Beyond Procedural Universalism: Relational Ethics and the Limits of Global Public Health Codes.

BACKGROUND: Alcohol consumption during pregnancy is a significant public health problem and can result in a continuum of adverse outcomes to the fetus known as fetal alcohol spectrum disorders (FASD). Subjects with FASD show significant neurological deficits, ranging from microencephaly, neurobehavioral, and mental health problems to poor social adjustment and stress tolerance. Neurons are particularly sensitive to alcohol exposure. The neurotoxic action of alcohol, i.e., through ROS production, induces DNA damage and neuronal cell death by apoptosis. In addition, epigenetics, including DNA me

Treatment of irreducible second metatarsophalangeal joint dislocation by double percutaneous osteotomy: Haspell and distal metatarsal mini-invasive osteotomy.

Subluxation or dislocation of the second metatarsophalangeal joint may be commonly associated with crossover toe, metatarsalgia, and painful calluses. This retrospective study aims to evaluate the clinical and functional results in patients with irreducible second metatarsophalangeal joint dislocation treated by double percutaneous osteotomy in one step: Haspell's osteotomy and Distal Metatarsal Mini-Invasive Osteotomy. A total of 39 patients were included in this study. 31 patients had a simultaneous procedure on the first ray for hallux valgus correction. The American Orthopaedic Foot and An

Impact of different foot orthoses on gait biomechanics in individuals with chronic metatarsalgia.

BACKGROUND: Foot orthoses (FOs) are commonly prescribed to reduce pain and improve function in individuals with musculoskeletal disorders, including those with chronic metatarsalgia (CM). Reducing the mechanical overload under the metatarsal heads during locomotion is the central point of the treatment for CM. Medially wedged FOs (MWFOs) with a metatarsal pad could further reduce pressure loading under the metatarsal heads and modify foot and ankle biomechanics compared to standard FOs (SFOs). RESEARCH QUESTION: Do MWFOs further decrease the peak plantar pressure under the metatarsal heads in

Morphological Variations of Foot Lumbricals and Their Clinical Significance: A Cadaveric Study.

BACKGROUND: The lumbrical muscles of the foot are intrinsic musculature playing a critical role in maintaining toe alignment and facilitating smooth gait mechanics. Despite their importance, morphological variations remain understudied, which can impact clinical diagnostics and surgical outcomes. OBJECTIVE: This study aims to investigate the morphological variations of the foot lumbricals and their clinical significance using cadaveric dissection. MATERIALS AND METHODS: This study examined 150 lower limbs from 75 formalin-embalmed human cadavers (43 males and 32 females) over the period of 2

One-Stage Technique with Calcaneal Graft for the Treatment of Brachymetatarsia: A Case Report.

Brachymetatarsia is a rare congenital anomaly characterized by the shortening of one or more metatarsals, which can lead to functional impairment, pain, and aesthetic concerns. This case report describes a 17-year-old female patient with brachymetatarsia affecting the third and fourth metatarsals of the right foot, which was unresponsive to conservative treatment and caused persistent pain while standing. To address this condition, a single-stage surgical approach was performed using an autologous calcaneal bone graft to lengthen the affected metatarsals. Additionally, the second and fifth met

[Investigation and treatment of metatarsalgia due to avascular osteonecrosis of a sesamoid].

The Renander disease or avascular osteonecrosis of one (or two) sesamoid of the first metatarsal is a rare and poorly studied pathological entity. It is part of the differential diagnosis of metatarsalgia, a more common complaint in clinical practice. We report the case of a 14-year-old patient presenting with pain on the plantar area of the left first metatarsal, aggravated while fencing. Clinical examination and MRI suggested avascular osteonecrosis of the medial sesamoid, for which conservative treatment resolved the complaints. The presentation of this clinical case gives the opportunity t

Myofascial induction technique changes plantar pressures variables compared to a simulated laser. A randomized clinical trial.

BACKGROUND: The fascial system is made up of large number of mechanoreceptors and cells that regulate their tension, such as myofibroblasts, in addition to an extracellular matrix. The treatment of myofascial induction has shown in the static imprint in a quasi-experimental study comparing the effect before and after the technique, but there are no studies compared with a control with no real intervention. The objective of this research is to check the effects of the myofascial Induction technique, compared with a simulated Laser in plantar footprint variables in asymptomatic subjects. METHOD

Sesamoid correction achieved during the learning curve for Scarf-Akin osteotomy without lateral soft-tissue release: a single-centre prospective observational study.

INTRODUCTION: Scarf-Akin osteotomy (with or without lateral soft-tissue release (LSTR)) is commonly performed to treat hallux valgus (HV). An insufficient correction of sesamoids can be a risk factor for early recurrence of the condition. We set out to determine 1) the radiographic correction achieved after Scarf-Akin osteotomy performed without LSTR and 2) the degree of correction of sesamoids obtained during the learning curve of the technique. MATERIALS AND METHODS: In this prospective single-centre study, the first 25 feet (25 patients, mean age 55.2 years, 14 left) undergone Scarf-Akin o

Biplane Chevron Osteotomy in Hallux Valgus: Biomechanical Effects of Lateral and Plantar Displacement Under Simulated Standing Load.

BACKGROUND: Plantar displacement during chevron osteotomy may improve load distribution in hallux valgus, but biomechanical evidence is limited. This study evaluates how lateral and plantar displacement affect contact properties in the first metatarsophalangeal (MTP-1) and the first tarsometatarsal (TMT-1) joints under standing load. METHODS: Twelve cadaveric feet with hallux valgus and 4 normal specimens were tested under simulated standing load. Biplane osteotomy was performed with lateral displacement (2, 4, 6 mm) and plantar displacement (0, 2, 4 mm). Joint contact metrics were measured u

An overlooked cause of forefoot pain in athletes: Avascular necrosis of the lateral hallucial sesamoid.

Avascular necrosis (AVN) of the hallucial sesamoids is a rare and often overlooked source of forefoot pain, particularly in athletic populations. We present a case of a 39-year-old female athlete with a 1-year history of progressive pain localized to the plantar aspect of the left first metatarsophalangeal (MTP) joint. The pain had intensified over the previous 6 weeks, especially during weight-bearing activities. Notably, the patient had undergone surgical excision of the right lateral sesamoid 20 years earlier due to a similar diagnosis, which is visible on current postoperative radiographs.

The use of 3D-Printed cutting guide improves the accuracy of distal chevron osteotomy for hallux valgus surgery.

BackgroundDistal chevron osteotomy is a common procedure for hallux valgus. Alteration of the osteotomy axis and angle can result in excessive change in length of the first metatarsal and hence the increased risk of transfer metatarsalgia. However, many of the Chevron osteotomies were performed freehand without objective guidance.MethodTwo surgeons with different experience levels (Resident trainee, consultant) using two techniques (freehand or with cutting guide) to perform 15 distal chevron osteotomies on 3D-printed identical first metatarsal sawbone models.
