S100A4-TLR4-TGF-β axis as a therapeutic target for Dupuytren's contracture in diabetic patients.

Dupuytren's contracture is a superficial fibrotic disease of the hands that causes flexion contractures of the affected fingers. Diabetes mellitus (DM) is a risk factor for Dupuytren's contracture. However, the exact underlying mechanisms by which DM is involved in its development and progression remain unknown. This study investigated the involvement of glycometabolic disorders in the pathogenesis of Dupuytren's contracture. RNA sequencing revealed that S100A4 expression was significantly increased in Dupuytren's contracture-derived fibroblasts under high-glucose conditions compared with low-

Dupuytren's Disease of the Distal Interphalangeal Joint: A Systematic Review of Case Reports and Case Series.

Background and Objectives: Dupuytren's disease (DD) most commonly affects the palm and metacarpophalangeal/proximal interphalangeal joints; distal digital involvement at the distal interphalangeal joint (DIPJ) is uncommon and incompletely characterised. This systematic review summarises reported cases of DD involving the DIPJ, with or without proximal interphalangeal joint (PIPJ) involvement, focusing on clinical patterns, management, and reported outcomes. Materials and Methods: A systematic search of PubMed, Embase, and Scopus from database inception to July 2025 identified published case re

INTRODUCTION: Wide Awake Local Anesthesia No Tourniquet (WALANT) is an established method of local anesthesia in hand surgery. In rare cases, ischemic complications may occur due to the adrenaline contained in the tumescent solution. The vasoconstrictive effect of adrenaline can be antagonized by the local injection of phentolamine. To date, little is known in Germany about the incidence and treatment of ischemic complications, as well as the state of knowledge regarding phentolamine. MATERIAL AND METHODS: An online survey of hand surgeons in Germany was used to evaluate their knowledge of th

Investigating the genetic basis of fibrotic pathway dysregulation in Dupuytren's contracture.

PURPOSE: Dupuytren's contracture (DC) is a common fibroproliferative disorder with strong genetic predisposition. This study aimed to investigate the association between genetic variants in key fibrotic pathways and DC susceptibility in a Lithuanian population. MATERIALS AND METHODS: We conducted a case-control study including 116 patients with DC and 103 healthy controls. Genotyping of four single nucleotide polymorphisms (SNPs) involved in fibrotic pathway regulation was performed: COL1A1 rs1800012, TAB2 rs2179367, SFRP4 rs1668357, and SFRP4 rs1668347. RESULTS: The COL1A1 rs1800012 A allel

The Likelihood of Future Dupuytren Disease Intervention After Initial Treatment in the Same Digit, Another Digit, and Contralateral Hand.

PURPOSE: Patients treated for Dupuytren contracture in one digit are at risk for future intervention in other digits or the contralateral hand. Identifying predictive factors can aid in patient counseling. METHODS: A single institutional database was queried for procedural codes for collagenase treatment or surgical fasciectomy. A total of 658 patients were identified in this query. Out of this cohort, 341 patients were eligible for inclusion. The cohort consists of 141 initially treated with fasciectomy and 200 initially treated with collagenase, between 2015 and 2019. The primary outcome wa

THE UNITÉ RHUMATOLOGIQUE DES AFFECTIONS DE LA MAIN (URAM): TRANSLATION INTO PORTUGUESE, CULTURAL ADAPTATION AND VALIDATION.

OBJECTIVE: To translate and culturally adapt the Unité Rhumatologique des Affections de la Main (URAM) scale, a specific instrument for functional assessment in Dupuytren's disease (DD), into Brazilian Portuguese. METHODS: Following the methodology of Beaton (2000), the scale underwent five stages: translation, synthesis, back-translation, expert committee, and pre-testing. After adaptation, the Brazilian version of the scale (URAM-BR) was administered to 40 patients with Dupuytren's contracture. Internal consistency was tested using Cronbach's alpha, reproducibility was analyzed using the in

Dabrafenib-Induced Cutaneous Eruptions in a Patient With Langerhans Cell Histiocytosis.

Langerhans cell histiocytosis (LCH) is a rare malignancy marked by clonal proliferation of Langerhans cells, with BRAF V600E mutations identified in over 50% of the cases. BRAF inhibitors (BRAFi), such as dabrafenib, have shown efficacy in treating BRAF V600E-mutant LCH. However, BRAFi therapy is associated with cutaneous adverse effects, including the development of new melanocytic nevi, verrucae, and keratinocyte carcinoma (KC). This case report describes a 78-year-old woman with BRAF V600E-mutant LCH who developed multiple dermatologic side effects following dabrafenib salvage therapy. Seve

Evaluating the Efficacy, Safety, and Patient Selection Criteria for Radiation Therapy in Early-Stage Dupuytren's Contracture.

Dupuytren's contracture (DC) is a fibroproliferative disorder characterized by progressive contractures of the hand. The abnormal thickening of the palmar fascia can significantly impair hand function and lead to progressive pain, significantly reducing patients' quality of life. Surgical interventions are commonly employed for moderate to severe cases and have long been considered the gold standard. Surgery, however, is often associated with morbidities, including complications, infection, and a significant rate of recurrence. Modern-day radiation therapy (RT) has emerged as a promising, thou

Million-dollar nerve dysfunction after Dupuytren's contracture surgery.

The thenar muscles are supplied by the median nerve's recurrent motor branch (RMB). Its integrity is crucial for normal thumb motion, despite its small size and frequent neglect. We report a 70-year-old man experienced thumb weakness after Dupuytren's contracture surgery. Sensation was unaffected, but there was noticeable thenar atrophy and decreased strength. Ultrasound and magnetic resonance imaging revealed evidence of local compression, and electromyography verified a partial RMB injury. An uncommon but clinically significant RMB dysfunction is demonstrated in this case. Wide incisions or

Causal effects of diabetes and alcohol intake on Dupuytren's disease: a two-sample Mendelian randomization study in UK Biobank and FinnGen.

BACKGROUND: Dupuytren's disease (DD) is associated with metabolic and lifestyle traits. However, these associations remain uncertain because exposures frequently cluster, and conventional studies are vulnerable to residual confounding and reverse causation. METHODS: A two-sample Mendelian randomization (MR) analysis using genetic instruments for diabetes, obesity-related, smoking, and alcohol-related traits from genome-wide association studies of large European ancestry was conducted. Summary statistics of DD were obtained from UK Biobank and FinnGen and combined using meta-analysis. Multipli

Use of Matriderm in 2 cases of Dupuytren's contracture release.

Dupuytren's contracture is a chronic fibroproliferative disorder of the hand characterized by progressive thickening and shortening of the palmar fascia, leading to flexion deformities of the fingers, particularly the ring and little fingers (1). The exact pathogenesis remains incompletely understood, however, both genetic and environmental factors play significant roles in disease development (2,3). This report highlights the use of Matriderm, a single-layer matrix consisting of collagen and elastin, possibly aiding postoperative outcomes of Dupuytren's Contracture. A 43-year-old female patie

Isolated Dupuytren's Disease in Proximal Phalanx of the Little Finger Mimicking Giant-Cell Tumor: A Rare Case Presentation.

Background and Clinical Significance: Dupuytren's disease (DD) is characterized by abnormal myofibroblast proliferation and excessive collagen deposition, leading to the formation of pathological fibrous cords. It typically affects the palmar surface of the hand, where these contractile cords cause progressive flexion contractures of the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints. Lesions involving the proximal interphalangeal (PIP) joint without significant flexion contracture may be misdiagnosed as soft-tissue tumors or inflammatory lesions based on imaging findings,

Surgical Fasciectomy Versus Collagenase Injection for Recurrent Dupuytren Contracture: A Randomized Controlled Trial.

BACKGROUND: Collagenase injection is effective in primary Dupuytren contracture but efficacy in treating recurrence has not been evaluated in randomized trials. The aim was to compare surgical fasciectomy and collagenase injection for recurrent Dupuytren contracture. METHOD: In a single-center trial, 59 patients with recurrent Dupuytren contracture were randomized to fasciectomy (28 patients) or collagenase injection (31 patients). The primary endpoints were change in total active extension deficit (TAED) in treated fingers from baseline to 12 weeks and re-recurrence rate (TAED worsening ≥ 20
