Treatment of lateral epicondylitis.

Lateral epicondylitis associated with cumulative trauma as seen in the clinical setting has usually involved long-term care with fair results. This paper describes the structured treatment program for lateral epicondylitis developed at the Michigan Hand Rehabilitation Center in Warren, Michigan. Results of the program, as reflected in five cases, have been a decrease in treatment time and a high incidence of return to work with no symptoms or reduced symptoms.

Tennis elbow.

The term "tennis elbow" has been used in the medical literature for over 100 years to describe a variety of conditions on the lateral aspect of the elbow. Recent writers seem to agree that the two most common causes for this condition, which may be either distinct or coexisting, are lateral epicondylitis and radial tunnel syndrome. Lateral epicondylitis is not limited to tennis players; 95% of the reported cases occur in non-players. The presenting symptoms are pain over the origin of the extensor carpi radialis brevis and weakness in the wrist and digital extensors. The location of maximal te

[Rupture of biceps tendon after injection of steroid].

A case of distal rupture of the biceps tendon preceded by injection of steroid on account of lateral epicondylitis three and five months previously is presented. This complication is extremely rare although the treatment is extensively employed. It is recommended that steroid should not be employed as the first choice. In addition active measures should be employed to avoid recurrence.

Effect of standard and Aircast tennis elbow bands on integrated electromyography of forearm extensor musculature proximal to the bands.

Classic tennis elbow, or lateral epicondylitis, has been described as an overuse or misuse injury resulting in a tendinitis. The extensor carpi radialis brevis (ECRB) and extensor digitorum communis (EDC) have been implicated as primary culprits in this pathology. Tennis elbow has been treated using a constrictive band placed several centimeters distal to the origin of these two muscles. Aircast (Aircast Inc., Summit, NJ) has developed a new style of band that employs an air-filled bladder as the counterpressure element. This study tested the effect of both standard and Aircast bands on EMG ac

Tennis elbow.

Tennis elbow, or lateral epicondylitis, is a common condition of the elbow resulting from overuse during athletic or occupational activity. Functionally, it is a tendinitis of the origin of the forearm extensor muscle mass. Diagnosis is mainly based on symptoms and signs, including lateral elbow pain during twisting and gripping and tenderness at the lateral forearm exacerbated by stress testing of the forearm extensors. Comprehensive treatment should include rest, use of ice, antiinflammatory drug therapy, use of sleep-cycle modifiers, and sometimes surgery in otherwise unresponsive cases.

[Evaluation of the results of conservative treatment in cases of chronic lateral epicondylitis of the humerus in workers in the metal industry].

The causes of occurrence of tennis elbow (epicondylitis humeri lateralis) in metal workers were analyzed. It has been found that the disease occurred most often in locksmiths and turners with over 20 years of service. After treatment of 35 patients which consisted in transfer to light work, paying respect to plaster cast immobilization, injections in the region of the muscular insertions and physical therapy, good results were achieved enabling 21 patients to resume their jobs.

The surgical treatment of chronic lateral humeral epicondylitis by common extensor release.

This retrospective study reports on the surgical treatment of chronic lateral epicondylitis. Thirty-four elbows were followed for an average of four years after surgery. Only those patients with persistent or recurrent local pain and muscle weakness, nonresponsive to conservative measures for at least six months, were considered for surgery. A complete release of the common extensor tendon permitted about a 1-cm distal muscle slide to a new resting length. The results indicate 73% excellent, 18% good, and 9% failures. Complete pain relief was obtained in 25 of 34 (73%) elbows, and eight of 33

Elbow pain.

Nonarticular causes of elbow pain include muscle strains, ligamentous injuries, epicondylitis, olecranon bursitis, and compressive neuropathies. Overuse and trauma commonly cause these conditions. The history and physical examination differentiate them from an intra-articular process such as synovitis. Laboratory analysis of fluid aspirated from a swollen olecranon bursa is necessary to differentiate infection or gout. X-rays are useful in avulsion fracture, osteochondritis dissecans, and epiphyseal separation. Electromyography with nerve conduction velocities can localize the site of nerve en

Chronic upper limb pain syndrome (repetitive strain injury) in the Australian workforce: a systematic cross sectional rheumatological study of 229 patients.

The epidemic of chronic upper limb pain is the most important and controversial issue in industrial rheumatology in Australia today. Two hundred and twenty-nine consecutive patients referred with chronic upper limb pain which had been labelled "repetitive strain injury" or "overuse syndrome" were assessed according to a protocol designed to give insight into the questions: Is the pain genuine or falsely reported, i.e., malingering? If genuine is it due to a physical injury, a pain syndrome, or a mixture of both? Twenty-nine patients fulfilled criteria for specific rheumatological diagnoses (fi

Tennis elbow, a curable affliction.

Forty-five patients with 47 cases of chronic lateral epicondylitis treated surgically were reviewed. Physical examination revealed increased pain with resisted wrist extension, particularly when the elbow was straight. The presence of contracture of the anterolateral elbow capsule at surgery explained the pain with extension. Release of the capsule with the extensors provided relief in 87% of patients. Operative treatment is successful in most cases of chronic tennis elbow when conservative means are exhausted.

Racquet sports--patterns of injury presenting to a sports injury clinic.

In an 8-year retrospective study, 631 injuries due to the racquet sports of squash (59%), tennis (21%) and badminton (20%) were seen in a sports injury clinic, males predominating (58 to 66%). The proportion of squash injuries was higher than expected and probably relates to higher physical stress and risk of contact in this sport. Also they occurred mainly in persons over 25 years (59%) i.e. the reverse for sport in general. Acute traumatic injuries were seen especially in squash players, a majority affecting the knee, lumbar region, muscles and ankle. Tennis injuries differed most with later

An epidemiological study of lateral epicondylitis (tennis elbow) in amateur male players.

Our epidemiological study investigated lateral epicondylitis (tennis elbow) among 150 nonprofessional male tennis players (75% of the players approached consecutively) interviewed and examined physically at four local tennis clubs. The interview and examination covered over sixty possible risk factors, including demographic data, playing habits, anatomical measurements and racket characteristics. The "average" painfree player is 38 years old, has played tennis 11 years, and plays 5.5 hours a week, usually with a racket whose head area is 570 cm2. The "average" tennis elbow sufferer is a 45-yea

Randomized trial of oral naproxen or local injection of betamethasone in lateral epicondylitis of the humerus.

A randomized pilot study, comparing oral naproxen and a single betamethasone injection, was carried out in 21 patients suffering from lateral epicondylitis of the humerus ("tennis elbow"). The naproxen dosage was 250 mg per day for two weeks. Six milligrams of betamethasone in a long-acting form was given as a local injection. To achieve "blindness," the patients receiving naproxen were also given an injection of saline into the area of maximal tenderness at the epicondyle, while the patients getting the betamethasone-injection were given oral placebo tablets. At a clinical control after two w

The prevalence of humeral epicondylitis: a survey in general practice.

The characteristics of all patients with humeral epicondylitis who presented over a two-year period in a group practice were examined to clarify the epidemiological features of this condition. In all 77 patients were seen. There was no observed difference in incidence between the sexes, lateral epicondylitis being more common than medial in both sexes. Medial epicondylitis is more common in the community than is generally recognized. Epicondylitis is a relapsing condition with a strong bias towards the 35-54 years age group. Analysis revealed no relationship between incidence and socioeconomic

Lateral epicondylitis. A review of structures associated with tennis elbow.

139 limbs from embalmed specimens were dissected to reveal the attachments of extensor muscles in the vicinity of the lateral epicondyle. M. extensor carpi radialis brevis was found to consist of a keel-shaped tendon with attachments to m. extensor carpi radialis longus, m. extensor digitorum communis, m. supinator; and to the radial collateral ligament, the orbicular ligament, the capsule of the elbow joint and the deep fascia. On 29 limbs, a prolongation of the muscle was identified attaching proximal to the lateral epicondyle. On nine specimens a bursa was evident between the capsule over t

Is therapeutic ultrasound effective in treating soft tissue lesions?

Of 76 patients with lateral epicondylitis, 38 were randomly allocated to receive ultrasound treatment and 38 placebo. All 76 were given 12 treatments each over four to six weeks. The conditions of 24 patients (63%) treated with ultrasound and 11 (29%) given placebo improved, the difference being significant at the 1% level. Improvement in particular clinical variables (pain score, weight lifting, grip strength) also showed an advantage for the patients given ultrasound treatment. A simple underwater radiation balance showed considerable fluctuation in ultrasonic output, and frequent checks of

[Thoracic outlet syndrome and its significance for surgery of the hand (on the etiology and pathogenesis of epicondylitis, tendovaginitis, median nerve compression and trophic disorders of the hand)].

At present, the term "thoracic outlet syndrome" (TOS) is used as a term to include all factors compressing the nerves and vessels situated in the outlet of the thorax and the costo-clavicular area. It is marked by neurological and vascular disorders; their manifestation can be either spontaneous or posttraumatic. In case of surgical treatment the transaxillary approach (according to Roos) proved to be the best, and is indicated in therapy-resistant TOS, in certain cases of arterial complications and in outlet obstructions of the subclavian vein. Complete resection of the first rib and the most

Treatment for lateral epicondylitis: variables related to recovery.

The purpose of this study was to determine the effectiveness of a treatment regime utilised for lateral epicondylitis that is designed to expedite return to activity and to identify variables related to treatment outcomes. Patients treated in a sports medicine clinic between 1978 and 1980 received, by mail, a brief questionnaire investigating the variables of the injuries and level of success of the treatment. Specific techniques ensured confidentiality and validation of information. Through the analyses conducted, an inverse relationship in the total group between the level of success and the
