Brain imaging in dystrophinopathies.

Duchenne and Becker muscular dystrophies (DMD, BMD) are caused by genetic variants in the DMD gene coding dystrophin proteins. In addition to the impairment of muscular function, these patients also present frequent cognitive and neuropsychiatric disorders due to impaired expression of several brain dystrophins: Dp427, Dp140 and Dp71. Cerebral imaging has shed new light on the impact of dystrophinopathies on brain structure, function, and connectivity. We review the brain imaging results obtained in patients and divers animal models, showing presence of mild cortical atrophy, slight modificati

Practical Approaches to Nutritional Interventions for Malnutrition, Sarcopenia, and Frailty in Community-Dwelling Older Adults and Care Home Populations: A Narrative Review.

BACKGROUND: Average life expectancy has increased substantially on a global scale; this demographic shift presents considerable public health challenges, as increased longevity is accompanied by higher prevalence of chronic diseases, functional decline, and dependency. Malnutrition, sarcopenia, and frailty frequently coexist in older adults, particularly among care home residents. These interrelated conditions contribute to adverse clinical outcomes including falls, hospitalisation, and reduced quality of life. Despite their growing recognition, gaps remain in understanding optimal nutritional

miR-486 attenuates myocardial ischaemia/reperfusion injury by inhibiting apoptosis and autophagy activation via the FOXO1/PTEN/AKT/mTOR axis.

BACKGROUND: Effective therapeutic interventions for myocardial ischaemia/reperfusion injury (MIRI) are lacking. The expression of miR-486, a cardiac-enriched microRNA, is downregulated in ischaemic myocardium; however, its functional role and underlying molecular mechanisms in MIRI remain unclear. METHODS: To overexpress miR-486 in vivo, MIRI rat models were administered miR-486 AAV. Cardiac function was assessed using echocardiography, infarct size using 2,3,5-triphenyltetrazolium chloride staining, and collagen deposition using haematoxylin and eosin and Masson's trichrome staining. Lactate-Informed Exercise Prescription in Mild Cognitive Impairment and Alzheimer's Disease: A Translational Framework for Preserving Cognition, Mobility, and Independence.

BACKGROUND: This narrative review proposes a conceptual, lactate-informed framework for individualized exercise prescription in mild cognitive impairment (MCI) and Alzheimer's disease (AD). Exercise is recommended in these populations, but fixed external doses do not account for heterogeneity in disease stage, frailty, sarcopenia, cardiovascular risk, or recovery. MAIN TEXT: We integrate clinical exercise evidence, geriatric assessment, lactate physiology, muscle-brain signaling, implementation science, and trial methodology. The framework uses peripheral blood lactate as one component of a m

Targeting Muscle-Specific E3 Ligases in Cancer Cachexia Through Structure-Based Virtual Screening of Small-Molecule Inhibitors.

Cachexia is a multifactorial metabolic syndrome characterized by severe skeletal muscle loss that cannot be reversed by nutritional support. Common in cancer and other chronic diseases, it significantly increases morbidity and mortality. Muscle wasting is primarily driven by activation of the ubiquitin-proteasome pathway through the E3 ubiquitin ligases MuRF1 and Atrogin-1, which promote protein degradation and muscle atrophy. This study used computational modeling and docking to identify small-molecule inhibitors targeting these ligases, including repurposed drugs with anti-cachectic potentia

Investigating the Role of Tetranectin in Ischaemic Heart Disease and Heart Failure.

BACKGROUND: Cardiac diseases represent 32% of deaths globally1. Ischaemic heart disease (IHD) is a leading cause of heart failure (HF). Progressive fibrosis in IHD contributes to the dysfunction in HF2. Investigation of novel proteins with prior association to extracellular remodelling provide invaluable information behind the underlying pathology of cardiac disease. Tetranectin is a plasma protein previously linked to cardiac remodelling. Recent literature highlighted its potential to compete against N-Terminal pro Brain Natriuretic Peptide as a diagnostic biomarker for HF3. This study aimed

Association of the sarcopenia index based on the serum creatinine to cystatin C ratio and genetic risk with venous thromboembolism: a population-based cohort study.

BACKGROUND: Venous thromboembolism (VTE) results from the interplay between environmental and genetic factors. The relationship between sarcopenia index (SI) and VTE risk remains unclear. The combined effects of SI and genetic susceptibility on VTE risk have not been fully elucidated. We therefore investigated these associations using data from the UK Biobank. METHODS: Cox proportional hazards models were used to assess the relationship between SI and genetic susceptibility with incident VTE. Restricted cubic spline models were applied to examine the exposure-response relationships. RESULTS:

Early detection of myocardial fibrosis in patients with coronary microvascular dysfunction in the absence of late enhancement: role of T1 mapping and extracellular volume analysis.

BACKGROUND: Coronary microvascular dysfunction (CMD) can exacerbate myocardial fibrosis, which may further impair microvascular function. However, the relationship between CMD and the extracellular volume fraction (ECV) in patients with ischemia with no obstructive coronary artery disease (INOCA) who are late gadolinium enhancement (LGE)-negative remains undetermined. We used T1 mapping and ECV to assess diffuse myocardial tissue abnormalities and extracellular compartment expansion in LGE-negative patients with CMD. We then investigated how these markers correlate with global myocardial flow

Short- and long-term effects of dronedarone on Cav3.1- and Cav3.2-T-type calcium channel current.

T-type calcium channels (TTCCs), including the Cav3.1 and Cav3.2 subtypes, play a critical role in regulating cardiac electrical activity. Their abnormal expression is closely associated with arrhythmias and cardiac dysfunction caused by myocardial infarction (MI). Dronedarone, a multi-target antiarrhythmic drug, is widely used to treat atrial fibrillation and atrial flutter but has shown adverse effects under pathological conditions such as MI. This study aims to investigate the direct effects and underlying mechanisms of dronedarone on TTCCs. Using neonatal rat cardiomyocytes, HEK293T cells

Nationwide Implementation of Newborn Screening for Spinal Muscular Atrophy in Canada.

BACKGROUND AND OBJECTIVES: Newborn screening (NBS) for spinal muscular atrophy (SMA) and reimbursement of disease-modifying therapies (DMTs) are managed provincially in Canada, creating inequities in diagnosis and treatment. We describe NBS methodology, reporting practices, and DMT reimbursement across Canada and report the first national birth prevalence of SMA. METHODS: We conducted a cross-sectional survey of laboratory directors from each provincial NBS program, validated against publicly available government sources (censored December 31, 2025). Birth prevalence was calculated using a Po

Emerging evidence suggests that circadian disruption and muscle wasting form a self-reinforcing loop.

Circadian disruption and skeletal muscle wasting frequently co-occur in critical illness, shift work, and aging. We propose a bidirectional self-reinforcing loop in which each process amplifies the other. Environmental disruption of the skeletal muscle clock impairs protein turnover and mitochondrial function, triggering atrophy. The resulting loss of muscle mass weakens peripheral clock output and disrupts systemic circadian rhythms. The forward arm is supported by genetic evidence, particularly from global Bmal1 knockout models and human sleep-deprivation studies. The reverse arm is supporte

Emerging roles of OSM and EDA2R-NIK signalling in cancer-associated muscle wasting.

Cancer-associated cachexia (CAC) is a multifactorial metabolic syndrome characterized by progressive skeletal muscle wasting that reduces physical function, compromises treatment tolerance, and contributes to cancer-related mortality. Although systemic inflammation is a major driver of CAC, the mechanisms through which circulating inflammatory signals are translated into muscle-specific catabolic programmes remain incompletely understood. Recent studies have identified oncostatin M (OSM), a member of the IL-6 cytokine family, as an important mediator of cancer-induced muscle wasting through OS

Frailty syndrome in the elderly: differences between women and men.

Frailty syndrome is a clinical condition characterized by a decline in physiological reserves, increasing vulnerability to stressors. Although its foundations had been previously explored, it was in the 21st century that its phenotype was established, defined by the presence of at least three out of five criteria: Unintentional weight loss, weakness, slowness, fatigue, and low physical activity. This model has been widely adopted due to its clinical utility. Frailty is not a homogeneous process, and recent studies show that it affects women more frequently, although men experience worse clinic
