Development of 1 month core-shell microspheres combination formulation containing teriparatide for osteoporosis treatment.

Teriparatide is a representative anabolic agent for the treatment of osteoporosis; however, its therapeutic efficacy relies on intermittent exposure, necessitating frequent administration and thereby limiting long-term patient compliance. To address this limitation, we developed a programmable pulsatile delivery platform based on uniform core-shell microspheres fabricated via a W1/O/W2 double-emulsion process combined with premix membrane emulsification (PME). Through precise regulation of polymer phase separation, teriparatide is preferentially localized within the microsphere core. Core-shel

Comparative longitudinal assessment of bone mineral density measured with radiofrequency echographic multi-spectrometry (REMS) and dual X-ray absorptiometry (DXA) in a cohort of peritoneal dialysis patients.

UNLABELLED: In a peritoneal dialysis longitudinal cohort, REMS and DXA showed similar bone density, but only DXA detected significant cortical bone loss. REMS tended to give lower bone mineral density values and did not correlate with DXA changes. REMS may complement DXA when DXA findings are difficult to interpret or when closer longitudinal monitoring is needed, but larger studies are needed to define its role. BACKGROUND: Dual X-ray absorptiometry (DXA) is the gold standard for bone mineral density (BMD) assessment in chronic kidney disease patients, but it can have limited accuracy and ra

Social Determinants of Health and Access to Orthopedic Care Among Seniors in Little Havana.

INTRODUCTION: Little Havana, Miami, Florida, is a predominantly Hispanic neighborhood with many adults and seniors aged 65 and older at risk for osteoarthritis, osteoporosis, and associated fractures. Despite this need, access to orthopedic care is limited by socioeconomic, linguistic, and transportation barriers. This study aims to descriptively characterize community-specific social determinants of health, hip fracture hospitalization rates, and local orthopedic service availability among seniors in Little Havana to identify potential gaps in service availability and inform future research a

Regional hip bone density patterns in femoral neck and intertrochanteric fractures: a single-center retrospective study of 500 older adults in midtown Shanghai.

BACKGROUND: Hip fragility fractures in older adults commonly present as femoral neck or intertrochanteric fractures, which may reflect different regional deficits in proximal femoral bone mineral density (BMD). This study assessed whether QCT-derived regional hip BMD patterns can distinguish femoral neck from intertrochanteric fragility fractures. METHODS: Retrospective analysis of 500 patients aged ≥65 years (2020–2024). QCT-derived T-scores for femoral neck (FN), trochanter (FT), intertrochanter (IT), and total hip (TH) were evaluated. Multivariable logistic regression (age, sex, FT T-score,

Influence of Local Bone Density on the Functional Outcome of Proximal Humerus Fractures Treated with Proximal Humerus Interlocking Osteo Synthesis Plate.

BACKGROUND: Proximal humerus fractures (PHFs) constitute a majority of the most common osteoporotic fractures. Osteoporosis can complicate the surgical fixation and healing of PHFs and should be evaluated preoperatively. Bone density measurements can affect the treatment methods and outcomes. This study aimed to investigate the effect of osteoporosis using bone mineralization indices assessed from plain radiographs on functional outcomes. METHODOLOGY: Thirty-five patients who presented to our tertiary care center with PHFs between 2018 and 2021 were retrospectively evaluated among patients wi

Osteoimmunology: Can We Prevent Bone Loss in Rheumatoid Arthritis with Modern Antirheumatic Drugs?

In rheumatoid arthritis (RA), local and systemic inflammation and immobility lead to both generalized and local bone loss and an increased risk of fractures. Osteo-immunology, the shared mechanisms and crosstalk between immunology and bone has revealed insight in the pathogenesis: the upregulated inflammatory cytokines (TNFa, IL-1 and IL6) and, apart from that, anticitrullinated proteins antibodies (ACPA) stimulate the osteoclastogenesis with upregulated RANKL leading to increased bone resorption, while recent data also show an inhibited bone formation. Initially with the TNF blocker inflixima

Delivery System Based on Akkermansia Muciniphila Loaded Salidroside for the Treatment of Osteoporosis in Zebrafish Model.

Glucocorticoid-induced osteoporosis (GIOP) is a frequent cost of dexamethasone (Dex) therapy; salidroside (SAL) shows promise but suffers from rapid clearance and poor oral bioavailability. We present an oral, living co-therapy in which viable Akkermansia muciniphila is vacuum-loaded with intracellular SAL and protected by a pH-responsive Eudragit L100 enteric coat (SAL@AKK@EL100). This construct effectively retains probiotic activity, shields the cargo through gastric transit, and shows prolonged intracellular retention with gradual release. In Dex-challenged zebrafish, SAL@AKK@EL100 reversed

Clinical outcomes of unilateral versus bilateral percutaneous vertebroplasty under local anaesthesia: a prospective randomised study.

Percutaneous vertebroplasty (PVP) is widely used to treat painful osteoporotic vertebral compression fractures (OVCFs), yet the comparative value of unilateral vs. bilateral access remains debated. The aim of this study was to compare unilateral and bilateral percutaneous vertebroplasty performed under local anaesthesia in patients with single-level OVCFs. This randomised study was conducted on 196 adults (mean age 71.7 ± 6.5 years) with single-level OVCFs treated under local anaesthesia and followed at baseline, discharge, and at the 6-month follow-up. Patients were allocated to unilateral or

Alzheimer's Disease Risk Factor APOE4 Exerts Dimorphic Effects on Female Bone.

Individuals with Alzheimer's disease (AD) are at an increased risk of bone fracture, while osteoporosis in women is one of the earliest predictors of AD. Yet the mechanisms linking cognitive decline and skeletal deterioration remain poorly defined. Proteomic analysis of cortical bone from aged 21-month-old mice revealed strong enrichment of neurodegeneration-associated proteins, including apolipoprotein E (Apoe) and amyloid precursor protein. Apoe localized specifically to osteocytes, with expression in aged female bone nearly twice that of young 4-month-old male bone. Because human APOE allel

Mettl3-Mediated m6A Methylation of Pdgfrb Regulates the Angiogenesis-Dependent Bone Formation.

CD31hiEMCNhi (type H) vessels orchestrate the bone metabolic microenvironment, yet the epigenetic control of their endothelial identity remains unclear. N6-methyladenosine (m6A), catalyzed by Mettl3, is essential for mRNA fate and emerging as a regulator of skeletal homeostasis. After isolating and validating type H bone microvascular endothelial cells (H-BMECs) from mouse femora, we used lentiviral shRNA and endothelial-specific Cdh5-Cre;Mettl3fl/fl mice to silence Mettl3 in vitro and in vivo. m6A-seq and RNA-seq pinpointed downstream targets; qPCR, Western blot, MeRIP-qPCR, RNA stability, mi

Rhotekin contributes to osteoblast differentiation and bone homeostasis by interacting with Chibby 1.

Rhotekin, an effector protein for Rho proteins, is implicated in tumorigenesis, yet its function in other biological processes remains largely unexplored. In the current study, we investigate the function of Rhotekin in osteoblast differentiation and bone homeostasis. Our data show that Rhotekin is expressed in bone and adipose tissue, with its levels increasing as mesenchymal progenitor cells differentiate toward both osteoblasts and adipocytes. When Rhotekin is overexpressed in progenitor cells, it facilitates osteogenic differentiation while inhibiting adipogenic differentiation, whereas kn

Carbohydrate polymer-based biomaterials for light-triggered Photo-Immunotherapy: Design strategies, immune mechanisms, and translational outlook.

Recent advances in biomaterials have highlighted the immense potential of carbohydrate polymers (CHPs) as biocompatible, multifunctional platforms for cancer therapy. Indeed, the structural diversity, tunable chemistry, and inherent biological affinity of these polymers enable the precise design of systems for immune modulation. Moreover, light- and laser-driven strategies have emerged as powerful tools for achieving spatiotemporal control over therapeutic activation. Therefore, integrating CHPs with photothermal or photodynamic nanocomponents enables these hybrid systems to induce localized t

Spatiotemporal patterns of femoral fractures in older adults: healthcare access and regional inequalities in southern Brazil.

BACKGROUND: Femoral fractures in older adults are associated with lethality rates of up to 30% within the first year and significantly compromised quality of life, leading to high levels of disability, institutionalization, and burden on health systems. By 2050, the global population of older adults is projected to exceed 2 billion, leading to an exponential increase in these events. In Brazil, the incidence of fractures is high and often linked to inequalities in access to diagnosis and prevention, particularly in regions with limited infrastructure. This study aimed to analyze the spatiotemp

Response to Letter to the Editor for "Femoral Bone Mineral Density Shows Stronger Correlation With Pedicle Screw Insertional Torque than Lumbar Bone Mineral Density or Hounsfield Units: A Retrospective in Vivo Study".

We thank the authors for their insightful comments regarding our study evaluating the relationship between bone mineral density (BMD), Hounsfield units (HU), and pedicle screw insertional torque. While trajectory-specific CT-based assessments represent an important and evolving approach to evaluating local bone characteristics, existing literature has generally demonstrated only moderate correlations between DEXA or conventional HU measurements and insertional torque. In our study, we extended prior work by evaluating both systemic and vertebra-specific metrics, including femoral and lumbar DEX

Deciphering the interplay between serum urate and bone mineral density: insights from observational and genetic perspectives.

BACKGROUND: Urate commonly influences bone metabolism due to its antioxidant properties, yet this relationship remains inadequately understood. Our study aims to investigate both the phenotypic and genetic relationships between urate and heel estimated bone mineral density (eBMD), a proxy marker for osteoporosis. METHODS: Leveraging UK Biobank individual-level data and summary statistics from the hitherto largest genome-wide association studies, we evaluated phenotypic associations using linear regression and restricted cubic splines (N = 424,105), then explored genetic correlations, causalit

Unlocking bone repair in osteoporosis by targeting the angiogenic niche.

Osteoporosis (OP) and its related fragility fractures represent a significant global health burden, primarily characterized by reduced bone mass, deteriorated microarchitecture, and compromised self-repair capacity. While current mainstream therapies focusing on anti-resorption and anabolic pathways can slow bone loss, they often fall short in reversing established skeletal deficits and, particularly, in facilitating fracture healing. Growing evidence underscores that angiogenesis is fundamentally coupled with osteogenesis, forming the cornerstone of bone homeostasis and injury repair. However

Hand bone densitometry associates with nailfold capillaroscopic severity of microvascular damage in systemic sclerosis: a pilot study.

OBJECTIVES: Patients with systemic sclerosis (SSc) exhibit systemic and more pronounced micro- and macro-architectural bone damage compared to healthy subjects (HS). The extent of the microvascular damage in SSc may be associated with the severity of compromised systemic bone integrity. The aim of this study is to investigate and score the status of the hand bone mineral density (BMD) in patients with SSc, using a new dedicated hand software and to score the microvascular status of the same hands by using nailfold videocapillaroscopy (NVC). METHODS: Bone mineral density (BMD/g/cm2) and bone m
