Development and Validation of the Brief Inventory of Treatment Expectations in Chronic Pain (BITEC).

BACKGROUND: Expectations shape therapeutic outcomes, yet their systematic assessment remains limited in clinical and research settings. To address this gap, we developed and validated the Brief Instrument for the Assessment of Treatment Expectations in Chronic Pain (BITEC) using Item Response Theory. METHODS: The study comprised four phases. (I) Twenty-one items were generated from expectation constructs and refined to 11 through a Delphi review (≥ 80% agreement). (II) Comprehensibility was tested in 30 women with fibromyalgia, and the scale was applied to 484 chronic pain patients; items (0-

Amygdala-centered mechanisms of pain amplification and chronification in fibromyalgia and migraine: Narrative review.

OBJECTIVE: This review aimed to examine the role of the amygdala as a central hub in the pathophysiology of chronic pain, with a specific focus on fibromyalgia and migraine. We synthesize evidence on how structural, functional, and neurochemical alterations within amygdala-centered circuits contribute to pain amplification, emotional dysregulation, and central sensitization, and discuss their implications for circuit-based therapeutic strategies. METHODS: A comprehensive, theory-driven literature synthesis was conducted, integrating findings from neuroimaging, neuromodulation, molecular neuro

Comorbid DSM-5 mental disorders and chronic pain: What kind of relationship? Findings from the P3 cross-sectional study.

PURPOSE: Mental disorders and chronic pain frequently co-occur. The prevalence of this comorbidity varies widely across investigations and is certainly overestimated using screening self-reports. The main aim of this cross-sectional research was to calculate prevalence rates of mental disorders in a chronic pain population during their first consultation in an Italian specialized pain clinic using the narrow diagnostic criteria of the DSM-5. Additionally, we examined the relationship between this psychiatric comorbidity and a broad range of socio-demographic and clinical parameters related to

Health self-efficacy and general health in fibromyalgia: hope, hopelessness, and role functioning as serial mediators.

Fibromyalgia involves symptoms of pain, sleep disruption, and fatigue, which can have a deleterious impact on mental and physical health, and role functioning. Health self-efficacy, the perceived ability to manage and cope with health problems, may mitigate risk for functional disability and improve health perceptions. As a contributor to goal-oriented motivation, self-efficacy may facilitate hope and reduce hopelessness, thereby promoting willingness and motivation to engage in functional activities, with consequent benefit to perceived health. Our sample of 416 persons with self-reported fib

EEG connectivity features associated with fibromyalgia revealed by machine learning.

INTRODUCTION: We present connectivity-based features associated with fibromyalgia, derived from raw EEG data at the sensor level. METHODS: These connectivity features were identified through a data-driven method, employing machine learning. We carried out some automatic, moderate pre-processing and extracted spectral connectivity features. Machine learning experiments then followed, employing feature importance analyses and feature selection techniques for building high-performing classification models; finally, based on robust cross-validation and test evaluation, we obtained the features as

The efficacy of acupuncture treatment for fibromyalgia syndrome: a systematic review and meta-analysis.

BACKGROUND: Fibromyalgia syndrome (FMS) is a common condition in rheumatology that affects patients' physical and mental health. Some studies have demonstrated the efficacy of acupuncture in alleviating pain in patients with FMS, but there is still insufficient evidence to support the improvement of pain and associated symptoms in FMS patients through acupuncture. Therefore, this study investigates whether acupuncture has therapeutic effects on patients with FMS. METHODS: We searched 8 databases to identify randomized controlled trials (RCTs) evaluating acupuncture interventions for FMS. We u

An internet-based cognitive-behavioral self-management intervention for patients with hand osteoarthritis or fibromyalgia - Two randomized controlled trials.

BACKGROUND: Rheumatic and musculoskeletal diseases have a high burden. We aimed to evaluate the effectiveness of a therapist-guided internet-based cognitive-behavioral therapy (iCBT) on pain coping and secondary physical, psychological, and disease impact outcomes in hand osteoarthritis and fibromyalgia. METHOD: Two single-center, parallel-group, superiority randomized controlled trials were performed. In one study, 70 adults with hand osteoarthritis visiting a Dutch hospital were randomized to care-as-usual or care-as-usual plus iCBT (each group n = 35; ClinicalTrials.gov: NCT05872633). In a

Cost-Effectiveness of Pregabalin, Duloxetine, and Milnacipran vs Amitriptyline for Moderate to Severe Fibromyalgia.

IMPORTANCE: Fibromyalgia (FM) is a chronic pain condition associated with substantial quality-of-life impairments and economic burden. Although multiple pharmacologic options are recommended in clinical guidelines, the relative cost-effectiveness of off-label amitriptyline compared with FDA-approved treatments remains poorly defined due to a scarcity of direct economic comparisons and the use of heterogenous outcome measures in the extant literature. OBJECTIVE: To evaluate the cost-effectiveness of pregabalin, duloxetine, and milnacipran compared with amitriptyline in adults with moderate to

Efficacy and mechanism of supervised adapted physical activity on quality of life for patients with fibromyalgia: single-center, prospective, randomised clinical and neuroimaging study.

Fibromyalgia (FM) is a chronic overlapping pain condition that impairs quality of life (QOL). Adapted physical activity (APA) is recommended as a key non-pharmacological intervention, though its implementation and underlying mechanisms remain limited. Fibromyactiv, a prospective, randomised, single-blind clinical and neuroimaging study, evaluated the efficacy and mechanisms of a structured, supervised APA program in FM. Seventy-nine adults were assigned to supervised APA (n = 39) or standard care (n = 40) and followed for 12 months. The intervention consisted of diversified, low-intensity, fra

Knowledge, perception and attitude toward fibromyalgia among physical therapists in the United Arab Emirates: A cross-sectional study.

BACKGROUND: Fibromyalgia (FM) is a chronic condition classified by widespread pain, fatigue, and associated symptoms. Patients with FM are frequently referred to physical therapists, whose knowledge of assessment criteria and management strategies is critical for timely recognition and effective care. Early diagnosis has been shown to improve outcomes, whereas delayed recognition often leads to prolonged suffering and increased healthcare costs. AIM: The objective of this study was to examine the knowledge, perceptions, and attitudes of physical therapists in the United Arab Emirates (UAE) wi

Ambient green light reduces knee pain in osteoarthritis patients.

BACKGROUND: Regular viewing of dim green light has been shown to reduce the pain associated with migraine, fibromyalgia, and post-surgery. AIM: The present study examined whether visual exposure to ambient green light could also alleviate osteoarthritis (OA) pain. METHODS: Nineteen patients diagnosed with moderate to severe knee OA pain were exposed to dim white light (6.57 ± 1.00 lux) for 1-2 hours per day for 10 weeks. Following a 2-week wash-out period, patients then received green light treatment (wavelength = 525 nm, 6.82 ± 0.78 lux) for 1-2 hours per day for a further 10 weeks. The pri

Linkage of Medicare insurance claims to police-reported motor vehicle crashes: advancing traffic safety research in older adult populations.

BACKGROUND: Motor vehicle crashes (MVCs) are a leading cause of injury among adults aged 65 years and older ("older adults"). As the number of older drivers grows, it is increasingly important to understand clinical factors associated with an increased risk of MVC. A major barrier, however, is the lack of data. To address this, we linked two large-scale administrative databases, the New Jersey Safety and Health Outcomes (NJ-SHO) Data Warehouse, which contains information on all police-reported crashes in New Jersey from 2004 to 2019, and Medicare Fee-for-Service (FFS) insurance claims, which c
