BACKGROUND: An ageing population and increasing chronic health disorders explain the predicted rise in hand surgeries. Preoperative care has the potential to optimise patients' psychological and physical status, making it a promising area for enhancing surgical outcomes. Despite its established use in other surgical specialties, preoperative therapy in hand surgery remains underexplored.
OBJECTIVE: This study uses an anonymous survey to gather therapists' opinions on the hand procedure whose outcomes benefit most from preoperative therapy. The research focuses on five common elective hand pro

An accessory flexor pollicis longus tendon was found in 25 of the 53 patients who underwent trigger thumb release, as well as in two of five randomly selected cadaveric thumbs. Cadaveric and histopathologic analyses confirmed these variants. Their anatomical, clinical and evolutionary significance is discussed.

PURPOSE: Obesity is known to cause a low-grade inflammatory environment that can influence the development of musculoskeletal disorders. We hypothesized that patients with high body mass index (BMI), defined as ≥30, would develop carpal tunnel syndrome (CTS) or stenosing tenosynovitis (TF) earlier than patients with a normal BMI.
METHODS: A retrospective chart review was performed for adults diagnosed with CTS or TF between January 2016 and December 2020 at a tertiary center. The primary outcome was age at diagnosis of CTS or TF. A high BMI was considered ≥ 30, whereas a normal BMI was 18.5-

RATIONALE: Trigger thumb, characterized by A1 pulley thickening secondary to tendon sheath stenosis, represents a common clinical entity. While multiple therapeutic options exist, ultrasound-guided acupotomy has gained significant clinical adoption in China yet remains inadequately documented in the scientific literature. This study reports a case of trigger thumb managed with ultrasound-guided acupotomy.
PATIENT CONCERNS: A 49-year-old female presented with a 3-month history of audible clicking and discomfort during flexion-extension of the left thumb. Physical examination revealed pain, swe

PURPOSE OF REVIEW: This review examines extracardiac manifestations of cardiac amyloidosis and their potential role in early diagnosis and management. Multisystem involvement, including musculoskeletal, peripheral nervous system, and other extracardiac findings are highlighted to serve as red flags for earlier detection.
RECENT FINDINGS: Early screening for high-risk individuals over 50 with CTS or spinal stenosis could enable earlier intervention. Peripheral neuropathy and CTS often precede cardiac involvement by 5-10 years, while spinal stenosis can precede it by 5-15 years. Other indicator

PURPOSE: Temporarily withholding antithrombotic medications for surgical procedures can increase thromboembolic risk. Many patients undergoing minor hand surgeries take these medications, requiring careful perioperative management by surgeons. Although general guidelines exist for periprocedural antithrombotic management, guideline adherence among surgeons performing minor hand procedures is unknown. This study aimed to assess current pre-procedural antithrombotic management strategies employed by hand surgeons for patients undergoing minor hand surgeries, specifically focusing on how closely

INTRODUCTION AND IMPORTANCE: Trigger finger, or stenosing flexor tenosynovitis, is a condition characterized by a mismatch between the flexor tendon and its surrounding sheath at the A1 pulley, which facilitates smooth movement. We are presenting a rare case of bilateral index and ring fingers triggering in a 5-year-old boy, which necessitates surgical release, and a brief literature review.
CASE PRESENTATION: We report a case of 5-year-old boy who presented to our clinic with his mother with 1 year history of bilateral hands multiple fingers triggering. Mother reported on and off bilateral i

This study investigated the effectiveness and safety of ultrasound-guided non-incisional thread trigger finger release on cadavers using a newly developed domestic thread (Smartwire-01). Ultrasound-guided non-incisional thread trigger finger release was performed on 12 fresh cadaveric hands, including 12 thumbs and 48 long fingers. Two experts, experienced in an ultrasound-guided thread transecting technique, performed the procedures independently. The distal-to-proximal and proximal-to-distal approaches were performed in 6 hands each to determine which is safer and more effective. After the p

BACKGROUND: Patients with trigger finger who are refractory to nonsurgical treatments require trigger finger release (TFR) using A1 pulley release, while those with persistent triggering or severe proximal interphalangeal joint contracture may necessitate additional flexor tendon excision (FTE). This study characterizes the frequency of FTE performed at the time of primary TFR and compares outcomes between isolated TFR and TFR with additional FTE for trigger finger management.
METHODS: A total of 8551 patients who underwent TFR were retrospectively reviewed, among whom 218 (2.5%) required add

Although trigger finger release is a safe and effective procedure, complications occur due to improper technique. Incomplete release of the A1 pulley risks relapse of triggering, whereas release of the A2 pulley places patients at risk for bowstringing. Complications are often the result of poor exposure, misplaced incisions, and difficulty visualizing where the A1 pulley ends and the A2 pulley begins. The transition between A1 and A2 may be distinct in some patients and obscure in others. With the A1 pulley being the most common site of triggering, the purpose of this article was to illustrat

PURPOSE: Point-of-care ultrasound (POCUS) is increasingly recognized as a valuable adjunct to physical examination in various clinical settings. Despite this, integration into the routine clinical practice of hand surgeons remains limited. This systematic review and meta-analysis aims to evaluate the diagnostic and clinical utility of POCUS in hand clinics for the assessment and management of common hand pathologies.
METHODS: Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, MEDLINE and Embase were searched. Prospective studies evaluating POCUS

PURPOSE: This study aimed to identify the incidence of and risk factors associated with soft tissue hand conditions in pregnancy and postpartum. We hypothesized that the incidence of de Quervain tenosynovitis (DQT) and possibly carpal tunnel syndrome (CTS) and trigger finger would be higher in pregnancy and postpartum and that gestational diabetes would be a risk factor for these conditions.
METHODS: Using the PearlDiver administrative claims database, we identified pregnant females undergoing vaginal or Cesarean delivery from 2011 to 2022. This cohort was propensity score matched based on ag

INTRODUCTION: Dialysis-related amyloidosis (DRA) is a refractory complication of long-term dialysis caused by β2-microglobulin (β2MG) deposition. While β2MG adsorption columns remove β2MG, their impact on preventing re-surgery remains unclear.
METHODS: This a noninterventional, population-based cohort study used a large-scale medical claims database. This study included patients with chronic kidney disease undergoing maintenance hemodialysis (HD) and diagnosed with DRA. The exposure group received β2MG adsorption column therapy, while the control group was divided into online hemodiafiltratio

Introduction: Emergency department (ED) visits following elective surgeries place significant strain on already overburdened healthcare systems. In Nova Scotia, carpal tunnel release (CTR) and trigger finger release (TFR) are the most common elective hand surgeries performed. This study sought to determine the rates, reasons, and risk factors associated with ED visits following elective outpatient hand surgery. Methods: Patients who underwent CTR or TFR between April 1, 2016 and March 31, 2022, and visited any Nova Scotia ED within 90 days of surgery were identified using provincial healthcare
