The clinical values of laboratory inflammatory and composite indices in predicting rapidly progressive interstitial lung disease and prognosis in anti-MDA5 dermatomyositis patients.

OBJECTIVES: Rapidly progressive interstitial lung disease (RPILD) is the most severe complication of anti-melanoma differentiation-associated gene 5-positive dermatomyositis (anti-MDA5-DM) and is relevant to poor prognosis. This retrospective study aimed to identify the clinical value of laboratory inflammatory and composite indices in patients with anti-MDA5-DM and determine their role in predicting the incidence of RPILD and disease prognosis. METHODS: In total, 51 patients with anti-MDA5-DM were retrospectively enrolled from July 2018 to April 2025. Patients were categorized into RPILD and

A Case of Antinuclear Antibody-Negative Dermatomyositis with Interstitial Lung Disease.

BACKGROUND: Antinuclear antibody (ANA) is important for the diagnosis of autoimmune diseases. When ANA is positive, further specific autoantibody tests are needed to make a definite diagnosis. METHODS: This article reports a case of a patient with an autoimmune disease who had inconsistent results in the detection of antinuclear antibodies by indirect immunofluorescence assay (IIFA) and linear immunoblotting assay (LIA). RESULTS: This patient presented with negative ANA and positive anti-SSA/Ro52. CONCLUSIONS: IIF-ANA negative and LIA-ANAS positive exists in clinical tests. The combination

Lenabasum, a cannabinoid type 2 receptor agonist, exerts anti-inflammatory effects in dermatomyositis.

Dermatomyositis (DM) is an autoimmune disease classically affecting the skin, muscles, and lungs. Patients with DM have poor QOL, and current treatments fail to work in half of patients with DM. There is a need for an effective and safer therapeutic option. Lenabasum is a nonimmunosuppressive, nonpsychoactive cannabinoid receptor type 2 (CB2) agonist that promotes resolution of innate immune responses. The activation of CB2 reduces several proinflammatory cytokines implicated in DM. The purpose of this study was to conduct a multiplexed analysis of DM skin and PBMCs to examine the effect of le

Muscle Imaging in Inclusion Body Myositis: Refinement of MRI Criteria and Insights Into Upper Body Involvement.

BACKGROUND: The diagnosis of inclusion body myositis (IBM) can be delayed because of its heterogeneous clinical presentation and the lack of specific biomarkers. Muscle imaging has gained increasing relevance over the past decade and is now included among the supportive criteria in the international diagnostic guidelines. This study aimed to refine MRI criteria for IBM to facilitate clearer pattern recognition, increase their reproducibility and broader clinical applicability. We also aimed to provide a comprehensive evaluation of muscle wasting across the entire body, including less frequentl

Signal or noise? Apply myositis autoantibody line-blot immunoassays in real-world settings: implications for diagnostic accuracy.

OBJECTIVE: Interpreting myositis autoantibody line-blot immunoassay (LIA) is challenging when balancing clinical relevance with autoantibody positivity strength. We evaluated its diagnostic values in real-world clinical practice. METHODS: A retrospective observational study of consecutive myositis LIA results in National Taiwan University Hospital from April 2021 to March 2023. We categorized the LIA testing indications into: suspected IIM, single clinical domain (pulmonary, muscular, joint, skin, and cardiovascular), and autoimmune work-ups. Individual autoantibody positive predictive value

A 73-Year-Old Man With Intermittent Dysphagia and Esophageal Wall Thickening: What Is the Diagnosis?

This case report describes a 73-year-old man with more than 1 year of intermittent dysphagia, initially suspected of having esophageal malignancy based on imaging and endoscopic findings. However, multiple mucosal biopsies revealed only mild dysplasia without eosinophilic infiltration. Endoscopic ultrasound performed at our hospital revealed a preserved four-layer esophageal wall structure with diffuse thickening of the muscularis propria at 30-40 cm from the incisors. Definitive diagnosis was ultimately achieved through peroral endoscopic myotomy (POEM) combined with biopsy, which revealed ma

Post-assault myositis ossificans of the masseter muscle: A case report.

Myositis ossificans (MO) is a reactive, localized, benign lesion that typically affects the extremities in younger people. It is a unique and uncommon condition that has been difficult to fully understand regarding its causes, development, and effective treatment. Commonly seen in muscles of arms, and thighs, it seldom includes the masticatory muscles. Severe trismus results if it affects the masticatory muscles. Magnetic resonance imaging and computed tomography scans are useful methods for identifying the calcified mass. Histopathology is essential for achieving an accurate diagnosis and pla

When viral infections meet the anti-MDA5 antibody-positive dermatomyositis.

Anti-melanoma differentiation-related gene 5 (MDA5) antibody-positive dermatomyositis (anti-MDA5+ DM) is recognized as a distinct subtype of dermatomyositis, characterized by its frequent association with interstitial lung disease (ILD), particularly rapidly progressive ILD (RP-ILD), which is associated with a poor prognosis and high mortality. MDA5 functions as a cytoplasmic sensor for viral double-stranded RNA. The expression level of anti-MDA5 antibodies is positively correlated with disease severity. Notably, anti-MDA5 antibodies have been detected in patients infected with SARS-CoV-2. Whi

Strenuous Exercise Alters Brain Creatine and Glutamate/Glutamine (Glx) in Humans: Evidence From Dynamic 1H-MRS and 1H-MRSI.

The acute effects of exercise on brain energy metabolism are poorly understood. We used dynamic proton magnetic resonance spectroscopy (1H-MRS) to quantify responses of total creatine (tCr), glutamate/glutamine (Glx), and lactate across brain regions following a single bout of strenuous exercise. Sixteen healthy adults (nine female, age: 25 ± 3 years) completed ~15 min of cycling, reaching ~85% of predicted heart rate maximum. Single-voxel 1H-MRS was acquired at 3 Tesla from frontal and visual cortices before (PRE), 15- (POST15), and 30-min (POST30) following exercise. Multivoxel 1H-MRSI was a

Silymarin Attenuates Arthritis and Myositis in a Murine Model of Acute Infection by Chikungunya and Mayaro Viruses.

The alphaviruses chikungunya (CHIKV) and Mayaro (MAYV) are responsible for acute febrile illnesses often accompanied by severe and persistent joint and muscle pain. Due to the lack of specific treatment, research into antivirals against these emerging viruses is seen as an urgent need. Previous studies demonstrated that silymarin exhibits potent antiviral activity against CHIKV and MAYV. Then, given the promising antiviral profile of silymarin, and the prominent joint and muscle pain caused by these viruses, we evaluated whether silymarin could reverse these damages in a murine model of alphav
