The epidemiological, etiological, and clinical comparisons of primary and recurrent Dupuytren's contractures.

BACKGROUND: Dupuytren's contracture is characterized by the thickening of the palmar fascia. Although extensive literature exists on this disease, changes in lifestyle necessitate the re-evaluation of its epidemiology, etiology, and clinical features. This study aims to revise the current characteristics of Dupuytren's contracture and to explore potential relationships between these characteristics and recurrence. METHODS: Patients who underwent surgery for Dupuytren's contracture between January 2014 and December 2016 were included in this study. Electronic health records were reviewed to co

Collagenase Clostridium Histolyticum Versus Percutaneous Needle Fasciotomy for Dupuytren's Disease: A Systematic Review and Meta-Analysis.

Minimally invasive treatments for Dupuytren's disease (DD), such as percutaneous needle fasciotomy (PNF) and collagenase clostridium histolyticum (CCH), have become alternatives to open surgeries. This meta-analysis compared these treatments in terms of complications, patient satisfaction, clinical outcomes, and recurrence. Relevant studies up to June 2024 were identified through major databases, following PRISMA guidelines, and the study was registered on PROSPERO. Statistical analysis using Review Manager 5.4 found PNF had lower post-operative rates of oedema (RR = 0.15, 95% CI [0.09, 0.27],

Adverse events after adjuvant radiation therapy for Dupuytren's disease in the DEPART randomized trial.

This study reports adverse events after adjuvant radiotherapy in patients with Dupuytren's disease. Patients for planned limited fasciectomy, collagenase injection or percutaneous needle fasciotomy were enrolled in a randomized controlled trial. Sixty patients were randomized to observation or radiotherapy (30 Gy, 10 fractions) starting within 4 days of the procedure. During a follow-up period of up to 4 years, 114 adverse events were reported. Of 102 (89%) mild adverse events, 45 (44.1%) were attributed to radiotherapy. Of the 12 moderate to severe, three were attributed to radiotherapy. By 6

Experience in the surgical treatment of Dupuytren's disease using WALANT anesthesia.

Fasciectomy is a surgical procedure frequently used for the treatment of Dupuytren's disease. The WALANT (Wide-Awake Local Anesthesia No-Tourniquet) anesthesia technique has gained popularity for its advantages over general anesthesia, such as risk reduction and cost savings. However, one of the main challenges is pain management and anesthetic efficacy during surgery. This study aims to evaluate the feasibility of this technique in pain management, patient experience, and cost savings in surgery for Dupuytren's disease. Prospective study that included patients treated surgically for Dupuytren

Extensor mechanism dysfunction and hand deformities caused by Dupuytren's disease: Surgical and rehabilitation perspectives.

Dupuytren's disease is a common fibroproliferative disorder that primarily affects the palm of the hand. While the disease is known for its characteristic palmar contractures, it also has a potential impact on the extensor mechanism of the hand, resulting in the development of boutonniere deformity, swan-neck deformity, and persistent metacarpophalangeal joint contracture due to extensor capsule stretching and tendon instability. These imbalances are challenging to correct under general or regional anesthesia. Wide awake local anesthesia without a tourniquet allows active range of motion and i

Raynaud's Phenomenon After Fasciectomy for Dupuytren's Contracture: A Case Report.

Open fasciectomy is a widely accepted treatment for Dupuytren's contracture, with known complications such as vascular and nerve injury, stiffness, and recurrence. However, delayed vascular complications beyond these known risks remain largely unexplored. A 57-year-old nonsmoking man underwent open fasciectomy for Dupuytren's contracture of the little finger. Six months postoperatively, he developed Raynaud's phenomenon in the operated finger, characterized by cold-induced pallor that resolved with rewarming. Examination revealed sluggish circulation in the ulnar digital artery and altered sen

Modified percutaneous needle aponeurotomy for Dupuytren's disease: case series with functional outcome.

PURPOSE: Over the past three decades, percutaneous needle aponeurotomy (PNA) for Dupuytren's disease has become increasingly prevalent and offers numerous potential benefits. However, significant rates of recurrences are observed in literature. We aimed to evaluate the effectiveness of our technical modifications of percutaneous needle aponeurotomy that significantly separates the cord in the palm and digits, thereby minimizing recurrences. METHODS: We treated 23 consecutive patients with our modified technique. We use a larger-bore needle in the dorsovolar direction which involves a more con

Skin marker for MR-only surface high-dose-rate brachytherapy.

PURPOSE: Recent advances in surface high-dose-rate (HDR) brachytherapy imaging indicate that flap applicators, human skin, and fibromatosis can be visualized using MRI. Complete MR-only surface brachytherapy workflows would require skin marker identification to define clinical target edges. However, CT markers are not detected on MR images, and common MR markers are unsuitable for continuous surface target tracing. In this paper, we proposed an alternative skin marker that was evaluated for MRI and CT detectability and contourability using a brachytherapy treatment planning system (TPS). MATE
