyacrylamide gel were examined with a combination of Gram stain and fluorescence in situ hybridization. RESULTS: Bacteria were detected in biopsies from seven of eight patients. They inhabited gel and intervening tissue and tended to lie in aggregates. CONCLUSION: This study supports the assumption

Cholesterol granuloma of the tympanic membrane presenting as a blue eardrum.

Intramembranous tympanic membrane cholesterol granuloma (CG) occurs infrequently. Here, the authors report a case of CG in the tympanic membrane presenting as a blue eardrum in the right ear. In addition, a pinhole perforation noted in the anterosuperior area revealed a brown discharge. High-resolution temporal bone CT showed a bulging mass shadow in the middle ear and a soft tissue dense lesion that filled both the epitympanum and mastoid cavity. Tympanomastoidectomy was performed under general anesthesia. New bone formation was confirmed in the mastoid antrum and epitympanum, and the epitymp

Foreign body granulomas after injection of Bio-alcamid for lip augmentation.

Bio-alcamid is one of the newest agents on the market for soft tissue augmentation. Seven studies were documented in the medical literature that examined the safety of Bio-alcamid (Polymekon, Brindisy, Italy); all reported no cases of tissue migration, foreign body granulomas, allergenicity, or interference with the control of cell proliferation. On 2 separate occasions, a woman who had recently undergone lip augmentation presented at our hospital with submucosal nodules of the lip. Histologic examination revealed multiple foreign body-type granulomas composed of giant cells, epithelioid cells

Thigh mass associated with polyethylene wear-induced osteolysis after cementless total hip arthroplasty.

Polyethylene wear-induced osteolysis is a common complication of total hip arthroplasty. We report a case of a patient who had an extensile osteolytic lesion that presented as a tumor-like thigh mass associated with a polyethylene wear debris-induced granuloma 18 years after cementless total hip arthroplasty. The patient received a proximally porous-coated stem that had not fully circumferentially coated surface, and, therefore, polyethylene wear particles could be transported to the distal stem tip area by fluid pressure generated from the hip joint during activity. Biologic reaction to polye

Reactive lesions of peri-implant mucosa associated with titanium dental implants: a report of 2 cases.

The aim of this study was to report 2 novel clinical cases of reactive lesions of the peri-implant mucosa associated with titanium dental implants where metal-like particles were observed histologically. In both cases, the lesions were diagnosed as epulis, based on clinical evidence. Extirpation biopsies were carried out. Case 1 was diagnosed as pyogenic granuloma and case 2 as peripheral giant cell granuloma. The presence of metal-like particles in the tissues suggests that the etiology of the lesions might be related to the corrosion process of the metal structure. This is the first case of
