Synovial inflammation and immune-stromal crosstalk in osteoarthritis-related synovial fibroblast research, 2006-2025: a bibliometric analysis.

BACKGROUND: Osteoarthritis (OA) is increasingly understood as a whole-joint organ disease involving cartilage, synovium, subchondral bone, and the local immune-stromal microenvironment. Synovial fibroblasts and fibroblast-like synoviocytes (FLS) participate in synovitis, matrix remodeling, cartilage-synovium crosstalk, pain-associated microenvironments, and disease heterogeneity. However, the knowledge structure, main knowledge sources, and frontier evolution of OA-related synovial fibroblast research remain incompletely mapped. METHODS: We retrieved publications on OA-related synovial fibrob

Effects of microtrauma on joint structural changes in the Curdlan-injected SKG mouse model of ankylosing spondylitis.

Although microtrauma is considered a potential environmental factor in the development of ankylosing spondylitis (AS), there is a lack of experimental research regarding its precise contribution to disease pathogenesis. To induce physiological microtrauma, the right ankles of SKG mice were immobilized for a week using surgical plaster and a pipette tip to stimulate compensatory mechanical overload in the contralateral limb. Following the removal of the immobilization, the mice were intraperitoneally injected with 3 mg of Curdlan. In this model, the non-immobilized left ankle served as the micr

Multimodal characterization of equine synovitis: Correlation between arthroscopic, histopathological, and biomarker assessments in septic vs. aseptic cases.

BACKGROUND: Endoscopy and synovial membrane histology are complementary modalities for evaluating synovial inflammation. Cytokine involvement in non-septic arthritis is well documented but remains poorly characterized in septic synovitis. AIMS/OBJECTIVES: This study aimed to characterize the inflammatory cytokine profile associated with septic and non-septic synovitis and assess correlations among endoscopic, histological, and synovial fluid biomarker findings. METHODS: Three groups were defined: horses undergoing elective or therapeutic endoscopy (Group 1, n=26), horses with confirmed septi

Damage-Associated Molecular Patterns and Trained Immunity of Monocyte-Macrophage Populations in Osteoarthritis: Potential Molecular Links and Therapeutic Opportunities.

Osteoarthritis (OA) is now widely discussed as a whole-joint immunometabolic disease involving cartilage, synovium, and subchondral bone, rather than as cartilage degeneration driven only by mechanical stress. Tissue injury, cell death, mitochondrial dysfunction, and extracellular matrix degradation can lead to the persistent release of damage-associated molecular patterns (DAMPs), which activate innate immune responses through pattern recognition receptors (PRRs). A key unresolved issue is why inflammation can persist or recur when there is no continuous external stimulus. Trained immunity pr

[Seronegative rheumatoid arthritis combined with elbow pigmented villonodular synovitis: A case report].

We report a case of elbow pigmented villonodular synovitis (PVNS) combined with seronegative rheumatoid arthritis (SNRA) and review relevant literature to summarize the characteristics, treatment, and outcomes of this comorbidity. The patient was a 72-year-old female. She developed swelling and pain of the right elbow in 2002, and underwent surgical treatment. Postoperative histopathology confirmed PVNS. In 2007, she developed swelling and pain in the left elbow and underwent synovectomy, and synovectomy again yielded pathology consistent with PVNS. From 2021 onwards, she experienced recurrent

Beyond Synovitis: Molecular and Neuroimmune Mechanisms Informing Target-Joint Attribution and Pain Phenotyping for Arthroplasty in Inflammatory Arthritis.

Arthroplasty can provide substantial pain and functional improvement in inflammatory arthritis, but structural indications alone do not determine how much of a patient's overall symptom burden will respond to surgery. Persistent or widespread pain may reflect active inflammation, irreversible joint damage, neuropathic or extra-articular generators, and distributed pain amplification, which we use as a descriptive term for pain extending beyond the target joint and influenced by broader sensory, symptomatic, and contextual processes. This critical structured narrative review integrates direct i

Synovial biopsy: an underutilized diagnostic tool in clinical practice: an observational study.

BACKGROUND: Synovial biopsy, an important diagnostic tool to evaluate chronic monoarthritis, undifferentiated inflammatory arthritis and neoplasms, remains underutilized in clinical practice due to perceived invasiveness and limited access to specialized techniques. This study evaluates the diagnostic utility and histopathological spectrum of synovial biopsies in a tertiary care centre. MATERIALS AND METHODS: This retrospective and prospective observational study was conducted over three years (June 2020-June 2023) in a tertiary care institute. Synovial biopsies obtained via needle biopsy or

Tenosynovitis caused by Mycobacterium malmoense.

BACKGROUND: Mycobacterium malmoense is a slow-growing nontuberculous mycobacterium (NTM) increasingly recognized as a cause of extrapulmonary infections. Tenosynovitis due to M. malmoense is rare and diagnostically challenging because of its indolent course and nonspecific clinical presentation. CASE PRESENTATION: We report a 65-year-old immunocompetent woman presenting with progressive wrist pain, swelling, and impaired hand function without prior trauma. Symptoms developed after exposure to fresh fish and shellfish. Surgical exploration revealed extensive tenosynovitis with median nerve com

18F-Fluorodeoxyglucose PET in Rheumatologic and Autoimmune Disorders.

FDG PET detects the silent inflammation of autoimmune and rheumatologic diseases even before tissue damage, as activated leukocytes overexpress glucose transporters. It has an important role in large vessel vasculitis which includes early diagnosis, disease monitoring and response assessment to treatment. In inflammatory arthritides, it quantifies synovitis and enthesitis and detects early treatment response. In connective tissue and immunoglobulin G4 related diseases, whole body imaging maps silent disease, helps exclude malignancy, and guides biopsy. It is central to cardiac sarcoidosis and

Suspected Seronegative Spondyloarthropathy in a Young Woman With Ulcerative Colitis and Concurrent Bleeding Symptoms.

Extraintestinal musculoskeletal manifestations, including peripheral and axial spondyloarthritis, are well recognized in ulcerative colitis (UC) and are frequently associated with human leukocyte antigen B27 (HLA-B27) positivity. We present a 20-year-old woman with UC who developed inflammatory-pattern polyarthralgia involving the knees, ankles, wrists, shoulders, and lower back, with morning stiffness improving with movement, alongside bruising, menorrhagia, and mild leukopenia. Examination showed bilateral lower extremity ecchymoses and mild joint tenderness without swelling or synovitis. La

Metabolic-Epigenetic Reprogramming of Fibroblast-Like Synoviocytes in Rheumatoid Arthritis: Perspectives for Traditional Chinese Medicine.

BACKGROUND: In rheumatoid arthritis (RA), persistent synovial inflammation and clinical relapse cannot be fully explained by immune activation alone, as fibroblast-like synoviocytes (FLS) can acquire stable pathogenic states that sustain synovitis and joint destruction. Metabolic reprogramming and epigenetic remodeling are functionally integrated in RA-FLS, thereby promoting hyperproliferation, invasiveness, apoptosis resistance, and inflammatory memory. METHODS AND RESULTS: In this review, we summarize the interplay between altered cellular metabolism and epigeneticmechanisms, including DNA

Microsphere System Coloading Type I Collagen and Dexamethasone Sodium Phosphate for Osteoarthritis Therapy.

Knee osteoarthritis (KOA) has no effective nonsurgical treatment. Intra-articular therapy constitutes an effective localized intervention; however, persistent lymphatic drainage and synovial vascular exchange lead to rapid drug elimination. Encapsulating bioactive agents into microsphere delivery platforms enables localized intra-articular administration, with their scaffold-based spherical architecture serving as pharmaceutical and cellular carriers or tissue engineering scaffolds, representing a highly promising therapeutic avenue for KOA. Herein, PLGA was employed to prepare microspheres an

Targeting the NLRP3 inflammasome in rheumatoid arthritis fibroblast-like synoviocytes: pathogenic mechanisms, therapeutic strategies, and translational perspectives.

Rheumatoid arthritis (RA) is a chronic systemic autoimmune disease characterized by persistent synovitis, pannus formation, cartilage degradation, and bone erosion. Although conventional synthetic, biologic, and targeted synthetic disease-modifying antirheumatic drugs have improved disease control, many patients fail to achieve sustained remission, lose responsiveness over time, or develop treatment-limiting safety concerns. Fibroblast-like synoviocytes (FLSs) are increasingly recognized as long-lived pathogenic stromal cells that sustain synovial inflammation, promote matrix degradation, supp

TYK2-Wnt5a Crosstalk in Psoriatic Arthritis: A Hypothesized Immunopharmacological Axis for Inflammation and Structural Remodelling.

INTRODUCTION: Psoriatic Arthritis (PsA) is a chronic immune-mediated inflammatory disease marked by synovitis, enthesitis, bone erosion, and pathological new bone formation, via the Interleukin-23/T helper cell-17 (IL-23/Th17) axis. Tyrosine Kinase 2 (TYK2) mediates downstream signaling of IL-23, IL-12, and type I Interferons (IFN-I), whereas Wnt5a regulates non-canonical Wnt signaling in inflammation and skeletal remodelling. This review proposes and examines TYK2- Wnt5a crosstalk in PsA as a testable mechanistic hypothesis rather than an established pathway. METHODS: A narrative literature

The infrapatellar fat pad-synovium immunometabolic interface in knee osteoarthritis: a local amplifier of joint inflammation.

Knee osteoarthritis (OA) is a whole-joint disease in which synovitis is associated with pain and structural progression. The infrapatellar fat pad (IPFP) lies next to the anterior synovium and contains adipocytes, mesenchymal and fibroblast-like stromal cells, macrophages, vascular cells, and nerves. Alongside its mechanical cushioning role, the IPFP releases adipokines, cytokines, chemokines, lipid mediators, and extracellular vesicles. Adipocyte stress, matrix remodeling, fibrosis, and immune cell accumulation alter this secretory profile in OA. Single-cell analyses of paired human tissues s

Osteoid osteoma of the talus adjacent to the subtalar joint: a rare location and diagnostic challenge.

INTRODUCTION AND IMPORTANCE: Osteoid osteoma is a common benign bone tumor, but lesions located in the talus, especially those adjacent to the subtalar joint, are relatively rare and prone to misdiagnosis. Its clinical and imaging manifestations are often atypical, which can lead to delayed diagnosis and inappropriate conservative treatment. Accurate imaging evaluation and appropriate surgical management are critical for symptom relief and functional recovery.
