Outcomes of circular frame external fixation for tibia fracture in the elderly: comparison with a fracture-matched younger cohort.

INTRODUCTION: Circular frame fixation is a management method often used for complex fractures, or fractures with soft tissue compromise. Tibia fractures in older adults can be difficult to manage due to an association with osteoporosis, poor quality soft tissues, and a requirement for post-operative weight bearing to maintain function and independence. There is a paucity of evidence for the use of circular frames for tibia fractures in older adults. The available evidence suggests it is an effective treatment option, however, no study has compared treatment outcomes between older and younger p

Distal Ulna Hook Locking Plate Fixation for Unstable Distal Ulna Fractures: A Prospective Clinical Study.

BACKGROUND: Distal ulna fractures, particularly when displaced or unstable, play a critical role in the stability and function of the distal radioulnar joint. Inadequate treatment may result in persistent pain, instability, and impaired wrist function. Recent advances in angular stable fixation have led to the development of anatomically contoured distal ulna hook locking plates; however, clinical data regarding their outcomes remain limited. OBJECTIVE: To evaluate the radiological and functional outcomes of distal ulna fractures treated with open reduction and internal fixation using a dista

Second Metacarpal Cortical Index Correctly Predicts Osteoporosis.

BACKGROUND: Second metacarpal index (2MCI) is a quick and simple calculation obtained from hand or wrist radiographs that can be used to screen for osteoporosis and target high-risk patients. In this study, we investigate the relationship between the 2MCI and hip bone mineral density. MATERIALS AND METHODS: We conducted a retrospective review of 1,080 patients aged 18 to 100 years who received a dual-energy x-ray absorptiometry (DEXA) scan between January 1, 2004, and March 1, 2024, and had a hand or wrist radiograph within 1 year of their scan. Patients were excluded if the second metacarpal

Heat treatment-irrigation interaction governs the fatigue life of NiTi endodontic files: Insights from acoustic emission and photoelastic stress mapping.

This study investigates the combined effects of 96% ethanol irrigation and C-Wire heat treatment on the fatigue durability of NiTi endodontic files. One Curve® instruments were subjected to cyclic root canal instrumentation in PMMA blocks until fracture, with real-time acoustic emission monitoring and photoelastic visualization of isochromatic fringe patterns. Heat treatment (HT) increased the number of blocks until failure by a factor of 1.6 to 2.1 depending on irrigation status, while irrigation increased it by a factor of 2.4 to 3.3 depending on heat treatment status, consistent with a sign

Enhancing structural integrity of PMMA hip spacers for two-stage revision surgery through titanium reinforcement.

PMMA hip spacers used in two-stage revision surgery are prone to brittle fracture due to their low fracture toughness and the presence of defects that can initiate crack growth, motivating the need to assess the effectiveness of metallic reinforcement under quasi-static loading conditions. This study evaluates the fracture behavior of reinforced and unreinforced hip spacers using a validated explicit finite element model combined with a sub-modeling approach, in which a spacer embedded in a femoral cavity is simulated and a quarter-elliptical corner crack is introduced in the most highly stres

Mechanical properties and failure characteristics of geocells: A comparative experimental study.

The tensile performance of geocells depends on the mechanical responses of both cell walls and junctions. Three commercial geocells, liquation-welded polypropylene (LW), injection-molded polyester (IM), and ultrasonic-welded high-density polyethylene (UW), were examined through tensile testing, macroscopic observation, and scanning electron microscopy of cell-wall fracture surfaces. Five configurations were used: cell-wall tension, junction-inclusive tension, and junction seam, shear, and split tests. The cell-wall peak strengths of LW, IM, and UW were 143.03 ± 4.43, 185.61 ± 7.96, and 39.42 ±

The Role of Biological Age in Adult Spinal Deformity Surgery.

➢ Adult spinal deformity (ASD) outcomes are poorly predicted by chronological age alone; biological age markers, including frailty indices, sarcopenia measurements, and bone quality metrics, provide better risk assessment. ➢ Systemic frailty tools (modified Frailty Index, ASD-Frailty Index [ASD-FI], Risk Analysis Index) predict perioperative morbidity and discharge disposition more accurately than age-based models do. ➢ Musculoskeletal assessments, psoas and paraspinal muscle cross-sectional areas, fatty infiltration grading, and computed tomography-derived Hounsfield unit thresholds identify

External validation of the FRACTING, LEG-NUI, and NURD scores for predicting non-union after tibial intramedullary nailing.

BACKGROUND: Non-union remains a challenging complication of tibial diaphyseal fractures treated with intramedullary nailing. Several clinical prediction scores have been developed to identify patients at increased risk of impaired fracture healing. This study aimed to externally validate the FRACTING, LEG-NUI, and NURD scores for the prediction of non-union in an independent cohort of patients with tibial diaphyseal fractures. METHODS: We retrospectively reviewed consecutive adult patients with tibial diaphyseal fractures treated with reamed intramedullary nailing at a level I trauma center b

Whole genome sequencing identifies genetic variants associated with senescence-accelerated phenotype of OXYS rats.

Age is the primary risk factor for age-related diseases, the early onset of which manifests as premature aging, whereas delayed development forms the basis for healthy aging and longevity. Adverse genetic variants contribute to premature aging and its diverse manifestations, including increased multimorbidity. Identifying the genetic underpinnings of premature aging and developing preventive approaches are essential to extending the human healthspan, which requires fundamental research using animal models. In this study, we performed whole-genome sequencing (coverage depth >25×) of OXYS rats -

Denosumab Biosimilars Versus Denosumab in Postmenopausal Osteoporosis: Updated Systematic Review and Meta-Analysis of Randomized Trials.

CONTEXT: Denosumab is one of the first-line drugs for postmenopausal osteoporosis with high risk of fracture, but the cost limits its use. Denosumab biosimilars are a more affordable solution, though it is underutilized due to hesitation among clinicians and patients due to concerns for differences in efficacy and safety. METHODS: We conducted an updated meta-analysis of randomized controlled trials (RCTs) comparing efficacy and safety of denosumab biosimilars to denosumab. PubMed, Embase, Cochrane databases and Clinical Trials.gov were searched for studies comparing denosumab biosimilars to

Comorbidities and outcomes associated with young adult hip fracture: a systematic review and meta-analysis.

BACKGROUND: The prognosis of hip fracture in younger adults (YAs) aged 18-50 years is unestablished. Large studies that rigorously assess risk factors for hip fracture in YAs, their associated functional outcomes and mortality are lacking. While international guidelines recommend comprehensive bone health assessment and consideration of pharmacotherapy in YAs with major fragility fractures, rates of implementation of these guidelines are unknown. METHODS: We systematically reviewed the existing literature on YAs with hip fracture to identify (1) clinical risk factors for hip fracture, (2) fun

Natural polymer carboxymethyl cellulose based strong adhesive, self-healing, and flexible conductive organohydrogel for wearable strain sensing application.

Scientists are especially focused on conductive hydrogels, particularly regarding flexible sensors like strain and pressure sensors. The potential uses of these flexible materials encompass electricity storage, human motion tracking, interdisciplinary robotics, and artificial skin. In contrast, its elevated response time, diminished conductivity, substantial energy dissipation, limited stretchability, and extended strain sensitivity hinder its practical application. Achieving all of these characteristics inside a single hydrogel network is a challenging job. This study has led to the creation

Beyond antifracture efficacy: Osteoanabolic versus antiresorptive therapy for chronic pain after osteoporotic vertebral fractures. A systematic review and meta-analysis.

Osteoporotic vertebral fractures (OVFs) associated pain can last long, reducing health-related quality of life. Concurrent risk of new fractures warrants osteoporosis therapies, i.e., antiresorptive [bisphosphonates and the anti-RANKL monoclonal antibody (mAb) denosumab] or osteoanabolic [teriparatide, abaloparatide, and the anti-sclerostin mAb romosozumab] agents, the latter demonstrating greater antifracture efficacy, but unclear effect on post-fracture pain. The present systematic review and meta-analysis explore whether osteoanabolics exert better pain control compared to antiresorptives i

The "Sandwich" technique using a lateral locking plate and intramedullary reconstruction plate with bone cement for osteoporotic proximal humerus fractures: a case series.

INTRODUCTION: Although lateral locking plate fixation is widely used for displaced proximal humerus fractures, fixation failure remains a major concern in elderly osteoporotic patients, particularly when medial column defects are present. Poor cancellous bone quality reduces screw purchase, while medial comminution and metaphyseal bone voids weaken the ability of the proximal humerus to resist varus and axial loading. Existing augmentation methods address only part of this problem. Fibular strut allografts can provide intramedullary and medial support but may be limited by graft availability,

Osteoporosis causes Nephrolithiasis: A Bidirectional Two-sample Mendelian Randomization Study.

Background Epidemiological evidence suggests a strong association between osteoporosis (OP) and nephrolithiasis (NL), yet the causal direction of this relationship remains debated. This study aimed to elucidate the bidirectional causal relationship between OP and NL using a two-sample Mendelian randomization (MR) approach. Methods We utilized genome-wide association study (GWAS) summary statistics from Taiwan Biobank (TWB; osteoporosis: 14,507 cases/ 79,846 controls; nephrolithiasis: 6,318 cases/ 88,035 controls) and BioBank Japan (BBJ; osteoporosis: 9,794 cases/ 159,138 controls; nephrolithia

Causal fracture from biomarker to clinical performance: A critical reappraisal of the 2D:4D ratio in surgical skills and the developmental-stress-training interaction model.

The second-to-fourth digit ratio (2D:4D) is a sexually dimorphic morphological trait widely used as a retrospective biomarker of prenatal androgen exposure. This critical review reappraises its causal validity as a predictor of complex behavioral phenotypes, using surgical technical performance as a test case, and proposes a Developmental-Stress-Training (DST) interaction model. Each inferential link-prenatal androgen exposure to the 2D:4D phenotype, the phenotype to central motor and visuospatial development, and neurodevelopmental differences to surgical technical performance-was examined fo

Critical-sized bone defects: essential concepts for the higher orthopaedic examination.

PURPOSE: To provide a practical review of the assessment and management of critical-sized bone defects (CSDs) for candidates preparing for higher orthopaedic examinations. METHODS: This narrative review considers definitions and classification, clinical assessment, acute management, reconstructive techniques and salvage options. RESULTS: A critical-sized bone defect (CSD) is unlikely to heal without intervention because of its size and location, although no numerical threshold defines criticality. Assessment should consider skeletal stability, infection, vascularity, soft tissues, host facto
