Increasing recognition of statin-associated anti-HMGCR antibody-positive autoimmune myopathy: a single-center retrospective study.

OBJECTIVE: The use of statins, which are inhibitors of the 3-hydroxy-3-methylglutaryl-coenzyme A reductase (HMGCR) enzyme, may in rare cases be associated with immune-mediated necrotizing myopathy. This condition is characterized by the presence of autoantibodies directed against the HMGCR enzyme. Previously considered very rare, the incidence of this myopathy diagnosis has markedly increased in our rheumatology unit over the past years. METHODS: We conducted a retrospective analysis of 13 patients diagnosed with statin-associated anti-HMGCR antibody (Ab)-positive immune-mediated necrotizing

Myeloid dendritic cell subsets characterise muscle in patients with inclusion body myositis and correlate with the IFN-γ pathway and effector T cell markers.

OBJECTIVES: Inclusion body myositis (IBM) is an idiopathic inflammatory myopathy characterised by type 1 inflammation, driven by muscle-infiltrating KLRG1+ and TBX21+ cytotoxic T cells. However, the cellular sources of the stimuli that trigger this effector response in the muscle of patients with IBM remain unclear. Given their role as antigen-presenting cells, we hypothesised that these may be myeloid dendritic cells (mDCs), which have previously been reported in skeletal muscle of patients with IBM. We used immunohistochemistry, immunofluorescence, single-nucleus RNA-sequencing (snRNA-seq) a

Anti-SAE Antibody-Positive Dermatomyositis Presenting With Widespread Erythema Complicated by Rosai-Dorfman Disease.

We report a rare case of dermatomyositis with positive anti-small ubiquitin-like modifier-1 activating enzyme antibody complicated by Rosai-Dorfman disease. A 41-year-old Japanese man presented with fever and general malaise, followed by developed progressive dysphagia. Physical examination revealed extensive erythema, along with characteristic heliotropic rash and erythematous keratotic plaques on the extremities. Laboratory examinations showed elevated muscle enzyme levels, and anti-small ubiquitin-like modifier-1 activating enzyme antibodies were detected by immunoprecipitation-Western blot

Clinical Characteristics and Concordance of Anti-MDA5 Antibodies: A Multicenter Australian Study.

OBJECTIVE: In Australia, anti-MDA5 antibodies are exclusively tested by a line immunoblot assay (LIA). The clinical concordance of an LIA-positive anti-MDA5 result is unclear. We aimed to describe the clinical features and determine the clinical concordance of patients with anti-MDA5 antibodies. METHODS: Electronic records of a multisite cohort (patients with positive anti-MDA5 on LIA from three states in Australia from January 2017 to September 2024) were reviewed for clinical features of dermatomyositis (cutaneous manifestations, proximal weakness, interstitial lung disease [ILD]), laborato

All-cause mortality in statin-associated immune-mediated necrotizing myopathy compared with idiopathic inflammatory myopathy.

OBJECTIVES: Statin-associated immune-mediated necrotizing myopathy (IMNM) is a distinct idiopathic inflammatory myopathy (IIM) associated with anti-3-hydroxy-3-methylglutaryl-coenzyme A reductase (anti-HMGCR) antibodies. The present study determined all-cause mortality among patients with statin-associated IMNM. METHODS: In this retrospective cohort study of 142 adult patients with IIM evaluated at a tertiary academic medical center, statin-associated IMNM was defined by IMNM with recent statin exposure. IIM was defined by standard criteria. The primary outcome was all-cause mortality. Age-ad

Patients with anti-PM/Scl-positive and idiopathic inflammatory myopathy resemble anti-synthetase syndrome.

BACKGROUND: Anti-PM/Scl autoantibody has been associated with an overlap between polymyositis (PM) and systemic sclerosis (SSc). However, due to limited studies, the relevance of this autoantibody in patients with idiopathic inflammatory myopathies (IIMs) without SSc was analyzed. METHODS: This single-center retrospective cohort study was conducted between 2004 and 2024. A total of 93 adult patients with IIMs (66 with dermatomyositis and 27 with PM - EULAR/ACR 2017) without SSc were included: 16 anti-PM/Scl(+) and 77 anti-PM/Scl(-). Characterizing local antibody responses in the muscle of inclusion body myositis patients.

OBJECTIVE: Sporadic inclusion body myositis (IBM) is the most common adult idiopathic inflammatory myopathy. IBM etiology has been elusive, due to both degenerative and autoimmune disease features found on muscle biopsy, and significant disease heterogeneity. Investigating the role of antibodies in the muscle of IBM patients may improve our understanding of disease pathogenesis. METHODS: We used an IBM xenograft mouse model in which muscle biopsy tissue from IBM patients (n = 98) and controls (n = 131; including 54 from other types of myopathy) were implanted into immunodeficient mice (NOD-Ra

Anti-Mi-2 positive interstitial lung disease (ILD): A progressive disease comparable to other myositis-ILD.

BACKGROUND: Evaluation for interstitial lung disease (ILD) often involves sending a myositis panel that includes myositis-associated and myositis-specific antibodies (MAA and MSA respectively) such as anti-Mi-2. Little is known about anti-Mi-2 positive ILD. We sought to determine the typical presentation and prognosis of anti-Mi-2 positive ILD. METHODS: We performed a retrospective chart review of patients in two ILD referral centers in Boston, MA with a positive anti-Mi-2 antibody between 2012 and 2024. Patients were identified by query of the medical record for patients with anti-Mi-2, and

Dysregulated type I/III interferon system in circulation from patients with anti-MDA5-positive dermatomyositis.

Anti-melanoma differentiation-associated gene 5 (MDA5)-positive dermatomyositis (DM) is often complicated by rapidly progressive interstitial lung disease (RP-ILD), leading to early mortality. Previous studies on the pathogenesis of anti-MDA5-positive DM highlighted type I interferons (IFNs), while recent investigations reported the significance of a type III IFN, IFN-λ3. We investigated a range of cytokines, including type I/II/III IFNs, in serum samples from anti-MDA5-positive DM patients collected at diagnosis before treatment introduction. Elevations of IFN-β and λ3 were identified as the

Alu Overexpression Leads to an Increased Double-Stranded RNA Signature in Dermatomyositis.

OBJECTIVE: Dermatomyositis is an autoimmune condition characterized by a high interferon signature of unknown etiology. Because coding sequences constitute <1.2% of our genomes, there is a need to explore the role of the noncoding genome in disease pathogenesis. Our genomes include roughly 1.2 million Alu elements occupying approximately 10% of the genome, which can form double-stranded (ds) RNA capable of triggering MDA5 leading to interferon production. METHODS: We aligned muscle biopsy RNA sequencing data to the telomere-to-telomere reference genome and quantified short interspersed elemen

Idiopathic inflammatory myopathies: a comprehensive review, new concepts on diagnostic imaging and whole-body MRI.

Idiopathic inflammatory myopathies (IIM) constitute a heterogeneous group of rare autoimmune, chronic inflammatory muscle disorders, resulting in both musculoskeletal and systemic manifestations. They are the leading cause of potentially treatable myositis in both pediatric and adult populations. Precise diagnosis is imperative, as treatment options differ for each disease, directly influencing the patient's therapeutic response and prognosis. The integration of pathological, clinical, serological, and imaging data is vital because of the nonspecific or nearly normal histopathological findings

Anti-HMGCR-Antibody-Positive Statin-Induced Myositis: A Pilot Case Series on Treatment with Bempedoic Acid and Immunosuppressive Therapy.

BACKGROUND/OBJECTIVES: Immune-mediated necrotizing myopathy (IMNM) is a severe inflammatory myopathy marked by proximal muscle weakness, elevated creatine kinase (CK), and the presence of anti-HMGCR antibodies. Statin exposure is a recognized trigger for anti-HMGCR-positive IMNM, which may persist despite statin withdrawal. This pilot case series explores, for the first time, the use of bempedoic acid-a liver-specific lipid-lowering agent with minimal muscle toxicity-as an alternative to statins in these patients. METHODS: We report 10 anti-HMGCR-antibody-positive IMNM patients (6 females, 4

Treatment resistance in inclusion body myositis: the role of mast cells.

Inclusion body myositis is the commonest acquired myopathy in those over 50 years of age. Although it is classified as an idiopathic inflammatory myopathy and the most frequent finding on muscle biopsy in inclusion body myositis is an endomysial inflammatory infiltrate, it is clinically distinct from other myositis, including a lack of response to immunosuppressive medication. Neurogenic changes are commonly reported in inclusion body myositis and inflammatory changes are observed in muscle following neurogenic injury. The objective of our study was to explore whether neurogenic inflammation p

Clinical characteristics of idiopathic inflammatory myopathies related to anti-SRP: a single center experience.

Idiopathic inflammatory myopathy (IIM) with autoantibodies recognizing the signal recognition particle (SRP) is characterized by prominent proximal weakness, infrequent extramuscular involvement, dramatically elevated creatine kinase levels, and myofiber necrosis with few inflammatory cell infiltrates in muscle tissue. To enhance understanding of the clinically used diagnosis and treatment of this disease, this study presents a single-center experience and analysis of a Chinese cohort with anti-SRP IIM. The most recent European Neuromuscular Center criteria were used to include Anti-SRP IIM pa

Focal myositis: a literature review of clinical and immunopathological aspects.

OBJECTIVES: Focal myositis (FM) is a rare and restricted skeletal muscle inflammation, presenting as a solid mass with a typical lower leg localization and benign prognosis. In most cases the process solves spontaneously or after immunosuppressant therapy, but sometimes it recurs or progresses to a systemic inflammation. The basis of the disease are mostly unknown. METHODS: Hence, we provide an update of histopathological features of FM, in order to better define the underlying pathomechanisms of this disorder. A PubMed literature search was focused on the case reports published in English fr
