Biphasic Brachial Plexus Blockade Following Interscalene Administration of Liposomal Bupivacaine (Exparel): A Case Report.

This case report describes a 70-year-old male who experienced a biphasic blockade following an interscalene block with 10 mL of liposomal bupivacaine (Exparel) mixed with 5 mL of 0.25% bupivacaine for shoulder arthroscopy. The patient exhibited complete motor and almost complete sensory recovery by postoperative day (POD) 1, followed by a recurrence of motor and sensory deficits on POD2, which resolved spontaneously by POD3. To our knowledge, this represents one of the earliest documented recurrences of dense motor blockade after brachial plexus block with Exparel. Potential mechanisms, includ

Prospero prevents lineage-specific loss and gain by regulating cyclin E and mitosis during Drosophila neurogenesis.

In Drosophila, neuroblasts (NBs) undergo self-renewing asymmetric divisions to generate ganglion mother cells (GMCs). Each GMC undergoes a terminal asymmetric division to generate two neurons. It was previously suggested that the loss of function of the chromatin-binding protein Prospero causes dedifferentiation of NB progeny into stem cells. Here, we revisit the role of Prospero during neurogenesis. We find that prospero loss-of-function causes CNS cells to mis- and co-express NB, GMC, and neuronal genes. A significant number of these prospero mutant neurons project axons. Therefore, NB proge

Temporal prediction captures retinal spiking responses across animal species.

The retina's role in visual processing has been viewed as two extremes: an efficient compressor of incoming visual stimuli, akin to a camera, or a predictor of future stimuli. Addressing this dichotomy, we developed a spiking neural network model of the retina trained on natural movies under metabolic-like constraints to either encode the present or to predict future scenes. When optimized for efficient temporal prediction ~100 ms into the future, the model not only captures retina-like receptive fields and their mosaic-like organizations, but also exhibits complex retinal processes such as la

Quadrant-specific anatomical architecture of the peripapillary retinal nerve fiber layer: a spectral-domain optical coherence tomography study.

PURPOSE: To evaluate the quadrant-specific anatomical organization of the peripapillary retinal nerve fiber layer (RNFL) and identify the independent determinants of global RNFL thickness in healthy adults using spectral-domain optical coherence tomography (SD-OCT). METHODS: This retrospective cross-sectional study included 188 healthy adults (18-76 years). Medical records containing SD-OCT-derived peripapillary RNFL and optic nerve head measurements were reviewed retrospectively. Only right eyes were analyzed to avoid inter-eye correlation. Quadrant-specific RNFL differences were evaluated u

Different patterns of eye-specific input are relayed to overlapping populations in the mouse superior colliculus.

The mouse superior colliculus (SC) regulates critical behaviors such as prey capture and visual threat avoidance, each of which depend on binocular visual processing. In contrast to our previous understanding, recent work has demonstrated widespread and diverse binocular interactions in the mouse SC. The SC receives direct input from contralaterally and ipsilaterally projecting retinal ganglion cells (contra-and ipsi-RGCs), but the ways in which eye-specific information is filtered in the SC remain unclear. Here, we utilized a trans-synaptic, intersectional viral tracing strategy to label SC n

Anti-inflammatory IL-4 shapes the architecture of presynaptic glutamatergic puncta of human and murine sensory neurons for pain sensitivity.

The pleiotropic anti-inflammatory cytokine interleukin 4 (IL-4) not only modulates immune responses but also plays important roles in the nervous system by mediating itch, antinociception and neuroregeneration. Here, we assessed the expression and function of IL-4, its receptors, and downstream signaling pathways in human induced pluripotent stem cell derived nociceptors (iNocs), organotypic mouse spinal cord cultures, and in in vivo models. In silico analyses of human and mouse dorsal root ganglion (DRG) single cell transcriptomes revealed that a specific subpopulation of murine and human sen

Exploration of biodistribution of mRNA-delivering lipid nanoparticles in the neonatal mouse inner ear.

Hearing loss affects millions worldwide, yet hearing aids and cochlear implants have major limitations. mRNA-based gene therapy could address the underlying molecular causes, but lipid nanoparticle (LNP) delivery in the inner ear remains underexplored and its biodistribution has not been systematically characterized. Here we establish and scale up an mRNA-encapsulated LNP formulation using a design-of-experiments (DoE) approach built on predefined critical quality attributes, and map the route-dependent distribution of reporter mRNA expression in the neonatal mouse cochlea. In HEI-OC1 auditory

Genetic susceptibility and neuroprotective gene therapy of normal tension glaucoma: a review.

Normal-tension glaucoma (NTG) is a distinct form of glaucomatous optic neuropathy characterized by progressive retinal ganglion cell (RGC) degeneration and visual field loss despite intraocular pressure (IOP) remaining within the statistically normal range. Although IOP reduction remains the only established disease-modifying strategy, continued disease progression despite adequate IOP control highlights the importance of IOP-independent mechanisms. Increasing evidence implicates vascular dysregulation, mitochondrial dysfunction, oxidative stress, impaired autophagy, neuroinflammation, axonal

A Ca²⁺ level-independent component of recovery from sensory adaptation supports continuous vesicle supply at cochlear hair cell ribbon synapses.

Maintenance of synaptic transmission can be accelerated by mechanisms dependent on intracellular Ca2+ signaling. However, at the first auditory synapse between sensory inner hair cells (IHCs) and spiral ganglion neurons (SGNs) in the cochlea, the process of synaptic vesicle recovery following strong, prolonged stimulation is unclear. To address this, we examined the Ca2+-dependence of presynaptic recovery in IHCs with measurements of postsynaptic activity in SGNs while depolarizing the IHC. Following vesicle-depleting stimulation of 1 second duration, SGN postsynaptic responses recovered compl

Echoes of Renewal: Rebuilding the Mammalian Inner Ear.

Hearing loss and dizziness are the most common sensory disorders worldwide. About 5% of the world's population is affected by disabling hearing loss, and the prevalence is projected to increase over the next several decades. Cochlear hair cells are essential in converting sound vibrations into electrical signals, which are then relayed centrally via the spiral ganglion neurons and lead to sound perception. The irreversible loss of cochlear hair cells is the most common pathology underlying sensorineural hearing loss in humans. In stark contrast, nonmammalian species such as chicken and zebrafi

Osteoid osteoma of the talus adjacent to the subtalar joint: a rare location and diagnostic challenge.

INTRODUCTION AND IMPORTANCE: Osteoid osteoma is a common benign bone tumor, but lesions located in the talus, especially those adjacent to the subtalar joint, are relatively rare and prone to misdiagnosis. Its clinical and imaging manifestations are often atypical, which can lead to delayed diagnosis and inappropriate conservative treatment. Accurate imaging evaluation and appropriate surgical management are critical for symptom relief and functional recovery. CASE PRESENTATION: We present a 20-year-old male with chronic left ankle pain that worsened at night and with weight-bearing. Initial

Diabetic retinal neurovascular unit injury: mechanisms, imaging evaluation, and integrated therapeutic strategies.

AIM: Diabetic retinopathy (DR) has long been regarded as an isolated retinal microvascular disease. Histopathological, molecular, imaging, and multi-omics evidence now supports disruption of retinal neurovascular unit (NVU) homeostasis as a central mechanism of DR, in which neuronal degeneration, reactive gliosis, and microvascular damage reinforce one another. This narrative review summarizes the anatomy and physiology of the NVU, its temporal evolution in DR, the molecular pathways of neurovascular crosstalk, quantitative imaging assessment, and integrated therapeutic strategies, and indicat

Advances in Stem Cell Therapy for Hearing Loss.

Hearing loss is the most common sensory disorder, causing substantial impacts on language development, literacy, mood, and cognition. Sensorineural hearing loss, caused by injury to the cochlear hair cells, cochlear nerve, or central auditory pathways, accounts for most cases of hearing loss. Existing therapies for sensorineural hearing loss include hearing aids and cochlear implants; however, these modalities have limited efficacy in noisy environments, variable hearing outcomes, and limited frequency discrimination. There are currently no Food and Drug Administration-approved cellular, molec

Evaluation of the brachial plexus and spinal cord with diffusion tensor imaging and synthetic MRI in obstetric brachial plexus palsy: DTI of brachial plexus and spinal cord in OBPP.

PURPOSE: Obstetric brachial plexus palsy (OBPP) is conventionally staged as a peripheral nerve injury, yet conventional magnetic resonance imaging (MRI) poorly reflects microstructural neural and spinal cord changes. We evaluate whether magnetic resonance image compilation (MAGIC), a form of synthetic MRI, shows strong agreement with conventional T2-weighted short-tau inversion recovery (T2-STIR) for postganglionic injury detection, compare diffusion tensor imaging (DTI) metrics between injured and adjacent intact segments of the brachial plexus and spinal cord, and explore whether these metri
