Long-term prognosis of patients surviving progressive multifocal leukoencephalopathy.

Despite improved survival in recent years, long-term outcomes in patients surviving progressive multifocal leukoencephalopathy beyond the first year remain poorly defined. This international multicenter retrospective study aimed to characterize the three-year prognosis of patients with progressive multifocal leukoencephalopathy who had survived at least one year, identify factors associated with favorable outcomes and late mortality, and determine recurrence rate. Data were collected through standardized questionnaires across forty-one centers in twelve countries. Patients were eligible if the

Viral infection and dementia risk: A systematic bidirectional Mendelian randomization analysis.

BACKGROUND: Viral infections have attracted increasing interest as potentially alterable risk factors for dementia; nonetheless, the results from observational research are variable and prone to confounding bias and reverse causation. This work seeks to systematically explore the potential causal associations between several viral infections and various forms of dementia by a Mendelian randomization methodology utilizing summary statistics and extensive genomic data from the Finnish FinnGen R12 database. METHODS: This study initially included 14 viral infections, including anogenital herpes,

Plasma Biomarkers of Neurogenesis Are Increased Among People With HIV After COVID.

OBJECTIVES: Although acute COVID does not seem to markedly differ by HIV status, the long-term impact of COVID in people with HIV (PWH) remains unclear. METHODS: Samples from 44 participants with or without HIV were obtained approximately 10 days after the initial COVID diagnosis (t = 0) and then 3 (t = 1) and 6 (t = 2) months later. Biomarkers of blood-brain barrier (BBB) and vascular dysfunction, neurogenesis, and inflammatory responses were assessed by multiplex profiling and enzyme-linked immunosorbent assay. RESULTS: The majority of inflammatory biomarkers either decreased or remained u

The Influence of Blood Brain Barrier Permeability on CSF-To-Serum Ratios of Neurobiomarkers in People With HIV.

People with HIV (PWH) have a higher risk of central nervous system (CNS) diseases and a timely differential diagnosis may be essential for patient management. Cerebrospinal fluid (CSF) biomarkers have proven effective in diagnosing neuronal and astrocyte involvement in neurological disorders, but the invasiveness of this method makes it difficult to obtain results; thus, easy-to-obtain matrices (e.g., plasma) have to be analysed. Consequently, the aim of this study was to quantify biomarkers in both serum and CSF with different kits, correlating levels obtained in the two matrices and understa

Cerebrospinal fluid markers of alzheimer's pathology relate to aMCI among people with HIV.

BACKGROUND: As people with HIV survive to older ages, they become more at risk for Alzheimer’s disease (AD) and its precursor, amnestic mild cognitive impairment (aMCI). Memory impairment is also common in other neurocognitive disorders (NDs), including HIV-associated neurocognitive disorders (HAND), which makes it a challenge to diagnose aMCI among PWH. Therefore, we assessed the utility of cerebrospinal fluid (CSF) AD pathology markers in distinguishing aMCI from HAND in PWH by investigating how these markers differentially relate to aMCI. METHODS: Participants included 74 PWH (Mean age = 48

Blood-based Alzheimer's disease biomarkers and cognitive trajectories in older people with HIV with undetectable viral loads.

INTRODUCTION: Cognitive impairment among people with HIV (PWH) remains common, yet underlying mechanisms remain unclear. Alzheimer's disease (AD) is the leading cause of dementia, and blood-based biomarkers offer a promising diagnostic alternative. We evaluated phosphorylated-tau 217 (p-tau217), neurofilament light (NfL), and glial fibrillary acidic protein (GFAP) as predictors of cognitive decline among virologically suppressed older PWH. METHODS: Thai PWH aged ≥50 years with plasma viral loads <50 copies/mL completed the Montreal Cognitive Assessment (MoCA) at baseline (2015-2017) and a fol

A revised neuropsychological test battery, CoCoBattery-Plus, for the diagnosis of HIV-associated neurocognitive disorders in Japan.

To diagnose HIV-associated neurocognitive disorders (HAND), several neuropsychological test batteries have been used in various studies and countries. In Japan, the Co-developing Comprehensive Neuropsychological Test Battery (CoCoBattery) was developed during a nationwide study conducted between 2014 and 2016 (the J-HAND study) to explore the prevalence of HAND and has been widely used thereafter. It consists of 14 tests covering eight key cognitive domains: language, attention/working memory, executive function, learning, memory, information-processing speed, visuospatial construction, and mo

Experiences of HIV-Related Stigma and Mental Illness Among HIV-Associated Meningitis Patients in Rural Uganda.

BACKGROUND: Inflammation in and around the brain in patients with meningitis can lead to confusion, cognitive dysfunction, and behavioral changes associated with mental health disorders. Stigma associated with HIV or mental illness can complicate meningitis, leading to misdiagnosis or delays in diagnosis and care. The frequency of misdiagnosis and/or co-occurrence of meningitis and mental illness among people living with HIV (PLWH) remains uncertain. We explored the experiences of meningitis patients and the barriers and facilitators to care related to HIV stigma and mental illness. METHODS:

Exploring variation in research priorities generated by AI tools.

BACKGROUND: Artificial intelligence (AI) tools based on large language models (LLMs) are being increasingly used by researchers and may play a role in health-related research priority-setting exercises (RPSEs). However, little is known about how these tools may differ in the types of research priorities they generate. METHODS: We examined research priorities aimed at improving treatments for four diseases: cancer, COVID-19, HIV, and Alzheimer. We compared the outputs from five AI tools (DeepSeek, ChatGPT, Claude, Perplexity, and Gemini) using SBERT-BioBERT embeddings and cosine similarity sco

Cerebrospinal fluid proteomics reveals inflammatory activation in aneurysmal subarachnoid hemorrhage irrespective of HIV status.

OBJECTIVES: People with HIV (PWH) are at increased risk of cerebrovascular abnormalities, including aneurysmal subarachnoid hemorrhage (SAH). However, it is unclear whether HIV-associated inflammation contributes significantly to the inflammatory response observed in the cerebrospinal fluid (CSF) during aneurysm rupture. Here, we used high-throughput Olink proteomics to compare inflammatory marker profiles in CSF between participants with aneurysmal SAH and PWH without aneurysms. DESIGN: This was a cross-sectional observational study which enrolled participants who were indicated for endovasc

Study protocol for neuroimaging using 7 T MRI in the investigation of baricitinib for reduction of HIV in the CNS: a randomized placebo-controlled trial.

BACKGROUND: While antiretroviral therapy is successful in conferring peripheral human immunodeficiency virus type 1 (HIV-1) suppression, eradication of HIV-1 reservoirs in the central nervous system (CNS) is an immediate goal in the management of HIV-1 infection. The primary goal of this randomized and placebo-controlled Phase IIb trial is to evaluate the effects of baricitinib versus placebo on HIV-1 CNS persistence markers in a ten-week mechanistic study, with secondary endpoints evaluating the effects on peripheral blood, neuroimaging, and neuropsychological measures. Given the large scope

Effects of acute THC challenge on behavior and neuroinflammation in HIV-1 Tg26 mice vary based on HIV status, chronic THC history, and sex.

Δ9-tetrahydrocannabinol (THC) has been studied for its neuroprotective benefits in disease and its ability to improve HIV-1-related symptoms in clinical and preclinical models. Chronic THC administration may cause reduced sensitivity to antinociceptive, hypothermic, and anxiolytic effects following acute THC administration, and HIV status may further influence these effects. Thus, the present study investigated the effects of an acute THC challenge after chronic THC exposure on behavioral and neuroinflammatory measures using the HIV-1 Tg26 neuroHIV mouse model. HIV-1 Tg26 transgenic [Tg26(+/-)

Structural and functional modifications of neuronal lipid rafts: implications for HIV-associated neurological disorders.

BACKGROUND: HIV-associated neurological disorders (HAND) remain a prevalent co-morbidity of HIV infection. Growing evidence implicates the HIV protein Nef in this process, as Nef increases the abundance of neuronal lipid rafts - cholesterol- and sphingolipid-rich membrane microdomains linked to neurodegeneration and neuroinflammation. Because Nef is released from infected glial cells in extracellular vesicles (exNef), we investigated whether glia-derived exNef alters the structural and functional properties of neuronal lipid rafts in a manner that could contribute to HAND pathogenesis. METHODS

Evaluating the Impact of Blood-Brain Barrier-Penetrant ACE Inhibitors on HIV-Associated Neurocognitive Disorders.

BACKGROUND: As people with HIV (PWH) live longer because of effective antiretroviral therapy, age-related comorbidities-including cognitive impairment-are increasingly prevalent. HIV-associated neurocognitive disorders (HAND) remain a major cause of morbidity. In prior work, we identified an association between blood-brain barrier (BBB)-penetrant angiotensin-converting enzyme inhibitors (ACEi) and reduced risk of dementia in PWH. We are now expanding our work and methodology to more the effects of BBB ACEi on the risk of HAND. METHODS: Using the Veterans Affairs Informatics and Computing Infr

A rest-task fMRI study of spatial working memory in HIV-infected individuals across cognitive states.

BACKGROUND: HIV-associated neurocognitive disorders (HAND) are common complications in HIV-infected individuals, and working memory impairment is one of the core features. Although combination antiretroviral therapy (cART) has reduced the incidence of severe HAND, mild HAND remains prevalent. This study aims to explore the functional brain characteristics related to working memory in HIV-infected individuals with different cognitive states using resting-state and task-based functional magnetic resonance imaging (fMRI), and to identify candidate imaging markers for early diagnosis and inform fu
